I’m 6 weeks from my post op on my right patellar tendon I was wondering how long did it take u to feel comfortable running again

I actually don't do much running so I haven't really gone for a run. I have done some intervals on the treadmill and I would say around 6 months that was no issue but don't remember exactly. I would prefer to do sprints and at this point I'm holding off on those until the strength in my injured leg matches my uninjured leg. I actually just made a video today where I talk about running because so many people ask me about it. Hopefully I'll upload it in the next day or two.

Hi Ryan, I appreciated all the update videos! I’m currently at 15 weeks post-operation, same as in this video. I’m similar to where you were in terms of muscle mass and general functionality, but my ROM is seriously lacking. In this position at 0:54, I’m only about 115 degrees. Any suggestions on how to progress effectively with ROM?

@@rexliu01 I feel like the farther you get with ROM the slower it starts to go at the end. In order to get to the point I was in this video I just did as much ROM stuff as I could do everyday. Typically I started by soaking my knee in a hot bath and massaging my knee. Then I would do my ROM work when my knee was still warm.

I like your sled pushing/pulling setup!! Regardless of patellar recovery, sled pulling (backwards in particular), is supposed to be really good for your knees in general.

Hey Ryan came across your video's yesterday. Im 29 years old and i literally am 3 days after surgery for the same injury. I been having issues sleeping normally at night like 2-3 hours at most and its really bugging me. Any helpful tips will be greatly appreciated it!

@@ItsF34Rz I had a lot of trouble sleeping at first as well. For me it was because my glute and hamstrings would cramp from wearing my brace. I took the brace off at night after a couple weeks but to be honest I didn't really sleep well for probably a couple months. I just had a hard time while my leg was straight. I move around a lot in my sleep so my improvement in sleep seemed to be linked to my range of motion improving.

@@ItsF34Rz I essentially had to wake up everytime I rolled over for at least the first couple months. Early on things just move slow and it's not a lot of fun. Hang in there and things will get better.

Hey man, love the videos. They are super helpful. I’m 16 weeks post op for this injury and the front of my knee is still really tight/stiff when I stand and walk. It’s hindering my gait alittle bit. Did you experience any stiffness in that front repair area that far post op? I can almost feel where they attached the tendon but have read this goes away with strength training. I’m even wondering if it’s scar tissue. Thanks bro!

@@chrisreed3358 hard to say if I had the exact same feeling but I believe at 16 weeks I still had some stiffness in my knee and walking still felt a little off even though it may have looked normal. I feel like even though I had plenty of range of motion it took me a long time for my gait to be completely normal again.

I’m about a week out since the injury happened terrible timing I couldn’t get a hold of any orthopedic surgeons through the holidays, I finally got a hold of one and have my appointment tomorrow. First couple weeks are pretty slow. After that I would focus on range of motion and then whatever strength exercises you can do mostly to muscles besides the quads like hamstrings, glutes and muscles in the lower leg. You can do light exercises for your quads as well. Then it seems like the 12 week mark is when they generally take restrictions off for weight training so you can start to really strengthen your quads at that point. It's a very long process but if you stay positive and work at it you can get back to being active.

great video's Ryan, i am currently at week 5 myself and will go 60 degree at 6 weeks (next monday). Flexing behind my desk and using the resistant band 2x day to lift and bend. 2 sets of 10x.

Wishing you a speedy recovery! I’m about five weeks post-op, and it’s definitely been a journey, to say the least. However, things do improve with time. Take it easy, don’t push yourself, and allow your body the time it needs to heal properly.

Hey Ryan! Hope all is well once again and great recovery to be able to do that at the 7 months mark. My patella injury is similar to yours as in I tore it at the mid patella as opposed to of the bone. I’m a little over 6 week mark post surgery and at about maybe 40 degree ROM. My recovery is slow but how far would you say is good to flex the knee till in degree so I know I’m not pushing to much? Doc says go at your own pace, not going to Physical therapist. Also what’s a good increment weekly in degrees that’s a good goal to aim for ROM?

@@davidmulat it's difficult to say because I hear people talking about how much pain they are in when they go to therapy and get their knee stretched but I never had much pain. I think the key is just consistency and working on it everyday. When my Dr told me to work on range of motion he said 5-7 degrees a week was good. Don't get too hung up on how fast you get there. I know two people who had this injury decades ago and they both wore a straight brace for 3 months before starting ROM work and both of them recovered completely. Just try to be consistent.

@ yeah man hooping in a men’s league some guy tried to take a charge on me while on a fast break and stepped on his foot my knee twisted and blew the patellar tendon off the knee cap. thanks man our recovery results are very similar! I was able to hit a 135lbs back squat for 4x6 this past Monday at full depth and split squat 70 lbs for 3x10. I ran over a little over a mile today. I still have issues in the leg extension over 10lbs lots of crunching like yourself. Plyos are getting better feeling a little more bouncy. I still have discomfort at times but it’s going real well I can’t wait to continue to see your progress!

Hiya I’ve just found your video. I completely tore my patella tendon off my knee cap 18 weeks ago. All I did was step off the kerb. I had to have holes drilled into my knee cap and the tendon re attached. I’m having physio and I’m in the middle of 4 weeks of hydro therapy. I was in full leg brace for 10 weeks to which I couldn’t bend my leg at all. I’m still finding it really difficult to walk as my knee doesn’t want to bend. My consultant has told me it can take from 6 to 12 months to heal. Fed up doesn’t cut it as I’m not allowed to go to work as I can hardly walk and can’t drive yet. Thanks for the positive video

@karencutler5444 hydro therapy actually seems like a really good idea. As far as things taking too long, I do personally know two guys who had this injury one was 20 years ago and the other was 34 years ago. Both of them were kept in a straight brace for 3 months and both have recovered completely. So keep your head up and you will get there.

Were you not wearing your brace at 6 wks PO? I'm 6wks PO and just had the doctor adjust the brace to 70 degrees. He said to only do that when I'm sitting. When standing he wants me to use the brace locked at 0 degrees.

@@cecilarchbold I wasn't wearing my brace at that point. My doctor wanted me to but I decided not to. I won't advise you to go against your doctors advice though so be careful. My understanding is most re ruptures happen between 7-10 weeks so going without the brace you have to take caution not to fall or anything.

Mentally it's on another level because even when you're progressing the progress is so slow. I've been injured before but nothing that set me back like this. The knee sleeve is just a copper fit l sleeve. I'm not even sure I need it but for now I'm wearing it for my workouts.

@@cebukid70 to be completely honest, on some level all the exercises make me nervous. Seems like a decent amount of people who rupture one end up rupturing the other one down the road. I'm just not willing to let that fear stop me from doing what I want to do.

@Ryanre-imagined-xe1vr in general it's an extremely rare injury. I read this morning that it only comprises 6% of all tendon ruptures and from what I've seen in terms of athletes, all pro athletes that have had this injury successfully came back. I'm a runner, by background, and my injury was from a freak household accident....never heard of someone getting this injury from running....also I've been lucky to never have had knee issues ever..bottom line we're both on our way to full comebacks 💪. My goal next year is to race a minimum half marathon... 5kTennis guy also said he made a full comeback so our chances are awesome 👌

@@OddOnesOutOOO No. My knee feels pretty normal with the exception of the tenderness under the kneecap when it's pressed and the numbness on the skin. For the first few months it felt like a rubber band I guess when I bent my leg.

@@derekjackson8201 I could do a straight leg lift within a few days of surgery. Don't get hung up on not being able to do it though. I have heard from people who couldn't do it for several weeks and in the end they were able to recover just fine.

When you say lift your leg straight do you mean relying on the brace to keep the leg straight and using hip flexors ( not tightening the quads)? Surely you should not be using your quads until the repair is healed (6 weeks)?

@@excellentbeginner1 I was able to hold my leg straight without the brace on pretty early. Contracting the quad is probably not a huge deal before the 6 week mark assuming you aren't using weight. I'm just a guy on the internet though so if your Dr. tells you to avoid it then best to listen to the Dr.

I’ve seen plenty of videos of people walking earlier, starting PT sooner, and a lot of folks online acting like they’re doctors. My best advice is to take your time and let your body heal at its own pace. I’m just two days shy of five weeks post-op, and while this journey has been challenging, it’s also taught me the value of patience. Initially, I felt frustrated when my doctor said I couldn’t start PT until week seven, but we trust doctors for a reason. I chose to trust the process, and now, at week five, I’m feeling great. The last thing you want is to push yourself too soon and risk reinjury. Wishing you a smooth and speedy recovery!

Hey Ryan great videos!! appreciate all the info you are sharing. Had a question, I have the same injury as you and I’m approaching 3 weeks post surgery in about 2 days. I was told to wait 6 weeks and keep the leg straight however from your video I’m hearing you were directed to start doing range of motion work staring from 2nd week post surgery. Can you let me know what kind of range of motion you were doing specifically at this stage from week 2 approaching week 3? and also wouldn’t waiting 6 weeks be too long? Thanks in advance

@@davidmulat at the two week mark I simply took the brace off while seated on a flat surface and would put my hands under my leg and lift to bend the knee. The other thing I started doing was sitting with my leg off the edge of a chair and putting a band around the back of my ankle and lowering it letting gravity pull it down and then I would pull it back up with the band. I think I talked about that in one of my videos but not sure which one. Could have been the 4 week video.

@@tindomania6367 I'm very close to the same ROM as my uninjured leg but not quite there. I am making progress but the closer I get the slower the progress is. When I was focusing on ROM I would usually get in a hot bath to get my knee warm and also massage my knee and surrounding muscles. Then I would do my range of motion exercises after that.

Man love your videos..your videos make me realize “aye it’s been another week already right on” So my 3month post op doctors visit is tomorrow. Super excited but nervous. On my 2month mark I had to slow down doctors orders because I was already doing wall sits, body weight squats at 40degrees, walking up stairs no hands and down with one hand on the railing, and lunge holds from my 4week mark.. It was a little discouraging when he(doc) said to still walk with my leg locked on my 2month doctors visit.. so I had to learn to be patient and not overdo it. I do have a question though please, If you were to tap the right or left side of your knee cap, not the patella itself but the soft tissue/muscle side of the patella, do you feel something a little off on the opposite side? Almost as if it was flapping??

@@lightz-in-paradise4562 I don't have the issue you're describing where something feels off beside the kneecap. I did read a post online where someone had that feeling though. As for your 3 month appt, hopefully you will get the OK to advance your workouts. It sounds like you're doing amazing. I know that my Dr seemed pretty conservative at first and then cut me loose at 3 months to do anything but running and jumping. My understanding is that the period between 7-10 weeks is where a lot of re ruptures happen so makes sense that he was concerned but you're past that now.

@@lightz-in-paradise4562 I haven't done anything at full speed like play basketball, but testing it out, I can pivot. My real issue is just depth and when my knee goes out over my toes. That is the position that puts the most strain on the knee so makes sense.

@@ss1mbaa since the injury I haven't really overloaded my knee. I did have one training session that I feel like set me back a few days. I generally have a little extra swelling and tightness the rest of the day after I train my legs. Also, I do still have some swelling around the knee. It has gone down a lot but it's still there.

Did your left quad catch up to the right? I've had my brace off now for a little over a week and walked 2 - 4 miles per day.. i haven't done too much resistance training yet but the walking brought up my quad a little bit.

@@ibgoliath8400 I don't recall exactly when I could go up and down stairs but I would say around 4 weeks I could handle them by stepping up with my good leg and then down with my injured leg. That's going slow. I was a little over 3 months before I cojld walk down a flight of stairs normally.

Good video man, thanks for the updates. These are giving me something to look forward to. I had a complete rupture of the tendons, so mine is about as bad as it gets. I'm on week 4 and I can finally stand on my leg without the brace and take a somewhat normal shower, and penguin walk around the house without my walker. I have my 6 week followup next week. I hope I'm good to start pt.

My knee is really stiff and my doc says it is scar tissue and may need another surgery to break that up/help with ROM. Did you experience any of this?

@@chrisreed3358 my knee was really stiff when I first unlocked my brace. I think when I was at 6 weeks I was only slightly ahead of you range of motion wise at around 85 or 90 degrees. I don't think you're that far behind. Maybe just need to start stretching it regularly. I remember only making the smallest gains on range of motion each day. It just takes time.

Hallo I am 4.5 weeks. I am making doctor meets every week. Every Teusday for me is a new Checkpoint. I can walk without help but not walk perfectly. My leg is super weak my muscles practically don't exist. I also have cut ACL and that's make my progress a little bit worse. Anyways tha k you for sharing your story. It's a big boost. On my 4th week I am at 70°.

At what point were you able to ditch the knee immobilizer? I'm on week 6 going into 7 and my Surgeon wants me to wear it for an additional X weeks...he was very non-committal on the timeline

@@cebukid70 I took it off at about 5 weeks. My Dr. wanted me to keep it on for 10-12 weeks but I just couldn't take it. He said he has had two guys re rupture in the 6-8 week mark so you may be dancing with danger if you take it off early. I'm not sure it was beneficial for me to take it off early either.

@@Ryanre-imagined-xe1vr Yes, my Surgeon was actually leaning towards me leaving it on "indefinitely" beyond the 6 weeks mark, but luckily, I had my PT appointment the next day and the PT said I was ready to remove it based on the fact that I was able to do 15 leg raises with no pain and no issues....this was probably the best news I heard throughout this ordeal. I swear I felt like taking the immmobilizer off and smashing it on a wall or a ground - especially around the 4 week mark...LOL. Since I removed it, I got my walking gait back after walking about 12 miles over the course of 4 days.

Thanks for all these videos by the way. I just hit my 6 week post-op mark on Monday 09/23 and very anxious to get back to my active lifestyle. I was able to free up my brace to 30° flexion and have a somewhat normal walking gait again (with brace on). My 1st post-op appointment with Surgeon is in 2 days and hoping that he gives me clearance to wean myself from this brace that I've really grown to hate after 7 weeks wearing this shit 😂. When were you able to walk again without brace?

@@cebukid70 I took the brace off sometime between 4 and 5 weeks. I went back for my follow up visit at 6 weeks and my Dr. really wanted me to wear it. I put it back on for a few days and just couldn't tolerate it so I stopped wearing it. That's not a recommendation, it's just what I did. I believe the time frame you're at right now is when you are most likely to re rupture so if you can stand the brace I would keep it on if your Dr is telling you to.

@@Ryanre-imagined-xe1vr funny story....I had my 6 week post op appointment with the Surgeon a couple of weeks ago and he was "biased" towards me wearing it for an extended period. He didn't say it outright but left it very vague and then said it's up to the PT's.. luckily, the next day the PT completely cleared me to take the brace off since I was able to do leg lifts for 20 reps very easily. I was very happy that day. Bottom line: The Surgeon will always want to protect "his work" and is not that interested in the duration of the re-hab. He probably just wants to err on maximum healing so that you don't come back with problems, or worse yet, a malpractice lawsuit..lol

Do you wear that knee sleeve regularly? Does it work well with stabilizing the knee?

@@cebukid70 for the first few months I wore it most of the day. Now I generally just wear it for my workouts or if I have some physical work to do. It doesn't really provide support. I initially wore it for compression to keep the swelling down as my knee would swell up when I was on my feet.

@@Ryanre-imagined-xe1vr Thanks for the info. I am at 9 weeks post-op as of today and I wear the support on long walks, but try not to rely on it during my workday. I noticed I feel pain when I let my leg "dangle" (like sitting on a high stool), and also during periods where I'm sitting down like a "normal person" and bending my leg. I'm finding that I have to straighten the leg out here-and-there to ease the discomfort, and like you, at this point in your recovery, it's still a bit swollen. 1 question I had for you......although I have better-than-average flexion (I'm probably already at 120°) it still feels a little tight....especially when I'm seated for a long time and get up. Anyway, thanks man. You're the only guy I really talk to who's had this injury. Thanks for listening and responding.

@@cebukid70 up until about 20 weeks or so after surgery I could only sit with my knee bent for so long before it started aching. After that it started to improve a lot and it still aches sometimes but I can leave it bent for long periods now without much issue.

I had a patella tendon advancement on my knee because my knee cap broke in half. The bottom part of my knee cap was removed. Im on week 5 and just started my pt. I can bend my knee only to 30 degrees without severe pain. I can walk with walker but get so tired. My pt says I need to be at 60 degrees by next week. Is she suppose to force me to bend my knee. I cant bend it anymore.

@@laurag8309 I'm not sure I would let a PT push you're knee too hard. Some pain is fine but there is a point where it's too much. I have heard a couple stories of people getting hurt in that situation. I would ask the surgeon. Worst case of you get to a point where if the surgeon is worried about it they can manipulate your knee under anesthesia.

I would suggest take most of what your PT tells you with a grain of salt. Most of them don't even have experience with this type of injury. I'm a year out from surgery, and I'm still trying to get my normal range of motion back, gaining maybe a degree or two every couple of weeks or so, if that. It's painstakingly slow. They're all programmed with this conventional wisdom that you have to be at 90° within 6 weeks or something like that, and in most cases it's just a bunch of nonsense. Just do whatever you can.

I’m about 13 weeks post op on my left knee!! Walking without the knee brace feels so good and without the crutches as well! However, I can’t still walk up and down the stairs.. and it hurts when doing squats or lunges, on the MPFL area (above the patella, to the right) Feel like I’m a bit far back and currently on 100 degrees, and it feels so hard to get full ROM. Hows the best way to stretch it? Love the updates btw!

@@ss1mbaa When I have discomfort in a deep squat or walking down hill it's in the MPFL area also. I have heard that often times that can be from a weak VMO. I'm working on some things to strengthen mine and I may even do my next video on that. For stretching, I like to get the knee and quad muscles warm first by taking a hot bath but a heat pad might work as well. I do think a lot of stiffness in the knee is just swelling and scar tissue and so some of it is just time.
