question: what if you did the bronchodilator or methacholine challenge and it increases, but less than 10%. Is it Asthma or COPD?

So on an exam question, if you gave them a bronchodilator and they didnt improve by 12% or more, definitely be thinking a cause other than asthma like copd. In real life, not the case, but exam questions they wont try to trick you, so learn it that way, but realize once youre out there practicing, these are just general guidelines to follow, but in no way 100% accurate for all patients. A patient with COPD may very well improve >12% with a bronchodilator, and a patient with asthma may not.

@@cramthepance thank you! I once had an exam question that said the pt bronchodilator results increased by 7%. There was no other info about the symptoms of the pt. they just had the results in a table and asthma and COPD were both listed as an option. i picked COPD. I never got the results so I'm not sure if I got that question right or wrong.

To diagnose intermittent or mild persistent, since the FEV is not reduced and is normal, after doing spirometry, is the proper diagnosis on a test question a methacholine challenge test?

Exactly. If they have normal PFT's, you want to try to induce them, and you do that with a methacholine challenge test.
