It was planned to launch on February 21, 1967, as the first low Earth orbital test of the Apollo command and service module. The reason why it started as a fire and grew out of control so fast and seemed to engulf him almost immediately, was because they pumped the cockpit with 100% oxygen from a prior malfunction. That oxygen ignited.

This is referenced in Apollo 13.
