Proptosis was so detrimental because your range with proptosis is directly dependent on shot speed. Tears damage are based on their amount of time in the air, so once they have been fired they only exist in the air for a set time, so with a shot speed like 0.6 it makes them go extremely short.

Fun fact, Sin, the spiders at 17:35 will never hit you if there's an even amount on the same wall and you stand still, but your tears will hit them. You could've gone to the front of the ledge and never gotten hit because the spiders would be shooting directly to the left and right of you.

I mentioned this in one of the comment threads but at 55:00, the issue appears to be related to how the game is coded to display the numbers. Sin got hit, dropping his chance down from 100% to 1%. He finished the fight a second later and instead of overlaying the "-1%" over the "-99%" it just added the numbers together instead.

Rotten hearts count as a half heart in the womb and beyond. So if you get hit you lose the rotten heart and the next half red heart or next rotten heart.

I feel like familiars like little Steven are good in cases where you start a run with low damage. The damage of the familiar is relatively higher than usual in comparison to your base damage the lower you start with. Considering you also had really low range and shot speed it’s essential to have some method of reaching further enemies faster. Once your build overall starts getting stronger you’ll start to see lil Steven “interfering” more, but I would have personally taken it as it probably had better dps alone then how you were going initially.
