Dr, once the Z and Z is effective, what happens when a person is weaned off the medication? Or what steps must one do when getting off Z & Z when it's effective? Also, any timeline you can suggest for the Z & Z to work? Thanks
@jackmutnick136318 күн бұрын
@lokeyfunny -- I recommend that you continue the Z/Z therapy until you are hive-free for 3 months. Then I suggest that once per week you decrease one of the tablets (I suggest stopping the Zantac first) until you are able to discontinue both medications. If you flare during the taper, return to the last dosing that was effective and continue x 2 weeks before attempting to taper again. The goal is hopefully the trigger for the hives will burn out since the histamine is being blocked, but if not, as some people do struggle with Chronic Spontaneous Urticaria, you at least have the Z/Z therapy in your back pocket if the hives return. Hope this helps. DrJackAllergy

What can I do ? Thank you

Thelma, without seeing you in person and reviewing your entire medical history I cannot provide any significant answers. It sounds like you have been seeing the proper specialists and I wish there is/was more they could do. Please do not lose faith as there is always alternatives. Maybe discuss other medications in the same class as hydroxychloroquine with your allergist. Wishing you well and praying you find some relief. DrJackAllergy

Thankyou for being so straightforward! I am suffering from chronic urticaria for about more than 4 years now! When the hives breakout, they take their time to go away! And its really really depressing and difficult to sleep at night. I am definitely going to try your Z & Z therapy. Although, I have tried Cetirizine before and it does not help me with my itching neither the welts. But i am definitely going to try Z & Z together. Thankyou!

If you have not tried my two medications twice daily therapy then please give it a go. If you continue to have hives, consider increasing to one of each tablet three times daily to reduce the hives and then decreasing as you are able. Since this is the 2nd time and for long periods of time you really should have a more extensive laboratory work up to see if there are triggers leading to your chronic spontaneous urticaria (CSU). Hope this helps. DrJackAllergy

Sorry to hear of this issue. Oftentimes hives just flare for no particular reason. If they did start in response to the Hep B vaccine (though not likely if they did not develop immediately after the Hep B vaccine), then you would need to discuss with your doctor. That said, hives tend to get much worse in the evening hours AND after hot bath/shower. We recommend bathing with room temperature water or at least much less hotter than you are used to, Also, I would recommend trying an oral antihistamine once to twice daily to get the hives under control. Please check out my other videos to see how best to treat the hives. I sincerely hope this helps. DrJackAllergy

I am allergic to Allegra .. it causes a severe dry mouth and swelling on my face and lips. Benedryl makes me sleepy. I am afraid to take Zyrtec pr Claritin. Having a terrible time. .
@drjackallergy1065 ай бұрын
So sorry to hear this. I would recommend that you try Loratadine (generic claritin) but break it in half, and if possible, break the half into quarters. Then start with one quarter Loratadine twice daily. If you tolerate this for a few days then slowly increase to taking half morning and half evening. If you tolerate after a few more days, increase to either 3/4 tablets twice daily, or even one tablet twice daily. Cetirizine (generic zyrtec) is a stronger antihistamine and more effective. Basically, TWO claritin are equal to ONE zyrtec. So if you tolerate the Loratadine but still have hives, I would recommend you try the same routine of breaking into smaller pieces and building up until you, hopefully, can tolerate zyrtec twice daily. I hope this helps and wish you safety and good health. DrJackAllergy

Our natural cortisol (steroid) drops in the evening hours. Sadly, itchy skin and hives seem to worsen during our nocturnal hours. Hope this helps. DrJackAllergy

Seems like I read that urticaria can occur in the dermis layer and present without raised areas of skin, yet with the itching, is this accurate/ Also, I found a journal article re: the comorbidity of LPR/GERD and urticaria, I found that very interesting. I'm asking Derm to switch me from Dupix to Xolair, since Dupix is not producing results. Intense outbreak of itching occurred on my scalp last August and Dupix has not worked. The itching is at times systemic. A lot happened last year, Jan I had a vicious case of the Flu, followed by a hiatal hernia diagnosis, followed by a bike crash resulting in 4 broken ribs, broken scapula and a nice chunk of meat missing form my left knee. The. crash happened in late June and the intense itching happened in early September. I started rowing around the time the itching happened, I am wondering if my scalp itch is due to a nerve problem. Since the Army I have shaved my head and the doc says there is nothing visible. Only report I had of redness was the ER visit in September when the nurse said my head looked very red. ENT is also treating chronic sinusitis so I am also wondering if the sinus issues are causing the scalp itch. God I sound like a train wreck. This is very frustrating because for 15 years I was a semipro athlete and then 2023 happened. Can the Flu really cause such a cluster of chronic issues? Could it be that the digestive issues are the root cause? I believe all boxes are checked for LPR but hell get my GI doctor to accept LPR as a real thing and you have achieved greatness. I need this itching to go away!
@drjackallergy1068 ай бұрын
You are correct. Often it appears as angioedema although hives are usually present though not always. LPR/GERD can be involved but that is also why the use of H2 antagonists are effective as well.
@Long_Machine8 ай бұрын
@@drjackallergy106 HI Dr. Jack, thank you for the response. I may need to circle back around to trying H2 blockers. Whatever is causing my issues, it's covering multiple medical disciplines, thus making it very difficult to diagnose and treat. if I didn't have the LPR symptoms or digestive issues, seems like possible allergies would make sense. We looked at eosinophil levels and trytase levels but I was already on the Dupix so not sure about those test results. Fifteen years as a semipro athlete and never had any issues and then bamn, 2023 happened! I read that our immune systems are heavily linked to gut health, so I'm wondering if this hiatus hernia is interfering with my immune system, because of the positive Bravo Ph and Barium swallow that indicate reflux. It's LPR though, I've had very few typical reflux events over the last 16months but the upper respiratory symptoms are chronic.

I was fully vaccinated and boosted for Covid. But ended up getting a Covid Infection in November 2023. In April, 2024 I had to take an antibiotic prior to a teeth cleaning due to a knee replacement. 3 days after I broke out in hives. I went to Urgent care, ER and my family doctor. The Urgent care prescribed a 9 day course of Prednisone which helped a lot, but after the course was finished the hives came back. I had to wait a week and suffer through the hives since my family doctor was out of town. He prescribed an 12 day course of Prednisone which I am currently in the middle of. My doctor says if they come back I have to go to an allergist. My question is if this is a latent reaction to the antibiotic (amoxicillin) or Covid? I just know the hives are miserable. I appreciate another opinion.

Hello dr Jack,since July 2023 I have been experiencing severe hives outbreaks along with angioedema, which has finally stopped. Had been working and a very old hotel where there is eternal renovations and repairs going on.one day, in the front desk office where I sit, they decided to break open the ceiling to fix some sort of leak from the upper floor.there was a giant hole in the ceiling from July to September. It is shortly after that breaking open of the ceiling that I started experiencing bites/stings which were extremely itchy. I initially thought maybe mosquitoes,but it was not mosquitoes, but some sort of microscopic mites that can’t be seen with naked eye. I finally figured that out after buying a cheap microscope, where I continued to see the same insect over and over again.to make a long story short,I seem to have developed an allergy now to any insect that bites me, and I live in Florida,so insects are everywhere. Quit that job back in September,resolving that I will no longer work here only to be devoured day after day by some invisible pest. I no longer have angioedema, but the hives appear every five days or so, and usually first notice a bite somewhere on my foot or leg, then an hour or so later, start breaking out in hives on my thighs,or below the knees,and although they are not nearly as itchy as in the beginning, they are still very annoying,especially when they appear at night.when this first happened to me back in July, I actually went to a clinic where I was given liquid steroids to drink, a prescription for prednisone and that was it. Walked into that clinic looking like Daisy Duck,with my upper lip swollen out of proportion and hives on my upper thighs.the doctor quickly gave me the liquid steroid and everything went back to normal in a matter of two minutes. I took the prednisone grudgingly, because hate medications, I only take vitamins, but zero medication, not even Tylenol. I was taking the prednisone only as needed,it tasted disgusting and didn’t give me long term relief, so with the advice of a local pharmacist, I started taking four Zyrtec tablets a day,which made a big difference. I then lowered the dose to two tablets and I am only taking them as needed,when I get a breakout.my question to you is, should I take the Zyrtec daily,whether I have an outbreak or not? I don’t want to be on Zyrtec for the foreseeable future. And if I do take it daily, how long would I have to continue to eliminate the hives? Please respond when you get a chance. Am very grateful for you , because you offer an explanation without blaming food, and that’s what most doctors on KZbin do,thank you and please help!!

Not unless you can meet with a specialist to see if an injectable medication is an option. In the meantime I would strongly recommend that you add an H2 BLOCKER to your current therapy. You have the H1 blockade already but sounds like you need to add something like famotidine or cimetidine or ranitidine to your current therapy. I’d recommend taking one to two tablets twice daily in addition to your current regimen.
