Anything done needs to first address the propaganda war. Some sort of rule on PR campaigns, journalism articles, news stories from pundits, etc, about labeling the source of the funding for the message in anything related to medicine. Like a calorie label, only the requirement being it takes up X percent of the message so it's not some subtitle you never see. Because, even if conditions are made that are incredibly scientifically sound? The PR war from big money will say exactly the opposite with at least half the population swallowing the message. There's way too much money in the hands of these groups that will be spent fighting good policy if said policy contradicts their interests.

I learned about the connections when I was a history student studying the changes that were made when the billionaires were the ones building the hospitals and universities that trained the medical professionals in the early part of the twentieth century. Many of them saw the potential for medicine to become a business rather than a service to the community and were often the investors that had caused the need for the hospital to treat the steel workers, miners, farmers, etc from the chemicals and such that they were promoting.

One more, doctor’s should have to share all treatment options. As a patient with epilepsy, after several failed drugs, I found the ketogenic diet. Doctors must tell patients of the medications and nutritional effectiveness. This goes for mental health, T2D, autoimmune diseases. Doctors can’t turn a blind eye and they must be open with their patients.

Both of my grandpas were MDs and i remember as a child then being horrified when the ad marketing advertisements began to be allowed on pharma products. The system needs complete reform to restore trust and only when trust is restored can we make Americans healthy again. It's monumental in scope, but really quite simple. Love your thoughts, Doc, on this and all that you've shared over the past years. I would add one thing to your list...perhaps selfishly, as a farm owner. As of June 11, 2023, the FDA changed the regulations regarding animal care medicines. We can no longer buy necessary animal care medications (primarily antibiotics) for our sheep at the local farm store. Now, those require a veterinarian's prescription and the regs require that the "...veterinarian has recently seen and is personally acquainted with the keeping and care of the animal by examining the animal or by medically appropriate and timely visits to the premises where the animal is kept." We don't even have a locally-available large animal vet, so we're SOL in being able to treat pneumonia and/or infections.

You are spot on about drug advertising on TV. Ever since the government made the stupid decision to allow it, it's been terrible for the public but a boon for drug companies.

The whole fee for service business model, "manage" don't prevent or cure, is a giant foundational conflict of interest in the healthcare system. It incentivizes repeat customers and penalizes fixing or preventing problems. On top of that you've got a big important industry that is largely governed by insiders, which is why the American Dental Association for example has never even suggested the idea of regulating sugar/junk food. The crazy thing is it would probably cause a Great Depression to actually fix it since healthcare is largest employer. To offer a solution, we have to align the incentives with the right health outcomes, meaning reward prevention and cure instead of penalize it. And of course some outsiders on chairing the various medical specialties.

Hello Vinay, I am a pharmacist and have been watching your content for many years and agree with most points about our medical system as I have seen it first hand in my practice. I am originally a Bernie Sanders supporter who voted for Trump in 2016 due to the DNC awfulness, but voted Kamala because even though the status quo is bad right now, I wasn't particularly impressed with his first term and didn't agree with many of Trump's policies and rhetoric either he stated out loud (Jan 6, "won't need to vote anymore after this one", "poisoning the blood of America", deporting millions, abortion, etc) or layed out in Project 2025 (which he says he's not following but who knows). Now that Trump has won, I am hopeful that he brings minds like you in to enact real change that fights corporate interests and is focused on bettering people and their lives. I am still hesitant that this will happen, but I am hopeful. All the love, Matt

Curring his first term I liked that Trump wanted to have posted prices at medical facilities so people could compare them. Second he also wanted intellectual property reform. I think this would go a long way for prescription medicine not costing as much.
