My flight instructor in 1980 was a former Army Air Corps fighter pilot during WW2. The airplane I rented belonged to a guy who used to fly DC3's across the Himalayas and his wife was one of the women pilots who delivered airplanes from the factory to the squadrons during the war. The common belief among pilots at the time was that Amelia Earhart was not a good pilot.

So several mistakes were made, by several people, in part because of time pressure. Recipe for disaster indeed. One thing, given how experimental this was, wouldn't it make sense to test all radio systems, sending and receiving, right after takeoff? If one of the antennas were destroyed she would know and could abort. But I guess if that was a busy airport they might not want to schedule that much radio time for her?
