Awesome! If the doctor gets all 21 guidelines correct, the implant material doesn't carry as large of a potential risk. However, the real problems arise with a replica implant when you don't control for all of the other risk factors, as most fatigue failures are time and force-dependent. Many complications from lower-quality materials and implant designs arise only years after restoration!

In engineering, the standard for minimum thread engagement of a bolt is for the bolt to fail in tension before the thread interface fails in shear. You have to account for the ratio of strength of the two materials (implant and bone in this case), bolt diameter, and thread pitch. 5 threads is not some standard. For steel in steel threaded connections there's a common general rule of thumb of 1-1.5x bolt diameter for engagement.

This is true. So for a 5mm diameter implant with a thread pitch of 1 thread/1mm of implant length the answer is 5 threads;-) I though it best not to complex the guideline with where it comes from and focus on the premise of predicting primary stability in fresh extraction sites. When we teach the Five Thread Guideline we cover bone quality, quantity, implant diameter, implant pitch etc to ensure the doctor has the best chance to provide the patient with ideal low risk solutions. If you are a dentist I am impressed:-)

I primarily use autogeonous graft, but also use a cortical/cancellous mix and occasionally xenograft in my practice. We'll add advantages and disadvantages to our filming list!

Ok that’s nice but I have 2 questions 1 the screw and bolt aren’t tapered like most dental implants so is there a difference here? 2 does this apply also to immediate loading too ?

1. You're absolutely right that the screw and bolt are parallel-walled, but for the purposes of thread load distribution, the principle works exactly the same. 2. The Five Thread Rule was designed for predicting dental implant stability in fresh extraction sockets, but it can also be applied to sub antral cases as well. Thanks for the fantastic questions!

If they're planned and placed correctly, and if you use the right implants (and the patient isn't a smoker), they should last the rest of the patient's life without complications!
