How refreshing to hear a surgeon recommend treatment before surgery! I know this is because you truly care about your patients' well-being. When a doctor recommends lifestyle changes, they have to trust the patient to follow through. When a doctor performs a surgery, he or she is in control of the treatment. Lack of compliance is such a huge issue for doctors, since they are the ones who will bear the responsibility of the outcome. Pressed for time, many doctors will resort to the options with the most compliance: drugs and surgery. I don't blame them, but I wish they would consider counseling their patients thoroughly about the benefits and risks, and about the benefits of lifestyle changes. Thank you for being in the minority! I hope that in the future many doctors will use this channel to educate their patients on these important but difficult topics.

I appreciate your time and thought to write the comment! I think many people perceive doctors being "impatient" and wanting a "quick fix" because there isn't enough time spent discussing the topic. The patient feels they are being rushed and ignored which destroys the trust in the treatment plan. I will often recommend surgery first for some patients but ONLY after discussing every detail so the patient feels their involvement and ownership of THEIR body.

I suffered for 2 years with chronic anal fissures, internal hemorrhoids and significant pelvic floor dysfunction. My bottom end blead almost daily for this entire time. I had the surgery to remove the fissures, the sphincterotomy, and the internal hemorrhoids. It was an awful process to heal and ended back in the hospital five days later with an infection. I did not feel that I had a choice but if you can avoid this set of procedures I would recommend. In case you were wondering I did complete pelvic floor pt and still do the exercises daily. Since the procedure I have been diagnosed with slow transit motility and chronic idiopathic constipation so I will end up being one of the 6 people who will have a recurring problems. I am on a very strong set of very expensive meds to work on improving motility and improving peristaltic in my large intestine

I did the surgery 1 month ago and let me tell you. The second day you are forced to have a bowel movement. It’s scary but with time it gets better. So far so good healing well. No stitches 20 minutes surgery. Spinal anesthesia done no pain at all. Eat well don’t get yourself to this stage.

Thank Dr. Chung for the amazing video. I’ve had this surgery about 10 days ago. 3 days after the surgery I wish I had the patience and waited for it to heal by it self instead of the suffering I had to experience before the surgery. However, 10 days after the surgery, I’m glad to say that I made the right decision. I’m recovering faster than I anticipated and thanks to my surgeon, his main and only advice was to clean the cut with iodene every 2-3 hours no matter what. It’s a terrible experience, extremely painful, and at some point you’d hesitate to even touch the cut with a feather, but I had to do it and he was perfectly right. As for the uncontrolled gas and stool passing, Yes you’re right and at some point I thought I’d regret this decision. But I assure anyone who’s considering to go for this surgery, it’s a matter of few days until it you get control of this. One last advice: eat lots of lettuce and cucumber, stay away from wheat, and take the softner every single night during the first 2 weeks. It will make your life easier and get you recovered within days. All the best.

with all due respect Dr. Chung, I've had this horrible ailment for about 4 months now. I've watched all your videos, I've adjusted diet, got stools soft. Used the nitro (which definitely helped and got me the most time between retears) but in the end I retear every 3-4 weeks. The last couple of tears happened with no pain and no reason period for it to open back up but it did. Once this thing goes chronic it's almost impossible to heal without surgery. I've read countless accounts of people going through the same things I did and the surgery was the only fix for them. I'm there now, next week I go for surgery and I'm counting down the days. I want my life back.

I'm sorry but I've tried prescribed ointments, OTC creams...they made my pain worse! I've been drinking 64 to 80 ozs of water, drinking Metamucil, eating 6 servings a day of fruit and vegetables...I swim, walk and have an active social life. The pain in my bottom is extreme. I am hoping the surgery I've opted for will help after my year long struggles with my daily bowel movements! Love your videos. Feel free to comment. 😊

I’ve been suffering for 4 months of chronic fissure. Did 2 months of nifedipine ointment multiple times a day, miralax, stool softener, fiber and half gallon to gallon of water a day with minimal results. I have minor bowel incontinence and can barely move around and have to sit on 4 inches of foam pad everywhere i go. I have surgery scheduled soon for LIS or botox injections hoping i can get back to normal.

@@jeremydawkins8399 6 weeks post op, turned out i had a fistula along with fissure. Had fistulotomy and fissurectomy and started nifedipine ointment 2 weeks post. I feel back to normal no pain whatsoever today.

@@D_Downs Thanks for the reply. Did you do botox or LIS? I am in the same situation as you and trying to decide which route to go. My doctor says just do the LIS as it's a higher success rate than botox but he is say the LIS recovery is just 3 to 4 days and then i should be fine with no pain. From what i have read that is not the case. Either way i have to do something i am in pain for 10 hours after a BM.

Hellos Dr! Great video 👍.. I’ve been suffering from an anal fissure for a few months now and it’s brutal pain . I went to see my colon doctor and was given an ointment to apply 3 times a day . I’ve been doing so and also taking new diet plans plus fitz bath but it seems my anal fissure is not healing . Just saw my doctor again and they’re now recommending I go through with either Botox injections or surgery . Wow it’s been 2 years since this occurred to me still can’t believe that time of my life.. the worst, I pray for your bettering ❤️🩹 Just try to keep calm & know that you’re not alone in this. So for me personally (& I’m speaking on my own behalf from my own experience, I’m no DR.) I had to make a great change at first forsure. I went ahead and avoided anything with nuts or really hard foods that I thought would make my poop too hard so I basically “softened” my diet. I also stayed off dairy 🥛 It was not doing my digestion well plus the gas was making my fissure hurt more. I religiously took warm Epsom salt baths. Like every morning and night, would aim for twice a day & just stay soaked up for about 20 mins. Now honestly what I praise my fixing for & at first thought that’s guys were scammers, was a product called Pranicura! If you google it you can find it, I believe it’s sold from overseas but they ship internationally. It’s a pink ointment that I would lather on about 3 times a day & man that was very magnificent for me. I had to buy the bottle like 3 times honestly & kept taking warm baths. Kept my diet clean & wow before you know it I got better somehow. I now no longer suffer from a fissure, it re opened once but hasn’t happened since. I still watch my diet, no longer use the cream, here and there take warm baths. But I did learn to control my stress cause my doctor said the more you anger, the more your causing your anal muscle to contrast & that’s no bueno for a fissure. I never ended up getting any surgery or Botox injections as first recommended to me. I had to endure so many painful nights just praying & keeping positive, reading comments, interacting with people who’ve dealt with it & just reading up on how to soften my stool. I seriously would suggest that cream to anyone in suffering, PRANICURA. & like I said just avoiding like oats, anything toasted, eating lots lots of fruit & drinking about a gallon of water a day. @@jorgefarias8749 Glad that's working for you but I'm not living my life avoiding all the things I like. From what I hear, after the surgery your life goes back to normal. Even hard poops won't reopen the fissure because of the lack of pressure. I think some of us get this because our sphincter muscle is just naturally tighter. I have IBS too so it couldn't have helped. But over the last couple of months I've gotten my diet pretty squared away. No hard poops but it always reopens right around 3-4 weeks like clockwork. I'm just tired of it.

Hello Dr Chung, my CRS did skin tags removal and fissurectomy instead of LIS as he mentioned he didn't feel my sphincter was hypertrophic so he didn't want to cut it. I am 7th day post surgery and every bowel movement burns for good 4-5 hours and I have a habit of going 2 times a day. Is fissurectomy as successful as LIS in my case too?

@@likhitha6038 It took me 1 year to completely recover. I would lose hope everyday but somehow kept at it. My fissure still reopens after a hard bowel but it isnt as bad as it used to be. I just recovered again from a reopening but I can feel the area is still sensitive. I try to not eat a lot of processed food like all-purpose flour food etc and I remain ok.

How I wish you were in Florida 😩 I’m trying to figure out if pelvic floor dysfunction is contributing factor to tight sphincter muscles 🤔 - Is it, Dr Chung?? Are there any exercises to relax that area? I finally had some relief after months and months of hell on steroids but it was short lived and opened up again. I’m trying so hard to eat a soft food diet and have kept up With softeners (probably will have to my whole life)- I lapsed with trying almonds and whole flaxseeds instead of ground flaxseed. Did that aggravate things? Absolutely cannot go through a colonoscopy because I couldn’t handle the prep. Too nervous about LIS - Lyme disease = very slow and impaired healing. I’m at my wits end (I even did an Exosomes iv at the height of the discomfort but no luck). I really don’t know what is next. My CRS was pressing surgery 4 -5 months ago. All of this from covid severe dehydration. Is there such a thing as too much fiber? I’m doing all the right things. I don’t know how to get past this. What are your thoughts on quaterization? CRS says it’s very painful. Any feedback would be greatly welcomed. 🙏🏼

Dear Doctor , i have been suffering from anal fissure for over 30 years ! with multiple tags .. lived all my life managing it with extreme care.. have ibs .. take probiotics had occasional flare ups .. always been very very careful about diet and avoiding constipation ... 1.5 years ago got sick severe cold .. bed ridden.. 1 super bad SEVERE constipation immediately had SUPER SUPER super bad flare up that does not end until today with extreme pain (and allll other symptoms u know) knowing that ihave done everything you recommend before and after i watched U ... Doctor my fissure was almost healed. I had no pain since September 20th. 2 days ago i decided to stop taking laxative because pain was almost gone. When i used to take laxative poop was soft and i could see traces of water so it looked soaked on water ( not like Diarrhoea ) . But since yesterday i think my poop is not watery so when i try to poop i feel like my muscles go up and i have to strain in order to poop. I tried my best not to strain even a little bit but poop was just out a bit. It wasn't hard poop it was just not coming out. Maybe i dont know correct way to poop ? Or i have anal stenosis ? Or its just my sphincter muscles acting up ?

I had a fissure repaired some 40 years ago only to be left with permanent spasm. All these years I have struggled trying to have diorea with fybogel just to go.I have recently been prescribed Rectogesic twice per day applied externally not inside??? Why externally??I’m 75 I am getting some small but significant results in relaxation but is it ok for me to stay on this permanently? My doctor is only a GP general practitioner. @@yourfriendlyproctologist hi I had a hemmroidoctemy and lis surgery 7 months ago .. I absolutely regret the lis surgery it has left me with horrible fecal leakage and incontinence... I have my diet well done get my grams of fiber both fibers.. drink gallon of water daily use stool softners and also take a bulkinh agent ...nothing has helped me !! I'm a 40yr old no kids amd have been left this way .. I'm very depressed and I'm so upset at my surgern that litteraly said the percentage is so low that no I will not have this problem yet the more time passes the worse its getting plus now have a skin tag. I have severe pelvic floor dysfunction and so I did pelvic floor therapy with a specialist for 2 months and it didn't help me at all if anything it's worse..I even did all my pelvic floor too exercises at home too 😢 😞 I don't know what kind of doctor can help me with this ?

There are still 1.5 hours left for the operation (surgery). Now my operation is in just few mins . I am very worried about it. Actually I am a very strong man, but when it comes to tearing the ass, everyone gets scared. The doctor has said that you will be able to walk after 2 days and will be able to do daily chores after 7 days, but as per the online comments and some other people's advice, I will have to suffer a lot for a month. Whatever happens, let's see what happens. I have been suffering from fissure and piles for 7 years and am ready to bear more pain to get rid of this pain. I have some fears and questions in my mind like 1) Will the pain of the operation be bearable. 2) Will I get rid of fissures and piles after the surgery. 3) Will I be able to share my experience with you for a few days after the operation 🥲.

@@NovelbrocolliMy surgery experience was very good. I had this problem for 7 years, I used all kinds of Ayurvedic medicines, I consulted the best doctors, I also took their medicines. (But now it has been more than 50 days since the surgery, after 14 days I am doing my daily work and lifestyle, surgery was one of the best decisions of my life, now I think that I should have done the surgery much earlier, now I am free from the hassle of pain 👍🙏🫡

Yes, I am absolutely fine, I had the surgery on 26-07-2024, after 7 days I started most of my light work. And after 14 days I started all kinds of work, like desk work, long driving. Definitely, the decision of surgery was the best decision of my life.@@saadiarafique1761

Dr Chung your content is the best on the internet. Please just pray tell me I will be normal soon? I am 10 days post op laser surgery for fissure and grade 2 hemms, one out and one inside. Yet I am stuck to bed. Bms are painful and the swelling is driving me mad. Will I live rest of my life in toilet and planning my day around my bms and sits baths. Will laxatives be the most important food in my life. This is a compromise. Please please answer. Help !

I’ve been suffering from an anal fissure hurting me so much , depressingly suicidal pain at this point , my doctor tried treatments but is now suggesting we move forward with surgery. I’m sure post op hurts and I heard it takes a while to heal . But isn’t it best to be hurting from something healing rather than allowing it everyday to cause pain from just being there ??

Can you recommend anything to wear under a swimsuit for leakage? I love to swim but haven't been able to because I'm afraid of a stain on my swimsuit. Liners swell up when in water

Good question! I would shower in the gym before putting on your suit. Then immediately wash your suit with soap at the gym. I do not believe the stain will be permanent.

No incontinence, I feel that I can hold it, but the intermittent bleeding and moderate pain are bothering me. The nurse says it is normal to have some bleeding. Do you have any recommendations to stop the bleeding? Is it part of the process? Regards.

Hi Doctor Chung, I am having this anal fissure issue. I have had the Botox shots about 4 weeks ago. On 1/2 of the area the burning and stinging has stopped, but the area where the actual fissure is, is still very painful 😢 I have asked my doctor for pain meds but he will not give them to me. I sooth myself with a dried wet towel between my butt cheeks. This seems to help sometimes, but im told the area should be dry. I can't possibly do that without pain meds. Am I hindering my healing because of the dried wet towel? I also have to said I am glad that I have stumbled upon your videos. You have calmed my nerves, something that my own doctor has not taken the time to do and I have learned somethings as well. I want to say thank you for your patience 🙏and I wish I was nearby.

I had hemorrhoidectomy and sphincterotomy 3days ago i am recovering now almost there's no pain comparing to the 1st day of my surgery and im having antipain and i can say i am doing well. I am having stool softers and Benefibers and sit baths 3times a day and a cream finally covered with a gauze, BUT the thing is on the second day after my surgery i was trying to take a walk around my room max 3min as the doctor prescribed. suddenly, i felt my stool going down through my leg all over the floor. what does that means??? i am afraid I'm having bowel incontinence. Please help !!

@SHS - Mini Vlogs the day I wrote that comment I had an emotional breakdown followed by a anxiety attack… i went to see the doctor next day with my family and he assured me that this will stop gradually, but I have to stop overthinking as that might disturb my healing… I tried really hard not to think about it for next few hours and next day I started to feel better… leakage gradually started to reduce and the wound also started to heal…a couple of minutes ago I took sitz bath and I always check if there is some poop before I dip my bum in tub… there was some leak and lil poop but it is 1/10 comparing to what I had at the time of posting of above comment. This Saturday I will complete my 3 week post op and now I am really hopeful that I will recover completely In next 2-3 weeks. I hope this helps.. In will share an update in next two weeks about the leak issue…

@@محمدالسنوسي-ر9ث hey…it’s been almost 1.5 month post op… things are a lot better I am still doing sitz bath and applying ointment twice a day…. There are not major fecal leaks at all for last 2 weeks… just some yellow liquid that comes out once or twice in a day or two…. I feel different when I poop … but I guess it is normal….there is no fissure, no blood and no pain now… just weird feeling as it not what it used to be… not sure how many more days I will have to continue sitz bath but I am planning to visit the doctor after 8 weeks post op for the check up and further guidelines….

I’m 7 days out from fissure surgery. No joke, this was painful. ( I’ve had a mastectomy and later reconstruction with expander …. Sucks ). I only took two prescribed pain pills and the switch to Tylenol. Once I passed my initial bm, having some incontinence …… hoping this is temporary until I’m healed completely.
