Norethistherone should also be on this list. It stops the bleeding almost immediately and can be taken safely for a month or so. Much quicker and better than Tranexamic acid.

This would be a progesterone only (hormonal) pill. Which I believe is the first category I talk about 😊. Yes it does help with the bleeding symptoms of fibroids for some!

I often don’t look at it as one hormone needs to be corrected. A lot of precise hormone signaling happens when the ovaries develop normal monthly cysts. So things such as stress, poor diet, sedentary could potentially throw off the balanced process. It’s why taking low dose hormone pills help prevent these cysts, it’s a consistent signal to suppress cyst development

Thank you for this video. My OBGYN is very resistant to the possibility of me getting a hysterectomy, but she's also kind of dismissive of the complications from other types of treatments with a VERY abnormally shaped uterus (unicornuate w/rudimentary horn, plus fibroids) - she keeps trying to get me to agree to an IUD. I'm worried she'll try to push an ablation rather than letting me get a hysterectomy.

I’m sorry, continue to voice your concerns and hopefully you can find something for you. It’s always ok to get a second opinion! Also if someone has a unicornuate uterus, an IUD would not work well, same for most types of ablation.

Return to work often is a collaborative decision with your doctor based on what someone does for work and the extent of the surgery. As far as bleeding after, I always tell my patients to expect some irregularity in the first 1-2 cycles, shouldn’t be heavy

Excellent video. The best channel on KZbin providing invaluable information on Fibroids. Please keep going with it. A few questions…How can one prevent scarring of the uterus? Can you explain how you reconstruct the uterus post a complex fibroid removal case via an open myomectomy? Can you also provide information on aftercare recommendations for healing post surgery. Any recommendations on how we can prepare our bodies prior to surgery? I’m asking as someone currently awaiting surgery for an open myomectomy and these are a few questions swirling around in my head that could also be beneficial for others in a similar situation to me.

We wish you a speedy recovery! Unfortunately not much can be done to prevent the scarring, it’s how the body heals. I would defer to your doctor on specifics, but generally a more active recovery is best

It's not "cramping" from a biopsy, it's absolutely horrific pain, stop down playing it. I had one recently & saying it was traumatic is under statement. Women should be at MINIMUM be given narcotic pain relievers & numbed for this "simple biopsy"

I’m sorry for your experience. Yes in our profession we need to continue to strive to be better than the past. I certainly offer all my patients some form of analgesia. On this channel there will never be minimizing, I’m sorry it must have been a misinterpretation, we are fighting the same battle

Thank you so much for this. I am an African American woman that will be subscribing and sharing you’ll channel. I feel it is very important for women to have multiple perspectives as our bodies are so similar and so different all at the same time. I really want a hysterectomy due to heavy periods and horrible cramping. As a 36 year old female and mother of 3, my doc has advices me to try this procedure and also have the tubes removed . My sister passed away from ovarian cancer, so removing my tubes will help to ease my mind with genetics along with him informing me that ovarian cancer starts in the tubes and not ovaries . What do you’ll think? I know this procedure is not 100% in stopping the bleeding and or cramps, but taking my age into consideration and doctors advising , I am trying this procedure 1st along with tube removal.

Glad to hear you are getting help! Yes it is a good procedure to help with bleeding. We don’t completely understand how much removing fallopian tubes reduces the cancer risk. Some, not all ovarian cancers would originate there. I am sorry to hear about your sister, I can only imagine. If you haven’t had genetic testing for cancer syndromes, please do so! That would be the best way to look after your health

I never understood the link red meats (grass fed) and fibroids. I have fibroids and I have a genetic predisposition. I eat red meat. My fibroids are big but asymptomatic. My mom had to have a hysterectomy way younger than I am. She had red meat sporadically and had really bad symptoms like bleeding a pain. Her first cousin had fibroids with bad symptoms as well but she could not afford surgery so she waited for menopause. My cousin was a…VEGAN!!! I am not understanding why they keep blaming red meat for EVERYTHING!!! Without real research done. I’m not blaming anyone but can we stop this red meat shaming?

No shaming here. But unfortunately we don’t have much in the realm of fibroid prevention. They found some associations, doesn’t mean it’s the end all answer for those struggling with fibroids👍🏼

I know this may be very long. However, I anticipate your your response. I was experiencing vaginal bleeding/ spotting (very light becasue it didn't fill a p/liner). I decided to take a hcg urine test after 2-3 days because the bleeding/ spotting didn't stop. The test result was positive (this was the Saturday). The following evening after (Sunday) taking the home hcg test, the spotting/ bleeding basically fill the p/liner. The following day (Monday) I deceided to go and get an ultrasound done (the spotting/ bleeding) was still filling a p/liner. I was concerned as to what was very and wanted to know what was happening (this was my very first time. I advised the sonographer/ doctor about what was happening during the examination (both abdominal and transvaginal ultrasounds were done). He told me that there was a large volume of flyid in the POD (on the report it stated 75cc and possible leaking ectopic pregnancy, everything else was normal and okay). At this time I was only like 1-2 weeks pregnant but i didn't know until waaaay later). I wasn't feeling any pain or discomfort unless someone pressed there hand or finger into my lower abdomen which i would consider normal) Question: Can a normal pregnancy be confirmed by way of an ultrasound scan at 1 to 2 weeks of pregnancy?

Thank you for the message. Unfortunately pregnancies can only be seen definitely with ultrasound around 5-6 weeks. Before that time, we can only follow lab work to make our best estimate if the pregnancy is developing normally. I think based on your message, they had enough concern to diagnose and treat for ectopic. The usual single dose of the medication rarely causes side effects. I hope this helps.

New subscriber! I'm getting a hysterectomy in Oct due to cervical dysplasia, CIN 3. I didn't have clear margins after my LEEP. I'd love to see a video on dysplasia and/or how to prevent vaginal cancer after a hysterectomy if there's things that can prevent it. Great video!

We are working our way through cervical cancer prevention right now! It is pretty rare I would recommend hysterectomy for CIN3, would usually prefer colposcopy and potential re-excision if needed. Especially if someone only had one leep before

My doctor told me my two options were to repeat the pap smear in 4-6 months and if it was abnormal, that would be when we would repeat the colposcopy and possibly the LEEP of needed or do the hysterectomy if I was 100% sure I didn't want kids. I even asked him if his wife was dealing with this what he would want her to do, and he said hysterectomy, so I went with that. Do you think I was given bad advice?

@LikelyLost I wouldn’t say bad advice, just not what I would recommend. I understand the no kids part but likely excessive and you still need close monitoring and potential vaginal colposcopy and biopsy in the future, which isn’t ideal.

Howdo u differentiate an ovarian cyst with ectopic pregnancy? I had an ectopic in nov’23 and it was detected very early in the pregnancy due to severe ovary pain and light brown discharge. They said I also have ovarian cysts. I am currently 8 weeks pregnant with some dull pelvic ovary pain again BUT NO BLEEDING. Pain comes and goes is this more likely ovarian cyst or ectopic?

It can be difficult diagnosis sometimes if the pregnancy is too early to see on ultrasound but at 8 weeks you should see the pregnancy in the uterus to confirm a normal pregnancy.

There can be some dysmenorrhea from fibroids but it would likely be cyclic, meaning around certain points of the menstrual cycle. Otherwise gastrointestinal issues are also very common by themselves.

Excellent explanation of various treatment options and, by far, the best channel on KZbin about gynaecological issues, especially fibroids. I have a quick query, if you could please answer it. What would be the best treatment option for someone with a 9mm x 8mm intramural fibroid located in the posterior wall of the uterus? What is the likelihood of this fibroid worsening, such as increasing in size or the development of additional fibroids in the future? Secondly, I read that prolonged exposure to oestrogen, including in women who have never been pregnant, can lead to the development of fibroids and might also increase the risk of breast cancer. Could you please clarify if this is correct? I would suggest addressing this issue in your future videos. Thank you very much, and God bless!

Thank you for the kind words! Luckily this fibroid is less than 1 cm in size so a lower chance it is causing symptoms. And symptoms would be the main driver whether to treat or not. So likely in this scenario, no treatment needed! As far as the estrogen exposure (I’m assuming you mean one of the hormone medications) while fibroids like physiologic hormones, there is not great evidence to suggest taking a low dose hormonal medication will put you at risk of growing fibroids. Also taking an estrogen will not increase your risk of breast cancer… but if one were to develop and its hormone receptor positive, it could potentially stimulate it more.

Unfortunately there is not, but even with a big fibroid, often it will not affect the pregnancy 😊

I have bulk symptoms suddenly in 2024 so I went to the Dr and they sent me for imaging. Lo and behold I have fibroids. Also I’ve had fatigue for over 10 years and have been chasing my tail trying to figure it out now I’m wondering if it’s been fibroids all along but I didn’t have massive bleeding or anemia. Just fatigue and sudden bulk!

Thank you so much for doing this series. I'll have my procedure soon. I thought i would've been grossed out by the surgery videos but they were actually helpful. One of the things my doctor mentioned is dissecting the fibroid because it was too large to be removed vaginally. Not sure if you've touched on that in other videos. Thanks again 🙏🏽.

Thank you! And we are happy it helped. Yes sometimes the uterus or fibroids are so big we need to make it smaller to remove it and keep the surgery minimally invasive. We call this morcellation and it can be done in a number of ways! I will try and touch on it soon in the videos

I'm really anxious about pap test , usually my gp has to contact me several times before I make decision to go in and have it done . After last time I was really grateful to go in as you hear horror stories like death of Jesscia Pathway and you realise that pap test is for your benefit! congrats on getting married 😊

I know sometimes it can be presented in a way that is not super easy for patients to understand, and that can lead to a lot of anxiety before providers interpret the results!

Am dealing with fibroids got diagnosed in 2017 just had a Doctor's appt today to see if they have gotten bigger which they have one of them the size they said 10 .9 cim the other few are 5.2 or 2 cim am not trying to go the way of surgery.but i did ask the Doctor were there any Natural things i could go about dealing with the pain am experiencing now. Am 50 years old. Am not dealing with Heavy periods because my periods they come and go i just had a period in March after it went away for 5 months. So we're in the month of May now haven't seen my period since March. But am going through pain mainly in my abdominal area on my left side were am experiencing pain.

Thank you for the question. Glad to hear the bleeding isn’t a symptom! If there is no other cause for the pain, it may be the fibroids… which often it is a discussion and depends how bad the pain is. You could certainly try these options but by no means is it a guarantee it will work. Menopause may shrink them, but they likely will never go away. If the symptoms are disruptive to enough, I would often encourage someone to consider minimally invasive surgery. But if the pain isn’t too much, you have time to try other remedies.

Can you remove intramural fibroids in a hysteroscopy? Or is this only suitable for the subserosal type?

Unfortunately only submucosal can be removed with hysteroscopy. There is a hysteroscopic ablative procedure called Sonata that can inactivate/decrease size of intramural fibroids.
