Is it common for people to have several psychiatric disorders at the same time?
@Dr.Gagarin
@Dr.Gagarin 4 ай бұрын
That is a great question. Sometimes, conditions are unrelated, like ADHD and schizophrenia. Then they co-occur as you'd expect statistically in general population, that is about 1% of ADHD patients also have schizophrenia. However, most of the time the conditions are relatively but we don't always know how, like 20% of OCD patients also have ADHD, that is 3 times the average population rate. Moreover, conditions like anxiety and depressive disorders co-occur predictably. At least 60% of patients with depression also have anxiety disorder and many patients with anxiety disorders will eventually develop depression.

Dr. Gagarin, thank you very much for the long-awaited new video! It's been extremely interesting and fascinating, as always. The way you explain the genesis of the mental health issues is so exciting. I do hope that Jeff is now OK and not experiencing those painful traumatic conditions he used to. I guess, DID may sometimes be the case with the criminal issues as well. If a person develops a tendency or a lust for killing, for instance, but is not, so to say, quite happy with it, because he/she has some moral code, some principles which judge his/her criminal behavior, then, maybe, DID becomes some kind of a coping mechanism when a person can blame another identity for committing crimes. Once again, thanks a lot for this video, and we'll be eagerly looking forward to the new ones:)

Squirrels go nuts about losing their nuts:) I'm sorry, Dr. Gagarin, that was just an awkward and clumsy effort of mine to joke... I've got no sense of humor at all) OK, leaving the jokes aside. I'd like to sincerely thank you for another informative, interesting, and exciting video. Thanks for such a thorough description of these different kinds of overthinking. I guess that it's a mental phenomenon that may be a symptom... or, maybe, it's better to say that it may accompany different disorders. Just like PTSD that you've mentioned in this video. Maybe, OCD and Borderline Personal Disorder as well? Like, for instance, when overthinking comes under the guise of some ritualized formulas that a person uses in their mind as a reply to certain outward factors that cause his/her anxiety. I guess, maybe, in such cases religious upbringing is also a factor that brings into being this type of a mind pattern. And here the overthinking becomes some kind of a ritualized rumination of certain words or spells that become sort of a coping strategy to deal with this constant anxiety or irritation a person senses inside themselves... But a pretty lame coping strategy, since it eats away so much time, depriving him/her of the ability to concentrate on either work or pleasure, or anything else. I'm sorry for another terribly lengthy and, I guess, incoherent comment, but your videos always give food for my thought, they kinda ignite that spark that starts a certain thinking process... I'm sorry if it's all inappropriate. Wishing you an awesome weekend!:)

Thank you very much for the new exciting video, Dr. Gagarin! I guess Maria's story is what's called a "co-morbid" case in psychiatry? In real life, I believe, professionals like you rarely meet with the "pure" cases of people whose mental problems confine to the framework of only one diagnosis. Most patients suffer from several mental disorders at the same time, with the symptoms intertwined in the clinical picture of their psychic dysfunction. So you have to prioritize what is more and what is less important, in order to understand the right continuity of treatment, and that may sometimes be a tough task... Let's hope that Maria's treatment proceeds successfully and she's gonna get into the condition of a stable remission. Wishing you a nice week ahead, Dr. Gagarin, and awaiting your new videos🙂

Hi, Dmitry. You nailed it! "Comorbidity" assumes that there are separate illnesses happening at the same time. However, what DSM identifies as separate illnesses may not be such. From my perspective, each person has their own personal "condition" that may incorporate various DSM diagnoses. We can't avoid putting people in little boxes because making diagnoses help to develop treatment. In my videos, I am trying to tell my patients' personal stories to show that people are not diagnoses. Real life is way more diverse and fascinating, but since it is not possible yet to create personalized treatments for everyone, I try to combine available evidence-based approach that include medications, social support and psychotherapy. Thank you so much for helping to shape my channel.

@@Dr.Gagarin Oh, please don't mention it) It is a real treat, an honor, and a pleasure for me to be able to somehow help the development of your channel. Thanks for your reply. Yes, every person is unique and has their own mental condition, but mental science, as any science in general, has to generalize to elaborate some general notions and laws to operate with, I guess... and then apply those laws to the separate cases, trying to use general methods for the unique situations. How did Hegel put it? The dialectics of the universal and the individual? I truly adore your approach, Dr. Gagarin. You make each story you tell an exciting narrative, and it's not only entertaining, but inspiring as well, letting other people know that there's always hope and possibility to improve smth., no matter how hopeless a case of mental disorder may seem. I sincerely wish you every success in the further development of your channel. And have a merry first weekend of the coming spring! Thanks a lot for this video, Dr. Gagarin. Thirty years together is quite a long term. I believe it really is the sign of stability of a long-lasting relationship. Although sometimes there are sad cases when people decide to separate ways with their partner even after being quite a remarkable number of years together with him/her. You know, recently the world famous rock star James Hetfield has decided to divorce his wife, even though they'd been maried for 30+ years, if I'm not mistaken, and have several kids together. But smth. has gone wrong... Of course, I truly believe that you and your beautiful wife will not repeat such a lamentable case, and I sincerely wish you to live together happily married for another thirty years or even more. Thank you for sharing your personal experience. Tomorrow's gonna be the birthday of Kurt Cobain, one of the crucial figures in the history of modern American culture, who (accoring to the official version, at least) took his own life at the age of only 27... I may be mistaken, but I believe you've made already some video about coping with the suicidal thoughts? I guess I should check more attentively the content of your channel. @@Dr.Gagarin You're welcome! Yes, I totally agree that Kurt Cobain was an absolute genius. And I can't even imagine what ordains you have to go through on a daily basis while dealing with the suiciders. I mean, you're a trained professional, you know the coping skills and protective mechanisms to deal with such negative experience, but still it must somehow influence the psyche of a practicing specialist. You help the people in pain in their struggles, but their battles become your battles, too. And each one of the patients has to fight his/her own battle, but you have to fight, to some extent, multiple battles simultaneously. And that may affect the mental condition, I guess, due to a cumulative effect of constantly dealing with a lot of negativity. I know that there exist, for practicing psychiatrists, some supervisors which help them deal with the working stress, with all the tension they get at work. But I wonder if those supervisors sometimes need supervision themselves. And those who supervise those supervisors, again needs supervising, and so on...

Thanks so much for the positive charge this morning which came with your video, Dr. Gagarin! May I please make one slight correction to the historical part? I am not an expert in the history of ancient Rome, but I believe the very first Claudius reigned from 41 AD (succeeding Caligula as the Roman emperor) until 54 AD (when he, in his turn, was succeeded by Nero, after being poisoned by his relatives or smth. like that). I may be mistaken. But, if it's true, then, I guess, that Claudius whom you've mentioned first in the video was not actually the first, but, as a minimum, the second one:) I remember another version of the legend about the priest named Valentine executed by the order of the emperor. It has a somewhat more romantic strain to it, stating that, while in jail, awaiting for his execution, that priest fell in love with the daughter of the prison warden. The daughter's name was Julia. And the priest started writing love letters to her (don't know if she read them or not), hence the tradition of the modern days' Valentines... A pretty romantic and funny fairy-tale, isn't it? Just like the tales of healing the blind by the Christian saints. Where is at least one officially registered case of such miracles since the time when the modern means of photo and video registration appeared, right? Where's the proof? But that's another story. You know, having turned forty, I really don't suffer much anymore because of being single on Valentine's day or any other day of the year. Maybe, it's because I have other problems, maybe, it's smth. that has to do with the reduction of the hormone secretion? As far as I understand your approach, you propose to substitute the missing elements of the '''love triangle'' with ''re-directing'' the psychic energy to certain other activities, with changing the focus of your love, so to say. The tips you give are truly very useful. Once again, pardon me for such a lengthy comment (I guess I should have tried writing novels, but, unfortunately, I don't have any literary talent☺), Happy Valentine's Day to you and all your close ones, and eagerly anticipating the new videos on your channel!

Thank you, Dmitry. I appreciate your comment very much. It definitely adds more depth to my "history research". Although I love history, I don't always keep up with details favouring more of a general idea The point I was trying to make is that there is no need to be upset about something that seems to be made up in the first place. It is interesting you mentioned talent. One idea I have for my future videos is to separate talents from skills. There is plenty of evidence suggesting that practice is much more important. Judging by your eloquent comments, you have enough talent to build the skills. Good luck with your novel 😉

My current relationship of a little over 2 years has been mainly remote, having only spent a month together. However, we will be living together in about 10 weeks to start our life together, which as we're both now in our fifties, I guess it goes to show that it's never too late to plan a future together. Thank you for another informative and instructive video, Dr. Gagarin! Matthew Perry was truly a remarkable actor, and, though never knowing him personally, I also sensed grief when I heard of his demise. You know, I haven't even known that he'd released a book on his addictions and mental health issues. I believe it's truly a fascinating read, though maybe quite sad at times. Addictions have destroyed the lives and careers of so many people... Though sometimes there are such exceptional and curious cases, like the one of Charles Bukowski who was a heavy drinker and consumed enormous amounts of alcohol and at the same time became quite a successful and popular writer, having died only at the of 74 because of leukemia (don't know whether his alcoholism had had any influence on the development of his fatal illness). R.I.P. to Mr. Perry and thank him for all his wonderful roles. Thank you so much for this new interesting video, Dr. Gagarin! Hope that Sofia is doing fine now. Choosing your future career, your professional path can be a pretty tough decision to make. What if Sofia probably gets disappointed with computer science after a couple of years of studying it and, for instance, decides to switch to the art studies? That would be another challenge for her to take. And sometimes such cases do happen: when young people feel very much inclined to some branch of knowledge, but when they start to study it deeply, to indulge in it, they realize it's not the vocation to which they want to dedicate their lives. I guess that can also be pretty traumatizing, for they have to switch to some other path, to find for themselves another field where they can get education and build a career.

Hi Dmitry. Thank you for your insightful comment. There is a big difference between being passionate about learning and actual vocation. Perfect example is cooking. I love cooking and baking, but would I like to be a chef or a professional baker? Probably not, because cooking one meal for my family is VERY different from making 200 meals for strangers with high expectations. One point of this video is to talk to the kids about advantages of a particular job instead of trying to mould them into a specific profession. Thank yo for watching. @@Dr.Gagarin Thank you so very much for your response, Dr. Gagarin. Then, probably, we should draw a line between vocation as a calling to choose some professional skill to build a future career, and a hobby, a pastime, even maybe our most loved way of spending time, but the one we can only regard as smth. for our souls and not as a basis for our financial prosperity. For example: if someone loves music, loves composing the tunes, verses, songs, loves playing musical instruments, but somehow he/she, due to certain circumstances, understands that he/she can't make a professional career in this field and has to choose, maybe, some mundane job to make a living, to pay the bills, but that person feels that that mundane job is not really his/her true vocation, with music being smth. which he/she does for the true delight of the soul. So that person has to compromise... Thank you for such a feedback. Dr. Gagarin, could you please speak about the intrusive thoughts and the possible coping strategies to deal with them? You've already touched this subject by saying that the less you focus on certain problem, the easier it goes away. And that also gives me hope and a way to deal, once again, with that from which I suffer. But, seemingly being an OCD patient, I'd love to hear about the possible coping strategies... I don't know. When these intrusive thoughts overwhelm your mind, when they become smth. ritualistic and you feel like you're obliged to repeat certain formulas over and over again just to get rid of that mental ghosts which are constantly haunting you. I'm sorry for such a lengthy and tiresome comment)

I can't speak for others, but for me personally your videos always do help to understand better not only certain theoretical psychological and psychiatric subjects, but my own personal struggles. That is very helpful, and I'm so grateful to you for that. That's such an interesting problem of this intertwining of grief and addictions. The way you explain it, that getting high on alcohol or drugs is also a kind of a coping strategy for grief (though quite destructive for the person who tends to use such a strategy), was so revealing for me. Truly, when one takes intoxicating substances, he/she does so in a futile attempt to run away from his/her problems. But thus a person rids himself from the necessity of going through the second stage and jumps straight to anger... A kind of a shortened way of processing grief, but it may also drastically shorten a person's life because of the addiction to such a method of solving problems. Thank you once again, hope you're having a nice week-end, and I'll be eagerly waiting for your new videos!

@@Dr.Gagarin I like watching your videos and reading your comments very much, Dr. Gagarin. I think you've got excellent storytelling skills. The cases you present are always so exciting and interesting to watch. I believe by sending your message out to the world, to the people who are willing and able to receive it, you build some kind of a relationship not only with the patients with whom you work, but also with your viewers. Some kind of a spiritual relationship. And watching your videos does have a great therapeutic effect. For the people prone to self-analysis your stories may help better understand themselves and also help in their personal struggles with the disorders they're living with. For nowadays we live in such a crazy epoch that, I guess, it's almost impossible to find a person completely devoid of any scent of some mental disorder. Thank you once again, have a nice day, and eagerly anticipating the new vids from you!

Thank you for the new inspiring video, Dr. Gagarin. Your reels are always so interesting to watch. Over 50,000 Americans committed suicide last year... It's really an alarming and awful number. You know, I have a friend who's been suffering from mental disorders for many years. Luckily, she's still alive, though she's tried suicide many times. I love and cherish her, but sometimes I get kinda frightened in communication with her because I cannot predict what possible effect some of my words may have. Because the human beings with a fragile psyche can sometimes misinterpret certain things, for instance, they can find in your words the meaning you didn't intend. I'm so sorry for the loss of your mother, Dr. Gagarin. Please accept my sincere condolences. Maybe, your mum is now in some better world... And how well I understand Simon... I feel so compassionate about him because I've gone through such experience myself. When you are young, when you are romantically involved with someone, when you encounter another person who really seems to be your soulmate... Sometimes it can be exactly as you've described: this person may become the center of your attention, that one psychological island on which you concentrate your affection, your love the most. And then, if the relationship ends, it can be like an overwhelming crisis for a young and inexperienced personality. With age you somehow learn to live through this, maybe sometimes becoming more cynical and desensitized, to some extent, but that's the mechanism your psyche uses to protect you from pain. But the younger people, they just haven't developed yet these coping strategies, these protective mechanisms which help to survive broken relationships. That's why they suffer so much... And it's all so sad... Let's hope that at least this year the rate of the suicides in the US won't be so high, in particular thanks to the specialists like you who help dealing with the trauma and help prevent possible suicide acts of people suffering from different breakdowns.

@@Dr.Gagarin Thank you for your kind words, Dr. Gagarin. Well, psychology and psychiatry, I believe, always deal with personal stories. Even stepping aside from the applied side (sorry for the wordplay) of these disciplines, the theory itself is always based on some empirical material which, in the case with psychology, psychiatry, psychopathology, always boils down to the personal stories. Case studies are essential for the development of these branches of knowledge and for understanding ourselves (and others) better. Gonna wait for your new videos with eager anticipation, as always, and wish you a nice Sunday and a fine week ahead!

Thanks a lot, Mr. Gagarin! I guess I've said it before, but I shall repeat that your videos do have an excellent therapeutic effect. I watch them always with such great pleasure, and they kinda calm me down. I've especially liked how you've put some philosophical basis to the themes you've analyzed in this video. Albert Camus was truly an eminent writer, playwright, and philosopher. Sort of makes me sad that we're gonna remember his tragic demise in just four days. Although he was suffering from tuberculosis, I believe he was far from desiring his own death at the moment that fatal car crash happened... Though who knows. Yeah, I know about the conception of the ''flow'', and thank you for mentioning it. I believe it was an American psychologist of Hungarian descent Mikhay Chiksentmikhayi (never sure if I spell his name correctly, so pardon me if I've made some mistake in it) who's written a book entitled ''The Flow'' and dedicated to the analysis of this concept. Chiksentmikhayi seems to have passed away not so long ago, in 2020 or smth., and has lived quite a long life. By the way, I don't think that you have any overweight. In no way. You're in a pretty good shape. And your cap's awesome. All the best in the coming 2024 to you, my favorite psychologist and psychiatrist!

@@Dr.GagarinThank you. I do like your use of an analogy to better explain something and make it easier to understand behaviours very helpful. Two areas that interest me on a personal level are coping mechanisms and over-thinking. Whether good or bad, a better understanding of these would be helpful.

Thank you so very much, Dr. Gagarin! It's really quite a useful set of tips for people like me dealing with depression. I really appreciate it personally, and I think it's so noble of you to try to help others in pain. I wish you a joyous holiday season as well. I believe in periods like these there are more cases of mental crashes, even suicide attempts sometimes, from the people suffering from mental disorders, so professionals like you have more work at the festive season. Stay strong and hold on! Thank you once again, and I wish a rapid growth and development to your channel and success in everything you do!

@@Dr.Gagarin Who knows, Doctor, who knows... It's hard to predict the future. And, I guess, it's almost impossible to build one, for life can destroy everything you try to build in one single moment. Probably, such is the sad existential essence of life. But I completely agree with you that we, like-minded people, should stick together. And the role of you and your noble colleagues in helping to alleviate the suffering of those struggling with mental pain is indispensable. Happy coming new year, Doctor! Best wishes for 2024, and eagerly anticipating new videos on your channel!

Boredom (04:45)! It feels so bad in ADHD because of dysfunction of Default Mode Network (DMN) of the brain (kzbin.info/www/bejne/rWmtn3eepZ1mnaMsi=SYt9efBZ9MAy9OOO&t=32). In ADHD boredom = brain death because DMN fails Some meds like bupropion can help (off-label) keep the brain awake without over the board excitement. Check out Part 2
