I had the surgery 16 days ago. in the recovery room I was screaming in agony. after getting morphene the pain went. since then the maximum pain has been 3/10 . and only occasionally . I started physio 2 days ago. got good movement . frustrating having to wear the support 24/7 but it was expected. going to be a long slow recovery again expected. had the big cuff, smaller cuff , bone ground down and balloon. was worse than expected. operation mr khan, London independent hospital. all the staff there were amazing.

What if it is a "complex" superior labral tear and I'm 34. I was told my bicep tendon is fine and my rotator cuff is mildly frayed. I also have great range of motion, but I can still tell something isn't right although my trainer and PT have said they don't believe I need surgery. The yhing is I power lift, Olympic lift and do martial arts. So in terms of longevity I am trying to figure out if I should get surgery or just stick with conservative treatment

When you hear “complex tear” it is usually not a good thing. That usually means there is a disorganized tear pattern seen on the mri. The only repairable type is a type 2 tear which is a clean and “simple” tear. However, it is hard to advise you just based on the information you gave me. Ultimately, you have to decide if this nagging pain is bad enough to warrant the down time and recovery period needed with surgery. Your surgeon can help you make this decision. If it comes to surgery, the repair vs tenodesis decision is often a decision that has to be made when the camera is in the joint. Good luck. Please share what you end up doing.

Dr. Sohn, Political views aside, it is my hope, and may I respectfully suggest that medical professionals such as yourself look into and shed light on "SIRVA" and brachial neuritis. I've been suffering for 8-9 months, and had no idea why my shoulders would be bothering me this bad. It wasn't until about three weeks ago when I remembered receiving the TDaP vaccine early Feb 2023. In addition, seems as though our government is already aware of this- hence, the Vaccine Injury Compensation Program. I can't begin to fathom how many others haven't connected the dots yet. I hope this helps others. Trust me when I say, the pain is real. Thank you for sharing your knowledge, and best of luck.

If you MUST use a brace, a wrist extension brace would be more helpful. It will support your wrist and reduce the tension in the wrist extensor tendons. Better treatments for tennis elbow include active release therapy or a PRP injection. My main concern with the brace is that it. A cause radial tunnel syndrome. That is a nerve problem that is often harder to treat than the original problem.

When it comes to stabilization surgery is the capsular shift like about 50 percent joint volume reduction/tightening versus the capsular plication like 30%? Does the plication only focus on the capsule near the labrum while the shift is more global?

Great question. in theory, you can do as much volume reduction arthroscopically as you can with an open capsular shift. In reality, we usually do not aim for as much capsule volume reduction when we do this arthroscopically since we are usually trying to simply restore the damaged anatomy. arthroscopic Plication in people with multidirectional instability is usually done circumferentially (front and back). In contrast, open capsule shift is done from the front and mainly reduces the volume in the front of the shoulder. When done correctly, the lower leaflet of the open capsule shift will pull the posterior and inferior tissue upward, tightening it indirectly.

So does the hydrodilation rupture the capsule just like the capsular release surgery would to gain ROM? And is the capsular release surgery basically a cut in the front and back of the capsule but the rotator interval area is where they cut the most out like a whole chunk of the ligament or that is also equivalent of just a cut?

thanks for the question. when I do hydrodilation, I do NOT rupture the capsule. It is simply a fluid bolus and an air bubble which allows more effective magnification of the patient's stretches. When we do the surgery, it is a cut in the capsule circumferentially as well as removing a portion of the rotator interval and any adhesions in the subacromial space.

Do you know if this ballon is available in Canada. I had my supersinatus repaired 8 yrs ago. But I now have a large supersinatus full thickness tear. I think the Ballon would be a good option.

Great question. I can’t find anything about this but I’d be surprised if it was not available. The main question in Canada is whether the healthcare system will pay for it or if you’d have to pay out of pocket.

I had it in November of 23 here in the United States, and now my insurance is refusing to cover it. They want the full amount from me out of pocket. Be careful if you decide to do it because the United States medical system isn't what it used to be!

❤thanx for this info. I took the flu shot and the area of the shot kepte in pain for almost nine months. A year later I went to dentist, reveived injection, went home andy shoulder in that area started aching as if there is memory in the cells. This kept on for half a day. Since 2021 May, Up until now I cannot properly lift up my arm.

Hello. It sounds like you mainly have a stiffness issue which is common after that length of time. The more concerning thing is when you are flexible when stretching but can’t lift up the arm with your own power. That usually means the cuff repair failed. Keep stretching!

Hi, The video is a great help. I want to ask that Is it normal to have a door hinge type of squeaking sound in the shoulder after a slap repair with knotted suture anchors ? Does that painless squeaking sound mean a failed surgery ? If so do you recommend a recovery surgery? If yes when would be the optimal time for it ? Thank you doctor

Hello. While I’m not able to offer medical advice on this channel, it does sound like you may have some abrasive rubbing coming from the knots (if they indeed used knots). Lately, most companies have gone to a completely knot less technology so as to avoid that situation. I’m some cases, the noises may dissipate as the body covers the knots with scar tissue. If there is pain, you may need to consider a surgery to remove the knots after the slap has healed. If they are going back to look at the knots, they will also see if the slap healed (and to consider bicep tenodesis if needed).

I"m fairly confident this is what's going on with me right now. I got the flu shot about 3 weeks ago and and I had the normal soreness the first 2 days but it's stuck around and has gotten worse. It's extremely painful to do something like take my shirt off. My question is, can I just wait this out to heal up or is it something they'd want to see for? I don't like clogging up the healthcare system with things I know they can't do anything about. Thanks!

I jus got an mri a week ago I found out I I have a type 2 slap tear with a proximal humerus fracture with a little fluid in my shoulder I been in pain ever since it happened on new years a month later I still can’t raise my arm or put my arm in certain positions doctors said physical therapy is not going to help😢. Does my tear sound bad ? and does it sound surgical ?

Hi biglionnnn. This channel is for general information. It’s hard to give you specific advice based on your question. However, a humorus fracture is a much more acute and important problem to heal from before addressing any problems with the labrum. It takes about six weeks for the bone to heal and then after that, it probably takes another month or two to get your flexibility back. It’s too soon to make any decisions about surgery unless the fracture itself requires surgery. I recommend seeing a shoulders doctor near you to monitor healing.

Just got a MRI with Fluroscopy. Just curious as to what this means and how bad is my shoulder is. I can still lift Ths is just the impression: Hill-Sachs deformity with nearly circumferential tear of the labrum. SLAP tear extending to an anteroinferior labral tear and chronic tearing of the posterior labrum. Tear extends to the biceps anchor. Mild bursitis and tendinopathy. It also says my posterior labrum has blunting, fraying and peeling on ABER view ?

Hi Joshuaroberts6891. Looks like a pretty bad injury of the entire rim of the socket. that is much more than a slap tear (although it includes a slap tear). I often see this type of result in my patients. It may be possible to rehab the shoulder a bit and avoid the things that hurt. However, if the pain persists, you may need to have a surgery to repair the damage. Good luck on your recovery from this shoulder injury.

Thank you for your reply! I’m a young very active person but haven’t been in pain. It just with certain exercises it pops out and then it’s really bad pain. So basically my entire labrum isn’t attached to the bone? Should I be worried about the tear extending to by bicep anchors ? Trying to see if surgery can be avoided. Tried pt but didn’t work. But I want to resume my usual activities such as bouldering

Thank you for this thorough explanation. I'm a 70 year old male diagnosed two months ago with a massive RC tear. My doctor at Beth Israel in Boston said it appears quite old with both my supra and infraspinatus muscles extremely retracted according to the MRI. Yet I didn't even know I had this injury until after installing a hood above my stove two months ago which, apparently was the straw that broke the camel's back. My doctor told me this balloon insert is not a total fix and that I would eventually need a reverse shoulder replacement. My question is, and I hope it's general enough to warrant an answer here: after the balloon dissolves and when/if the humeral head migrates upward again, why can't the procedure be done a second time so as to avoid the reverse shoulder replacement? Also, I am holding off on the balloon, or any surgery, due to the length of the recovery period and financial constraints. I have a mortgage to pay among other things. I can still work as a massage therapist without any obvious problems so that's what I'm doing saving money for the rainy day that's coming. But if I hold off too long and get arthritis in the joint, would I still be a candidate for this balloon surgery? These are questions I neglected to ask my doctor and he's not someone I can just pick up the phone and call for a quick response. I have other concerns too, such as If I fall into the percentage of patients who don't do well with this balloon procedure and end up worse off, needing a replacement which has a much longer recovery period. Sorry to lay this much on you! I would surely appreciate your input. Of course, I will eventually speak with my doctor. Thanks again.

Hi jpd464. Great questions. So yes, it’s usually a gradual weakening of the tendon, which eventually gives out. There is usually some sort of memorable injury that finishes off the tendon, but the tear was likely growing in size for quite some time. I always use the analogy that if the rotator cuff has been torn for a long time, the humeral head starts to migrate so far upwards that it starts to escape from the socket. it’s as if you drove your front wheels of your car off of the edge of a cliff. You don’t have enough traction to get yourself back onto solid ground. If you were to just get a tow truck to pull you and get your wheels back on solid ground, you could keep them there. The balloon acts like the tow truck, which gets the humeral head to center itself again in the socket. Even after the balloon disappears, the humerus should be able to maintain itself inside of the socket. I don’t think there is any reason why it couldn’t be repeated. However, this device hasn’t been out long enough to have any reports of that being done. as far as waiting too long and your concerns about arthritis, that is valid. However, arthritis forms slowly and the type of arthritis that happens with chronic rotator cuff. Tears is more of an asymmetric socket wear that is often relieved by placing the balloon spacer. One final point is that a reverse shoulder replacement is a bigger surgery, but the recovery is actually quite fast.

@@jpd464Generally, after about 6 weeks, people are back to normal activities with some ongoing stiffness and soreness. That pain seems to go away around the 10-12 week mark.

I was fortunate enough to be referred to Dr. Sohn for my frozen shoulder, and I am extremely grateful to him for his expertise, compassion, and care. I diligently followed his prescription of physical therapy and injection, but, although this works for most, it just didn't stop the rapid decline of my very painful 67-year-old shoulder. In November 2022, I couldn't even raise my fork to my mouth for Thanksgiving because of the pain. Dr. Sohn recommended manipulation under anesthesia, and that procedure completely restored my quality of life. Within a very short time, I could easily raise my arm and move it to my back and stretch it out to the side. Before this manipulation, he very carefully and clearly explained the procedure and discussed the risks. I felt completely comfortable in his care and remain so grateful to him that, upon seeing this video, I felt compelled to thank him again by way of this 'comment.'

Thanks for sharing your comment. I have seen that happen in one of my patients as well. If you read the information about the balloon, there are about 20% of people who do not benefit from the surgery. 4% of people who had the surgery also felt like they had worsening after the procedure. In those cases, a reverse shoulder replacement may be the best alternative. All the best on your recovery.

Im not 100% sure but since i started receiving C19 vaccines from 2019 until 2022. I have had an issue with my left upper arm which i had all 4 injections in. I get random muscle twitching and also if my arm is raised and/or turned outward at the shoulder i get a dull ache that feels to be deep in my upper arm. My right arm seems completely fine but i haven't had any vaccines in that arm

Yes. A slap can often be seen on a regular mri. However, some can be missed. An arthrogram is still the gold standard test.

Mid anterior-inferior labral tear extending from 3 o'clock to 6 o'clock. 2. Borderline subacromial impingement with mild anterolateral downsloping of the acromion causing trace subacromial/subdeltoid bursitis and tendinosis of the distal supraspinatus myotendinous junction. 3. Distal supraspinatus, infraspinatus and subscapularis tendinosis. Low-grade interstitial delamination of the distal supraspinatus fibers. 4. Severe acromioclavicular osteoarthritis. 5. I received the flu vaccine 12 days ago. The pharmacist hit my bone and injected without withdrawing. I knew about “too high” injections, so I’m always careful to remind the injection person not to go too high. Now I know about too deep. My pain has persisted and spread to my forearm. It wakes me at night multiple times and Advil doesn’t touch the pain. I’m hoping it will resolve but after almost 2 weeks it’s getting worse. SERVID VIDEOS should warn providers not to inject if hitting bone. I was always trained to back up a few mms if I hit bone; the pharmacist just injected. Arrgh.

Oddly, you don’t need another balloon. I think the balloon is replaced by a scar tissue after it dissolves. Also, there is a component of the deltoid being retrained while the spacer is intact. It may give credence to the idea that physical therapy may be able to do this without the ballon…

Isn't the point of it to replicate the subacromial space so it can be used for no subacromial bursa but intact rotator cuff. so why does it need to be used for irreperable Supraspinatus tear? If the bursa is not present then the humeral head would ride up too correct? If balloon implanted and when it dissolves it leaves scar tissue but is that's scar tissue basically the subacromial bursa?

If I understand your question, I would say that this is NOT just meant to replicate the subacromial bursa. The bursa is a very small filmy tissue which normally takes up very little space. The balloon is meant to fill the space normally occupied by the rotator cuff tendon. If the tendon is intact, there is no room for the balloon. So, it’s only meant to be used to open up the collapsed space with irreparable rotator cuff tears.

Hi. When the capsule is weak or attenuated, we have a few other options. In general, we can stabilize the shoulder using a dynamic tendon transfer called the DAS (dynamic anterior stabilization) or the trusty Latarjet procedure where we transfer bone and tendon to reinforce the area. We can also use dermal allograft to reconstruct the weak tissue.

I still have a sore shoulder, even though my vaccinations were over a month ago. The pain is not severe, just persistent. And is does get more noticeable everytime I move my arm/shoulder certain ways. I really feel stupid making an appointment over a persistent condition that is more annoying than anything else. Should I, in your opinion, make an appointment about this?

Yes. You should ask for a referral to physical therapy. If it continues to bother you, you may need to also get an MRI. Your doctor will be able to advise you much better after examining you as well.

Liked your video. How about a suprespinatus tear and a metal humerus? Dislocated shoulder 7 years ago and repair 3 years ago due to necrotic humerus head. Recommendation was reverse repair. I at least liked you said that hurt less because shoulder pain is no joke. Horrible. Thanks, Lisa

Hi Lisa. So sorry to hear about your shoulder pain. It is hard to advise you without seeing your X-rays. However, I’m general, the reverse is the usual next step. However, I think the balloon may be with a try. If your pain is improved, you may not need to have the reverse now. I think it’s worth a try but that would be considered an off label use.

Hello I have small rotator cuff tears nearly to medium, 2 of the supraspinatus tendon and one of the subscapularis tendon. Will I be able to swim do freestyle and my full time cleaning in the end? I don’t think we have the patches in nz. Thankyou for this video.

Hi Michelle. I mainly use patches for partial thickness tendon tears. In your case, it would probably not be necessary since the tears sound like they are full thickness (albeit small). Yes, you should expect to have full athletic activities after the repair and recovery. it takes about 5-6 months to get the results.

I’m very Sorry I found my mri paper. It is partial thickness articular surface tear of the posterior fibres of the supraspinatus tendon measuring 6.5mm in AP x 1.7mm in depth x 8mm in length. And partial thickness articular surface tear of the mid to anterior fibres of the supraspinatus tendon measuring 8mm in AP x 2mm in depth. A low grade partial thickness articular surface tear of the deep and distal fibres of the subscapularis tendon measuring 4mm in length. So will I go back to full athletic activities eventually please. Was told no surgery. Thankyou very much.

While it seems to work in a very specific patient population, ream and run has not caught on with most shoulder surgeons. I’ve found that the pain relief was not as predictable with ream and run. With new options like the inset glenoid, I’m even less likely to recommend ream and run to my young patients. The main champion of ream and run is Fredrick matsen at university of Washington.
