I tested on retail, that fall is 96% of your hp. It is not lethal. He could have been healed before. Or, in fact, if he dinged and was full hp, he would have survived and it would have been the most epic save in the universe. Also, backpedaling is a way to get less horizontal speed, thus landing on the wider part of the bridge, but it was still too short for that.

Not force a poorly planned hard as hell fight that the entire stream advised against it. You could argue "It was his choice" which is true, but the temptations of being on a popular stream altered his judgment.

If you watch ozy POV he had the jump, he was straffing to the right and then for whatever reason, he stopped at the edge, and back peddled off. If he kept straffing off the edge he lands the jump.

I think I've only ever done Urok Doomhowl once and that was in SoM. It was rough but a successful first try. 5man though, not as a 10-man raid. I'm not sure we knew to click on the pike during combat back then, though - maybe someone in the group did and handled it, but it wasn't within my field of view. We basically had full tier 0 and some had progressed into the tier 0,5 questline or had one or two drops from MC.
