Is the issue most likely impingement if there is no current tear shown in an MRI but pain/inflammation occurs after overuse?

Yes. That is usually the case. If there is no damage noted on the mri, it hasn’t progressed to the point of a partial tear yet. However, I would make sure you have the experienced eyes of a shoulder specialist looking at your mri to make sure that is the case. We often find damage in the tendon even when the radiologist says it is normal. The other thing to consider is bicep tendinitis. That usually does not show up on an mri but will often cause pain with use. See my video on that topic.
