I watched an Audiology Associates video right before I watched this and they said something about making sure that the person that they get their ears cleaned by has the correct letters after their name. So they are concerned about this as well even though they aren't saying it the way you have in the past.

As someone who works in the US healthcare system in the finance area, I agree wholeheartedly, but until we get rid of the the FICA wage ceiling, we can’t do anything else, and even then, all of the government healthcare programs that are handled at the federal level (Medicare and Tricare) are 100% administered by insurance companies anyways, with Medicaid getting up there in a lot of states. Sadly, we won’t be able to get the insurance companies out of healthcare, even if the federal government expands Medicare or anything like that.

Hi Neel. Appreciate the back story. Happy you were able to help him. Years ago I had a blockage whilst in holidays in Switzerland. When to the local doctor and he used a metal clunky looking instrument that sprayed water in my ears and got a huge bullet like black wax out. It didn’t hurt but I’m so glad I didn’t get an infection. Keep safe Neel. Xx

This is why I won’t go to my dr about my tinnitus in my left ear. I’m saving to come and see you this year from Hemel Hempstead. You are always so considerate of the patient and exceptional at your job. I don’t trust anyone else near my ears. Not saying there isn’t other audiologists who are exceptional who live closer, but I’ve been watching your videos for years and I’ve never, not once thought you was too harsh. I used to work as an ED tech, and we did ear irrigation. We literally had no training other than verbal instruction from the provider. We would use a 30cc syringe with an IV catheter, and a mixture of warm saline and peroxide. So this is so interesting to me.

U.S. Pediatrician here (currently working a variety of jobs in several fields... including working as a pediatric hospitalist... in an effort to stay sane in this our system). A lot of the differences between the US and the UK in terms of cerumen removal come down to our rules about billing and documentation. I have to document specific things for it to be a billable service. If I don't document it correctly, I don't get paid. It has to meet certain criteria to be "impaction" and I can only bill for removal of impacted wax. There is a reimbursement code for lavage and for curretage, but not for suction. Depending on who is reviewing the claim, they may say that it's covered under "instrumentation" (curretage), but they may not. Meanwhile, my employer may be telling me I need to see a patient every 10 minutes. My nurse (rarely an RN in a clinic setting) can lavage an ear, they're not allowed to do anything else. Guess what we end up turning to 98% of the time... the thing I can have the nurse do while I'm in with the next patient.

I remember taking a client to see the doc. He used warm water and a water pick to clean the wax from peoples ears. The amount of wax he was able to get out was amazing. It just kept falling out of his ear or I should say trickle. Big chunks at times. Didn’t seem to bother him while it was being done.

How dare they knowingly let untrained people to well I would say disguise themselve's as accredited audiologist to poke about in someone's ear,take their money & not give it a 2nd thought-shamsefull. In the US, microsuction is unheard of. If I want it done, I have to make an appointment with you or Rhys and fly out from Seattle. It’s very frustrating how difficult it is to get quality healthcare at times in the US.😢

What I find so disgusting are people claiming to be able to have their ear's made clearer for them is the fact it is money driven by private hospital's or pharmacy staff. I would NEVER ever since learnt by you is only go to qualified audiologists. Thank you Neel for going down the right path & all the reason's why. I could never thank you enough ❤

I'm writing and unofficial biography about Neel. Here's what I have so far. At age 4, during pre-pre-kindergarten, Neel spelled the word "ear" for the first time. At age 6, Neel developed an extensive list of all the words that rhyme with "ear". At age 9, Neel was overcome with the thought that if only people listened to each other better the world would be a more peaceful place.

The company may still be selling thw old metal syringes, but i havent seen one used in the past 25 years. So i dont think they are being used as much as you may think they are. While GP's may use irrigation to remove wax, ENT's never use irrigating for wax removal. I worked with a leading group of ENTs in the mid 1980's, and even back then non of the 7 doctors in the group used any kind of irrigation to remove wax. I have since then have been a patient to more than a few ENT's and non ever had irrigation set ups present for wax removal. I was a medical assistant and the i became an RN and i have used irrigation for wax removal when working in a GPs office, that was over 25 years ago. I would actually refuse to peform that procedure now. Ive learned more in recent years about irrigation and wax removal. This has only been my experience and im sure others have different experiences. As you know, the USA is a very large place as compared to the UK and every states have different regulations, aways under the guidence of the FDA.

This NEVER should have happened. This lady ended up really suffering. I guess in the fight to stop this new regulation from continuing on all you can do, and the other audiologists can do when they encounter these issues is document, document, document. If the patient even wants to contribute to the documentation, even better. I’m so sorry for all you’ve been through with this. You would think that a governing body would sit up and pay attention to someone with your knowledge and experience strongly maintaining that what they’re going to do is a very bad idea. These are ears! They’re important to our quality of life. As well, God forbid something happens and a person loses their hearing. Thank you for your caring and professionalism.

I got my ears irrogated (or should I say flushed with water) with a metal syringe at my gp's office last year. It was a bit painful (the nurse poked the canal wall a couple of times) and it seemed quite ineffective as she had to fill the syringe four times before anything came out of my left ear. I cannot say that my hearing improved noticably afterwards and I also developed some crazy itchiness in both ears. 😑 I live in Sweden btw.

In the US, a small bulb is sold as a standard piece of baby equipment. I found it useful to clear my daughter's nose, but it never got near her ears. Also, I have had irrigation in the past for ear wax, just once. Warm water jet, made me a bit dizzy, but it brought out a good sized bean of wax. Have not even heard of anyone using suction. A great pity...

I always thought the bulb was to be reversed. Apply oil or whatever drops into the ear. Then compress the bulb and use the release of the bulb to section what's in the ear out. I care for a lot of people with disabilities. And they are the last people that anyone looks in their ears. Almost no one is willing to treat them. Even the e n t's give up without even trying.

You do realise he gets paid per ear/per patient, so obviously the more patients he treats then the more money he makes. Why would he draw out appointments, and thus treat fewer patients. Is that too difficult a concept for your two brain cells to comprehend? Neel takes as long as he needs to clear each patient’s ear/s to his and the patient’s satisfaction. Still, it’s better that you mute the sound, you probably wouldn’t understand what he’s talking about.

Yea...when I was a kid, I got swimmers ear like a mfer. And my pediatrician would use a metal "syringe" to blast my ear with water. I would scream like a banshee, but it didnt hurt. I was more scared the 1st couple times. As i got older, i go a little more accustomed to it actually felt kinda cool.
