Best system in the world is the Royal Flying Doctor Service in Australia. RFDS has Pilatus PC-24's and PC12's, King Air B350's and EC145's. Not only does the RFDS do medical retrievals, but they also fly into to remote communities with doctors, nurses, dentists and their assistants and will stay for a couple of days performing clinical work so that the residents don't have to make their way to a capital city. Most of the large cattle stations are in excess of 1 million acres and employ up to 200 Jackaroo's and Jillaroo's, (cowboys and cowgirls). For many in outback Australia it is often 1 to 2 thousand miles to the nearest big hospital.

I got a couple questions that some people here might have the answer to. Which companies or cities, do the most scene flights? I have heard places doing 10 and other 30%. As a flight medic, are there companies you should stare away from, are there companies out there that is better to work for than others? Which companies have decent pay?

I think the government air ambulances are one of the best systems because they offer a wider range of services than other privately operated air ambulances. For example, The California State Highway Patrol AS350's offers law enforcement, search and rescue, and medevac services to many surrounding communities around California. Most of the government-run air ambulances tend to be less costly than that of private air ambulances.

Air Evac still has an office in West Plains, with a couple 206’s as yard ornaments out front. They were started about a year after St Johns in Springfield got their first helo by a local philanthropist gift. They have 3 135s at Ft Smith, Springdale, and St Robert MO. Cox in Springfield flies rare 902s out of the main hospital and in Bolivar. Having 2 Level 1s in Springfield means we get traffic from up to 3 hours away by car regularly.

For cardiac arrests, the golden hour us only ten.minutes long. Death rate increases by 10% per minute For ischemic stroke victims, the golden hour is less than four hours long. Clotbusters are only approved for use in that four hour window, the faster the better. Sorry, I don't carry around sources. But speed is documented to count, in many situations.

Another great video! Unfortunately the capitalism sometimes leads to wrong decisions resulting in bad accidents. I prefer the German system with a handfukl of operators running one of the best HEMS-system in the world, because it is financed by health insurance companies as well as the cities and their surroundings. It is not possible to popup a HEMS-base without official permissions and agreements. Learning from the past, they don`t fly if the weather-conditions do not allow it. They simply don`t have the financial pressure to do risky things.

I always felt that the pilot was possibly unfairly blamed. What if Bryant demanded that this pilot fly through dangerous weather? Who can say no to a celebrity? What if he said, "Look, this is dangerous..." and the "star" said, "I don't care! Get me to the place I need to go to!"

What was missing from the communication from the tower to the black hawk was the actual coordinates of the crj flight ! Which led to the assumption because of so much traffic ,they had visual ,but of the wrong jet ! Then the tower commences to clear a departing flight off of runway 1 in the direct path as the crj and black hawk....never mind that runway was assigned initially to the crj .....and the TIME it took for that diversion "was really the difference in life and death here ! Sadly it was a domino effect ! Incompetence

Good point but I bet they have this KED on this ambulance due to state regulations - not because they plan to immobilize some to it.

I am not so sure about the upset after AP disconnect, nor the high workload. If SAS disconnects, you simply hand fly the aircraft. That shouldn’t take more than half a second even if the pilot is doing something else with their hands at the time that is happening. It’s not that the pilot could be out of their seat when this happens. Or that - what you describe as high workload - was something that is any difficult for any rated pilot to perform.

ATC told PAT 25 that the aircraft would be landing on 33. PAT25 acknowledge. No one it stating this FACT. PAT25 should have been looking to their left. Seems to have been lost by the media. And now everyone just keeps saying that PAT25 was looking at the wrong plane. The other planes were lined up for runway 01. I am a civilian and can figure this much out. That Helo missed many, many things that night, except the aircraft that was landing.

I accept your articulate explanation. But the purpose of NVG, is to see more correctly and navigate. Kindly accept that the PAT 25 crew was negligent in the first place and the ATC was second to blame, believing someone, who like repetition always say “ Visual separation requested “ without even knowing what it means sometimes, because it’s a repetition. May all the Souls Rest in Peace.

What you state makes sense you never mentioned the near misses reported at that airport over the years. Also the fact the night b4 ntsb reported the same type airplane had to go round due to a chopper in its way. The systemic imo complacency that was present regarding near misses. And the fact that ivfeel ATC needed to take control of that flight and immediately use headings to get the chopper far away from a landing plane. Too much deference given to military. And remove those ridiculous routes !

This is not updated- the TCAS on the DCA worked and sounded traffic The strange think in yesterday’s press conference is that the ATC screen indicated that the helicopter was at 200 while the plane based on the black box was at 325 up or down 25 They are waiting on the helicopter black box analysis to confirm the altitude of the black hawk I am trying to put those two things together, as the ATC numbers are transmitted from the black hawk and the black hawk hit the plane, there might be some kind of malfunction which provided the black hawk with a false reading which was transmitted to the tower and shown on their screen while in reality they were higher. This is just a maybe and that is why everyone needs to hold laying the blame on any party till they investigate

The AoA is from the plane being inverted and full stick back. Pilot lost his spatial hitting the cloud while turning and rolled it over, then panicked, to tried pull up -- but that was really down into the ground. If he had enough altitude he would have looped.

In my (admittedly not humble) opinion: helicopters (and GA airplanes) shouldn’t be crossing under the approach path when there is a commercial flight on final! Until I watched the simulation by @SnakPakFlight I was misunderstanding the geometries involved. I initially thought the 200' ceiling was published to keep helicopters below any airplane on final, but it wouldn't, not at that point on final approach. Yes, the helicopter crew accepted visual separation. Yes, the helicopter crew was high [probably]. But the runway extended centerline is supposed to be protected airspace just like the runway surface is. Vertical, Horizontal, and Time separation should all be in place. I’m now thinking this is a poorly designed terminal area procedure. (The helicopter route, instead of having a ceiling of 200’, should have a holding position north of the extended runway centerline. &/o The helicopter route could have a floor such that the helicopters pass over airplanes on final. (See route 6.) But then it is even more critical to separate horizontally because you need clear airspace above the final approach for when a commercial flight does a go around. Routes 1 & 4 should probably have a ceiling of 300', don't let the helicopter cross the final approach if you cannot sequence them BETWEEN the commercial flights. Should the helicopter crew have requested & accepted visual separation? Almost certainly not. Should the helicopter crew have slowed down instead of continuing at cruise speed? Almost certainly. (Former army airfield commander, airport manager in civilian terms. My most recent flight as a UH60 PI was in 2006 and I have never flown in any airspace as congested as the MDW. I was never trained/qualified on the Seoul &/o DMZ routes although at one time I had significant staff responsibility for US Army use of that international airspace. My experience is dated, but relevant, I think.)

It was an accident waiting to happen. Running training drills thru a commercial landing flight path day after day 🙄 Just look at the map with obvious crossing paths of known routes should have been changed long time ago or restricted during airport hours.
