I just want to thank you so much for the quality of information you share in your videos. Your material was the only review material I used to study for my PANRE. I didn’t have much time because of my work schedule so I listened to the lessons as I drive to and from work. I took my exam last Wednesday, Dec 18th and got my results this morning that I passed. I'm on Emsam. It's been a miracle after four "standard" medications and ECT. I've seen multiple figures but the general range of efficacy for the usual treatments is 50% or a coin toss. Up to a third on the STAR*D protocols never reach remission. Transdermal selegiline takes about 2 weeks to begin making a difference while many others can take 6 weeks or more. Up to 50% of people on antidepressants stop taking them. There are many different reasons but lack of any noticeable difference and concerns about side effects are usually high on the list. Some studies estimate sexual side effects for up to 50% for men and women on standard treatments. I am a bit disappointed in how transdermal MAOIs are handled. I had to find the MAOI solution myself because the message is "use anything except this" so I was on a merry-go-round of ineffective medication for over a decade. I probably lost a good 15 years of my life to living like a piece of cardboard with extraordinary anhedonia and emotional blunting. It's rarely or never offered because it's misunderstood or even unknown to new doctors. The topical Selegiline does not have the "cheese effect" or hypertension issues related to diet at 6mg. No tests were done at 9mg or 12mg but I have seen no reports that are easily accessible that show this side effect happening on topical MAOIs at higher doses. The only report of serotonin syndrome I could find was a patient on multiple recreational drugs and wearing double their dose (two patches). Given that there are now two topical solutions this really needs revisited. I'm hopeful someone will see need to look at the results of the topical MAOIs in larger studies. pmc.ncbi.nlm.nih.gov/articles/PMC4200016/
