It should not be used for weight-loss. I've known many tweekers. In overweight people, as soon as they get off, they get bigger than they were before. But I guess it's safer than ozempic 😂. They still have to take it for life anyway.

Could this be add ? I (23 M) have some Problems that i did ignore for a long time or maybe even my entire life for example at work or when studying, I'm always wandering off after a few minutes and don't realize it. I have extreme procrastination when studying because i only get Motivation when i have almost no time and almost always started to learn for tests on the Night before and when i did learn a certain Part or removed some of the pressure i get carried away by my thoughts again without noticing, today i still start on the night before but when is just to much and i get also carried away by my thoughts i just give up and go to sleep (one time when i done that i even felt releaved and kind of free even tho i knew that i will fuck up the test), im a burden to my entire Family because i just cant be disciplined and learn like a normal person. They say that if you want it then your gonna do it if not then you just dont care enough. I know that they are fed up by me and my big brother thinks im a egoist for not learning and making my Parents more stressed and that im just cold and dont care that they're suffering. Even after this concersation i still cant do it better even if i dont want to be a burden to family maybe i am egoistical for not learning and it isnt add. Maybe im just to soft and lazy. The thing is i cant tell my brother that he should leave me alone because my Patents need finacial support when they retire which will be in the next years and i dont want them to struggle. I want to be able to help them but it also could be that i just dont care enough and actually am lazy. im doing a 3 year apprenticeship were im at the office for the practical part and then for a couple weeks at school snd always switching around every two months) i often move my leg up and down or left snd right or my pen without noticing then i make myslef stop which is very easy but half a minute or a minute later i catch myslef doing it again then i stop and it continues , sometimes have background music or sounds playing but it may also be normal. I was diagnosed with a tic disorder about 10 years ago was send to do therapy because they suspect it could be stress but it has never gone away completely, I read that it could also be a sign, but not necessarily. I have I also went to occupational therapy as a child for a while but my mother doesn't know anymore when i asked her. Sorry for the long text but do you think it could be add or not that much, I cant really tell if I'm just lazy and others have the same thing and are just tougher more disciplined than me or if it's really related to another thing. Also i hope i dont come across as someone who sees this as quirly or fun thing. Im sorry if the text is written in a messy or little confusing way but i dont have much time right now but i really wanted to get this out there now. Thanks if anyone answers. 🙏🏻

Imagine having 10 persons - previously undiagnosed - who are diagnosed with ADHD by an expert panel. Then imagine 10 comparable persons who themselves think they might have ADHD, but according to the same expert panel do not meet the criteria for having ADHD, without the experts informing them whether or not they have the diagnosis. Now make the experiment that all 20 persons are given lixdexamphetamine for ADHD to evaluate the improvement of symptoms and overall life quality. My guess is that psychiatrists will refrain from doing this kind of experiment as it will most like reveal that the diagnostic criteria for ADHD are so loosely defined that most of the population potentially can be diagnosed as having ADHD. As a Family Physician through 15 years, I have yet to see a patient convinced that he/she has ADHD and expressing a wish to receive medication for this, come back after being referred to a psychiatrist and not being diagnosed with ADHD.

From a short term perspective response can often be misleading but when it comes to long term adherence to stimulants , this is high - almost 64% in children’s and adolescents with a significant group stopping within two years. While specific numbers aren’t available for adults - adherence is an issue and side effects and loss of or ineffectiveness is a reason. Thus adhd management is not just about stimulants and nor is it about the short term - should be evaluated over the life span. We will know more in the coming years. I understand your points though.

Therapist usually trigger me and send me into a what appears to be a manic episode. I had to teach myself tricks to calm down after they stress me out. One trick is stepping in a shower with my clothes on. Its messes with my sensors and my brain goes from screaming crying manic to quiet and distracted from being heavey and wet. So my brain is more focussed on not being heavey wet cloths on skin to finding dry clothes.

I have been in and out of therapy so many times and normally I am happy before therapy. After therapy im mad and angry at the world. Therapy does not work for me from a regular therapist. My favorite person to talk to is my counselor at a outpatient rehab. I completed the program but i still stay becaue i love my counselor i see get to see once a month and ot works perfectly for me. Plus she is a Aquarius and im a gemini so air sign bond 😂😂😂. Also I find that i like the counselors in rehabs better than i do a regular therpaist. I learn more life skill tools from them than a therapist in a regular practice. Rehab helped me understand the core root of my triggers. Also how to learn how to let go. I have a double whammy too. Autism and Adhd and some therapist just dont understand me completely. I even gave a therapist one of my journals to read to get to know me more and he lost it. I asked for it back and he thought i said to get rid of it. Not exact words but same idea. After I did all that work for the dbt skills i was pissed because he didnt take the time to read it. I like to write better than talking sometimes. My words in my brain get lost from my brai. To mouth but from brain to typing or writting it works better.

The dose and duration has an influence as well - The larger and longer the worse the withdrawal. SNRIs are probably the harder ones to kick.

What do you recommend for motivation for adhd/bipolar. Stimulants help but then I get over focussed Buproprion and atomoxetine make me irritable

Please have a look at the video on stimulant confusion and why adhd is not just about stimulants. The hyperfocus may be indicative of the dose being too ‘high’. In the case of bipolarity , optimisation of mood stabiliser and evaluating and addressing other symptoms becomes important. Motivation has several domains - as part of hedonic drive - anticipatory, consummatory or motivational. Thus it needs an evaluation before the appropriate balance is achieved.

Autism ADHD Aids to my Theory of humans causing these problems by trying to play God and Goddess (Mother Nature is Goddess BTW) God and Goddess gave us all the tools in life through plants and animals ext. At one point in life I could not afford the antibiotics I needed to get ride of a infection before my leg was at the point of getting caught off. I was so persistent to live i looked on google natural plants antibiotic remedies. echinacea flower is a natural antibiotic. I ate the purple petals of these flowers every day and used antibiotic ointment to help stay alive because i could not afford medication. I also still have my leg and no infection. I was homeless living in the streets but i had access to a library that had a computer.

My grandmother on my dad side is full blood Korean. I believe when she was in her home in Seoul South Korea she had less health problems. Then when my grandfather on my dad side met her during war times. He and my grandmother had a baby my father Bruce W. Decker may he rest in peace. My grandfather brought both my grandmother and my dad back to America. From my understanding my grandfather was not a very nice man but I think he was just traumatized by war. i don't know him that well but i still love him. i don't know his background to understand why he was sad or angry. my grandfather is irish german and dutch. My grandmother on my mom side is 100 percent italian and born in america. My great great granparents came from italy. the are the La Salvias. My grandpa on my mom side is half itialian and half german. They are the Burgazoli's They also came from Italy. Napels italy maybe.

NEW IDEA!!!!!!! Can you by chance help me with an experiment? (Again, I do not want the credit for this I want it to go to you. My want is for the world to be safe for my daughter to live in and the other souls of the circle of life. I want her to be okay when I pass on to my next soul quest and die in this lifetime. I am also polytheistic. ) Calculate the difference of the mental health and physical health of someone from a different country other than America. See if their mental and physical health has changed when they came to America. Also if the government could fund money to test a theory. Take one person from each country that is not America with no mental health problems, no Physical health problems. Have those people move to America for a 5 year experiment trial with a visa for the experiment. Have them live a normal life as they would and see what changes happen in time from that person being in their own country and the problems they developed after living in America. Forgive me for not having the best grammar and if I may be a little confusing but I have a feeling you understand what I am attempting to say. This also plays a factor with Nature Vs. Nurture.

Dear Dr. I very much enjoy listening to your video and the example you used that included us views watching. Very smart move. My theory for ADHD and Autism is yes genetic background is one key component but also the food we eat. I am going to post a video that I found from another doctor on KZbin. I believe if the two of you put your brains together you can help us change the world. After I watched this video I started to cut out gluten and dairy from my diet. I noticed a huge difference within the 5-7 day time frame. I could not believe how the food has impacted us so much for those that live in America. My theory is that people of South Korea have a lower mental health psychiatric problem due to the food they consume. Maybe if you calculate the numbers of people in America with Autism and ADHD Diagnosis. Compare the numbers to the amount of people in South Korea with Autism and ADHD. My intuition tells me that the people of the South Korean culture will have less numbers vs. America. Why do you ask because of the food we eat. The chemicals we put into our body. The man made pills alters the chemicals in our brain there for when a pregnant mother takes something man made vs mother nature made she is poisoning her baby and herself not on purpose. We humans have been programed to do this by generation after generation from having the lack of knowledge. Since I changed my diet I yes am having withdrawals from food but I am also able to manage my mood swings better, I am able to have more energy and I even lost 10lbs in the 5-7 day period just from not eating dairy or gluten. Please forgive me for my writing is not as advance as it was the other day. I use Sativa Marijuana Medical to help me focus so I can think clearly. Due to my missing chemical I need that extra stimulation to help me. So I am using what mother nature has to offer when I am able to. If you have spare time can you also read a book call Moon Chosen and Sun Warrior by P.C. Cast. She uses a lot of herbal medicine practices examples in her fiction book based on spiritual truth in my opinion. I went to seek help because I thought I was becoming manic and my family had a history of Bi-polar. I was using cannabis to self-medicate and started using street THC cartridges all day every day. After going through multiple med changes with anti-depressants, mood stabilizers and then they prescribed me with Adderall and basically convinced my doctor I had ADHD, which I think I do to a point. The meds didn't help me be responsible and go to work and better my life. I started abusing Adderall and called my doctor and at that point I wanted to get off all the meds. I still maintained my cannabis addiction. Now I've stopped using cannabis for 3 weeks and haven't missed a day of work. I used to work maybe 3-4 days of a 5 day week, sometimes taking weeks or months off at a time. I was able to get my Master Electrician license even with all these circumstances. Now I'm applying to college at 37 to get an accounting degree. I never knew cannabis could make you manic even if you don't have bi-polar. Time will tell how I really am sober and if I really did have bi-polar. Addiction doesn't have to be a life sentence. <3

Also, I think there should be an avenue for university students and people with jobs that require high levels of executive function to be prescribed bupropion at lower levels perhaps 50mg -150mg, safely monitored by a doctor, to help them get through university and highly stressful jobs like lawyers, etc., that require significant executive function. This would decrease the risky university ADHD medication trade and e.g. lawyers using "non-prescribed stimulants" to get through their workloads.

Despite reducing her social engagements and part-time work hours, she continues to face challenges in maintaining focus on her studies, resulting in failing grades, heightened anxiety, and increased depressive symptoms spurred by social comparisons with her peers. - She discovers that a friend has been using Ritalin, a stimulant, to aid in studying and decides to purchase it from another university student. Due to limited availability, she purchases whichever dosage is available e.g. starting straight on 30mg of Ritalin, at a high cost. Although she notices a significant improvement in her study performance, she experiences side effects like insomnia and jitteriness. To manage the lack of sleep, she starts using sleeping tablets, but becomes dependent on them. Eventually she can't afford the Ritalin anymore due to increased demand from others that have heard about them at her university. She is now convinced that she can't achieve without them and drops out of university to avoid any further student loans. Scenario 2: - A university student is finding it difficult to balance her academic workload, part-time job, and social life etc., etc., - She reads online that GP's / Psychiatrists can prescribe cognitive enhancers or performance supplements. These medications are carefully monitored every two weeks for potential side effects, with the option to adjust the dosage by cutting tablets in half or even quarters to find an ideal balance for her needs. - This monitored prescription approach mitigates the risks associated with buying medications from friends, providing a new revenue stream for pharmaceutical companies in the realm of cognitive / performance enhancement, the government benefits from additional tax revenue, general practitioners oversee and monitor the usage, and overall, the system ensures safer, regulated access to performance-enhancing medications. TLDR; There should be a place for "tactical medication use". I feel benzodiazepines to decrease anxiety before surgery are already used as a "tactical medication".

For negative symptoms the evidence says yes. In clinical practice as well this is reflected . However to evaluate this change , one has to give 6-12 months and combine it with behavioural and psychological strategies for maximum benefit

I am so happy to come across this video.Thank you!! I have to agree that there is a lot of over diagnosis in youth. From my experience as a Counsellor. I have noticed that normal personalities and behaviors of who they are, they believe something is wrong, so would (google the signs) then Insist on testing for ADHD becoming medicated. I feel it is too easy to diagnose these days! Which is wrong! If a child struggles to focus, which of course, it can be a number of things, but each person is unique in their thinking and mind, depending on upbringing and safety.... with being unable to process emotions, without getting triggered fight and flight......, so will bury a part that for safety in self, so when quitening the mind to focus, the mind will panick and avoid.... Children feel there is something wrong with them these days.... Which is so sad, and as a result, drugged up to numb the mind.. Makes me feel so sad. The work I carry out in my sessions, I can tell if clients have a strong mind and how this needs to make sense to them.... I do hope these diagnostic systems that the education system are using are questioned more before all our youth are suppressing an important part of their personalities of growth... ❤

Did you know in the most recent revision of the DSM under autism they _specifically_ call out horse girls in the discussion on gender-related diagnostic issues? Honestly, it's a very welcome addition to the DSM (and the discussion itself was long overdue) but I feel like there's more than a few people who will feel like the DSM did them dirty by just calling them out like that 😂 Have fun studying it!

This might be off the beaten path for your channel but would you consider covering the 2023 article by Lieslehto et al., which examined different pharmacotherapeutic outcomes for BPD patients, and what implications this might have for understanding and treating BPD in future? I'd be fascinated to hear you cover this topic. (Admittedly my hunch is that there is probably a lot of undiagnosed or misdiagnosed people with ADHD, especially women, hiding out in that cohort who have developed BPD-esque presentation over time so I wonder if this might have skewed the results?)

I feel like there would be a lot of resistance to prescribing methylphenidate for BPD in clinical settings but this study holds a lot of promise for the future of BPD treatments. This is just speculation from an amateur but my hunch is that RSD and ODD are clusters of symptoms that are a behavioural response to that classic ADHD lack of inhibition, it's just that ODD presents in a very externalised, "classic" threat-response way whereas RSD is very much internalised and that both of these tend to fall roughly along the same lines that hyperactivity and inattentiveness are used to categorise ADHD. (I suspect that the more severe end of this often gets categorised as BPD eventually, especially if you take into account feedback loops in the social side of the psychosocial with a person who has poorly managed ODD or RSD.) If I had an unlimited budget and the perseverance to get a qualification, I'd love to research RSD and ODD especially in comparison to negativism as seen in catatonia.
