Why would anyone want to take a partial agonist for pain instead of a full agonist? What's the point in taking something that only hits a portion of your pain receptors when their are plenty of drugs that will hit all of them? I've been taking Suboxone for about 5 years now. I used to take 12mg worth of films every day. I am now at 2mg of films every day. Idk how to explain it, but it seems like the longer that I've been on this medicine, the worse my pain gets.. I started having REALLY bad pain at the age of 25 for some odd reason. Even though I have gotten down to 2mg a day now for a couple weeks, the pain is still the same as it was when I was taking 12mg a day.. I don't understand it honestly. I HATE taking Suboxone and I want to get to the point where I don't take it anymore, but I hurt SOOO bad within 4 to 6 hours of not taking the next dose. It seems like the longer I've been on this medicine, the shorter the effects last. I'm trying so hard to get off this but there are days where I'm hurting so bad that just moving takes my breath away. I truly hate this medicine and I truly hate having chronic pain. At times I feel so hopeless 😔

I forgot to add that I sleep as much as I can so I'd have less hours of hurting time to deal with. This medicine and this pain has kept me from doing so much in my life. I feel like I've missed out on so much because of the medicine's and my body's restrictions with pain.. I hate it!

@g.e . I can understand what you are going through. I have been taking Belbuca for almost 10 years to help with cronick hip pain . The medicine helps a little but, not take the pain all the way . I am aware about the damage that this medicine will cause to the body in long term but, I can't stop taking the medicine because stopping will means to die slowly hurting . I pray to God that one day this pain will go away and I will finally have some release from this horrible pain. I wish the same for you. I hope 1 day you will be able to be pain free. There is good news though - in Heaven, there is no pain, no suffering, no sadness and no death. So that's something I personally try to look forward to.

I feel this. I have been on hydromorphone 2mg x2 a day for the last 10 years and it's not working anymore because I've been on it for so long. My doctor wants to switch me to suboxone but I'm scared to make the change because I feel it'll work even less. I don't even know..I'm so sick of hurting all the time

in the Europe there are transdermal patches with much higher dose of buprenorphine. The maximum dose is 140 mcg per hour., (3,6 mg a day) It is comperable to fentanyl patch. Transtec 35 mcg (buprenorphine) is the same as 25 mcg/hour of fentanyl. The highest patch put into the body 70 mcg/hour. Eqipotent to 50 mcg Fentanyl. Two patches at the sime time is the maximal dose. You can easily rotate to 140 mcg/h of BUP from 100 mcg/h Fentanyl. Buprenorphine is very usefull and strong opioid.

Thats great and all for ADDICTS that are trying to get off heroin and straighten out their lives... but i think its VERY WRONG to offer tgis medication as a PAIN RELIEVER option to CHRONIC PAIN PATIENTS.. For 1) Chronic pain patients are a very DIVERSE GROUP with different levels and different QUALITY of pain they live with. For example, one patient might have a bone condition that causes long duration of dull achey pain, and a LONG TERM EXTENDED would help them.. though they shoukd still have -on hand- meds that help immediate pain. Another patient may have dull achey pain coupled with sharp traveling pains (like up the leg) throughout the entire day... or sharp pain in the joints when they walk.. in that case an extended release does not help.. they need round the clock immediate release meds like oxycodone, every 4 hours. This drug is not effect for most chronic pain patients abd its very wrong to FORCE PATIENTS onto this as their only option. For someone like me (I have Ehlers Danlos Syndrome).. i need both. Extened release morphine that works all day long.. and immediate release opoids for the sharp pains in my knees, calves, shoulder blades, left hip and neck. Its not fun. And its NOT FAIR. IM ONLY 41 and without opioid therapy id live a sad life in bed 24/7. I was born with a condition that cannot be fixed, its a genetic mutation in the gene that makes collagen.. which is like the GLUE of your body.. and it hurts daily. Im tired of chronic pain patients on opioid therapy being DEMONIZED. ITS VERY WRONG AND THE CDC LIED ABOUT THE STATISTICS THAT DRS CREATE ADDICTS!! THAT WAS A DAMN LIE BUT THEY DID IT ON PURPOSE BECAUSE THEY WERE IN LEAGUE WITH PHARMA COMP THAT MADE 'SUBOXONE' for addicts.. problem is.. heroin addicts werent exactly getting in line to get clean with these meds.. so they started LOSING BILLIONS and needed to find a DEMOGRAPH to sell it to.. they went after the LOW HANGING FRUIT! DISABLED PEOPLE!! ME! They decided the only other "demograph" using opioids they could PROFIT FROM is the chronic pain patients.. and thats when they concocted their plan to FALSIFY STATISTICS ON "ADDICTION" so they can then PEDDLE ADDICTION MEDS (SUBOXONE) to patients.. please THINK ABOUT IT. I have stage 3 crps am wheelchair bound. It's the worst pain on the national pain scale for 13 yrs I've tried everything until I got on a regimine that I've been on for 6 yrs. Now my doctor retires and leaves me without meds, records, no doctor wants to see me and the doctors that will don't want to prescribe me what helps...fentanyl, morphine, soma. They want to take me off everything. I've went everywhere in texas and cali I have 2 days of medicine left. My husband and I are believing in a healing breakthrough. I run a non profit and with the meds I only get out three times a week and have to recover the rest but I have a life. I can't even sleep and will be up 24/7 for weeks without medicine, ending up in hospital due to the pain. I want to fight this with you and many others. I'm going to the governors office monday and contacting news stations next. Something has to be done for chronic pain patients like us. It's great for addicts but wrong for those not addicted and just wanting to live lives. Chronic pain sufferers are a very diverse group as you said. Like you, there is no cure medically. We believe God will heal me, and sometimes He uses medicine and doctors to accomplish that. I've tried spinal chord stimulator, nerve block, ketamine infusion, lidocaine infusion, acupuncture, Physical therapy, massage, they want to put me on a pain pump when I've heard terrible things about those and suboxone since my doctor retired. I've never had an issue until past 3 yrs and I can't see any doctor who will continue my regimine in those two states. I've never had issues before. I've had every test imaginable many times every year, no red flags, etc. I shouldn't be lumped into a category of someone who is addicted because I want to live life and this has been what has helped after many years of struggling to find what helps and this condition only worsens, is the worst pain on the national pain scale and called the suicide disease. I've even been told to do assisted suicide by doctors!!! No I want to live!!! I'm praying for you and for all that are suffering for strength, comfort, peace and healing. Don't give up! Well said. Plus, dental decay from subs is real and expensive. I say no for pain management unless you are OAD. What a mess, we need consistent treatment from docs, especially if you have a retiring doc.

That's a fair opinion, I also have a genetic condition I was born with. I have severe hemophilia B, which is a bleeding disorder and relatively rare. Unfortunately in its severe form it means I can get spontaneous internal bleeding pretty much anywhere for no reason if left untreated but normally it happens in my arms or legs. Let's just say due to a bad situation I was subjected to in my childhood I had very little to no access to the medications offered to people with my condition, that being replacement clotting factor IX. Because of this I had many bleeds happen in my knees and developed hemophilic arthropathy in my right knee making it extremely painful for me to try and walk or to even move that joint. It creates a constant amount of pain and I was put on many different pain medications for it, the only one that has been helping me significantly has been the buprenorphine. I know that may go against what your saying and that doesn't mean I'm disagreeing with you, people shouldn't be forced to take any pain killer they don't feel is helping them. My pain is probably nowhere near the scale of what you or others in this thread may feel but I just wanted to say it actually has helped me.

My mother has CRPS, it makes me so upset that people like you and her are forced to feel such tremendous pain. My mom has a good doctor, but the biggest problem we've had have been the pharmacies. They keep dropping her because she's forced to pay out of pocket for her medications and they don't like people doing that for opioids due to some idiotic CDC guideline. I have spent hours on the phone calling pharmacy to pharmacy trying to find ones to get her meds filled for her, I found one for her but pharmacies only seem to put up with it for so long...

I am in tears reading this. I get it. I too have chronic pain caused by an inability of my kidneys to expel uric acid. This condition was misdiagnosed over 30 years ago, and I underwent 4 unsuccessful surgeries and given every kind of pain meds known to man. You all know the Rollercoaster. I'm 62 now, and a new doctor very quickly diagnosed me with...GOUT. Manageable with Allopurinol. But for 30 years those uric acid crystals sat in my joints and caused immense pain and PERMANENT DAMAGE THAT CANNOT BE REPAIRED. Butrans has been a lifesaver for me, but I'm at top dose now.

Bup doesnt help with pain and caused a major issue for me. I had a major medical event and couldnt get actual pain relief, opiates. This is a major problem. I have small fiber neuropathy. Mu body is shutting down and malfunctioning. I also amso sensitive i vant take chemicals. Ive had to stop all the chemical pharmaceuticals. Side effects and didnt help at all.

Great analysis of the substance. I am a student in pharmaceuticals and a previous full agonist opium addict. It is frustrating how many pharmacists believe it is the same, or interchangeable with other full agonist opioids. Some aren"t even familiar with the term 'partial agonist' . Here in the UK it is the brand 'Espranor' that is most commonly prescribed for buprenorphine trearment. I believe it is much more bioavailable than suboxone/subutex. Anyway, great video.

A pain clinic put me on Fentanyl 50 mcg patch 15 years ago for back pain. I had been on oxycontin prior. I have slowly weaned down to 12 mcg patch over the past few years. With every decrease it was difficult mostly at night. I recently got a new pain nurse practitioner who decided it was time to go off the fentanyl. With no help! I found a new pain Dr. who prescribed Belbuca. I took my Fentanyl patch off tonight and will start the Belbuca tomorrow when I start feeling withdrawal. I'm terrified! Its very expensive and I'm hoping I won't have to take it for many years. Thank you for the information!

When a person is getting sensitive with 100mcg fentanyl patches (after 16 years of smooth sailing) and withdrawing for three days every two weeks.... what then? Is that even fixable? Codine helps a bit but it'd be good to have a better solution or even a better partial solution...... I just dont want to rock the boat with the doctors. Its hard to know what to do... Any help very much appreciated...

You say it has a high affinity to the opioid receptor at the beginning of the video which makes it near impossible for analgesic effects from other opioids, then at the end you state it's a partial agonist which allows the ability for acute pain relief, preferably for short periods of time. I'm so confused. That makes absolutely no sense. To me it can be either one or the other. Is anyone else confused by this? So will something like ibuprofen or paracetamol offer pain relief on top of it, regardless of how this drug works?

Affinity to the receptor is one thing. Partial or full agonist is another thing.

Ibuprofen and acetaminophen will always offer relief no matter the other medicines you take cause they're not opiates and therefore they don't work attach to opioid receptors as their mode of action.

Affinity is how tightly it binds to the receptor. It binds tighter than most other opiates, so if you take other opiates they can't knock it off the receptor. Hence the blocking effect (particularly at high doses > 8-16 mg). The respiratory depression and euphoria have a ceiling effect but analgesia does not. Which is what makes it such an ideal opiate for both pain relief and addiction

I've been on butrans and they've given me opioids for breakthrough pain. Buprenorphine is an agonist and antagonist in the receptors, but it doesn't totally block. It makes it so there's a ceiling effect, which makes it harder to overdose but not impossible, especially for people new to opioids. If you take butrans and an opioid, for example, it'll still work and help pain. Personally, butrans was enough for me. It worked 24/7. It felt like I was finally back to a normal body with very little pain on my worst days and no pain most days. Edit: buprenorphine isn't for acute pain. It's for pain that lasts more than 6 months. I have chronic pain.0

I've just commented asking the same thing before reading yours, it doesn't make sense. To me she contradicted herself in this video. From what I've seen/heard/read elsewhere, it's near impossible to be treated for acute pain whilst on buprenorphine. The med has to be VERY strong and a whopping dose to kick it off the receptor. Ketamine for example. Also not many people are experienced with the drug in an ER setting. You want to hope there's a pain specialist on duty or anyone in the field who is really going to understand the situation you're in when on this medication! A paramedic for instance arriving on scene would not have the pain relief to sustain you for a major injury until you got to hospital. You have to remember, what you're on is your base line so any injury above that, is not going to cover it. It's a scary thought!

I take 60 Vicodin a month for retroperitoneal fibrosis, nine back compression fractures and arthritis for bad pain. Unfortunately three a day only last me 20 days. I don’t have any withdrawal symptoms on the 10 days I don’t take them. Wonder if this would work for me

No cause it blocks the receptor for opioid. It's basically like Tramadol or Suboxone or worse Nalaxone. It's medical uses are still being studied Let's be clear You need to have your doctor increase the number per month so you get 90 or else you'll be in pain sometimes which is why you have a doctor for pain. Not taking it on off days may be fine but if you go past 24 hours of no opioid you will go into withdrawal and that could cause a number of unwanted effects including a dirt nap. K. That said talk to a doctor about the lack of medication you have been subjected to Life is short and pain is a choice with modern day medicines It's honestly usually a choice doctors make for patients they do not care about. So be sure n tell them 1 3rd of your meds are not there each month n that's cruelty

That said I agree op should speak to Dr. No reason to be out of medicine one actually needs for pain. They might prefer something long acting. There are lots of options and it kind of depends on the person what will work best.

Be careful asking for MORE. Yes I agree that it’s cruel to not treat pain in this day and age, as there’s ways to monitor for abuse too, but with the current climate on opioids, asking for more might not be taken positively by your medical doctor. They might consider it drug seeking. All pain complaints are essentially drug seeking these days. It’s gotten out of hand. Hope you’re able to live life as comfortably as possible considering your health situation. Perhaps there’s some non-drug alternative you can try for the days you dont have any? Or, maybe try 2/day all month and ask if taking Motrin or natural pain supplement as the 3rd daily. Best wishes.

I’ve been on Percocet for 12 years after a back surgery and having chronic pain due to 3 chronic pinched nerves and the doctor just gave me this with a lower dose of Percocet, I am afraid to take it… I know what works best for me and it’s Percocet Why would the doctor give me this ?? I tried this and had a bad experience at the second 15mg patch but I did not have any explained expectation of adverse effects like my hives and crazy itching elbow joint. So I thought I had a reaction to an Australian white tail spider bite which were known to be in the yard I was digging in at the time. I then went up to the 20mg patch and had an episode of very bad brain fog. A mind is a terrible thing to loose! Been told I had too high a dose for my size / weight. But at the 5 and 10 I had very little relief. I have postherpetic neuralgia from shingles (PHP). Not sure what to do next.

My doctor has put me on Buprenorphine 10mcg/ hour. It’s been 2day& I’m still in alot of chronic pain should I call my doctor & let him know? I was thinking about putting another patch on my other arm to see if it will work better cause I don’t see or feel no relief

Don't know, they started me out on Buprenorphine patch 5mg, Not really strong enough, starting 7.5 and will see if that will do it. Used kratom for years but masking the pain and doing heavy activity only made my stenosis worse til the point that stronger pain medicine has to now be used. Hopefully a specialist can fix the problem and be done with it.

I have been taking bupe since 2008 and it works well without making me a zombie. I'd add that because I have been on massive amounts of regular opiate medications, I need a much higher dose then a transdermal. After the years I am taking 8mg four times a day

I have a belbuca film 600 MG. Everytime I take Suboxane I go through presumptive withdrawal. I puke, piss and shit my pants. I don't wish that feeling on my worst enemy. I am afraid to take the belbuca because I don't want to go through withdrawal again. However I here the Belbuca is different than a Sub due to not having a blocker or something. I am still afraid to take this thing. I'd rather take methadone However there's a long waiting list, how did my life get this shitty? It all started with a car accident. Torn ligaments and ripped tendons in my spine. I was prescribed oxytocin 30mg. One day I tested positive for Marijuana and they stopped my pain medication. I graduated to sniffing herion and fetenyal, now my life is at an all time low

My doctor is planning on starting me next week on suboxone for pain management. I used to take it years ago and it worked great. I haven’t taken or any other opioids for 5 years. I was in nearly the exact situation as you. To answer your question: No, you won't get sick and it's good u aren't taking anything because that's how u get sick. U have to not have anything in your system for the medicine to work properly. *I had 3 surgeries and my doctors were going to give me pain medication but I decided I wanted to reach out to a specialist and get signed up for wellness treatment instead of dealing with pain meds. I have been sober for years up until this and honestly I'm really glad I got signed up with Suboxone treatment because it's been really beneficial. It's all around helpful with a lot of things and personally I think it's a great alternative for those people who are either struggling with things or just are dealing with other physical discomfort stuff.

I have to disagree. It's easier to come off buprenorphine. And I only say it because I have came off of buprenorphine after taking it every day for 8 years. You really have to taper down your dose to a quarter of 1 mg. And that's when you stop. But make sure you stay at a quarter of a milligram and let your body get good and stable for at least a month or two and then you can stop and you're going to have minimal issues. Other opiates are practically impossible to taper down because of the shorter half-life, so at the end of the day Suboxone is much easier to come off of as it allows you to taper. I didn't even have any RLS when I stopped taking mine. I was on Suboxone 6 mg for 6 years and then I tapered down over the course of two years. Minimum withdrawal physically. The mental anhedonia sucks but it is what it is

Methadone killed any ambition I had to do anything other than sleep. I went from sleeping 18+ hours a day, unable to hold a job, ended up homeless living in a shed on a friends property. But when I switched to Suboxone, I did an 8 day titration from the methadone to the suboxone and went from a worthless lazy addict to a functioning human being again. I started suboxone 7/8/21 and I now have a FT govt job, my own place, and I just bought a car. I’d probably be dead had I not switched to suboxone. And I have tapered myself down and I haven’t really felt any withdrawal symptoms, if so they were very minimal. I would start to go into withdrawal on methadone after 17 hours after taking my dose so then I would get into my take homes and be fucked by the end of the week. Suboxone saved my life and allowed me to be “normal” again. No cravings, I don’t get high from it like I did the methadone, I don’t nod off like I did with the methadone. So I very much agree with you, suboxone is a much better medication and withdrawals are nothing compared to methadone. Methadone is evil and they need to stop using that for opiate use disorder. It’s trading one addiction for another.

Sublocade is a great way to come off bupe. Once I have a patient on it for about six months they can stop it and because the injection depot dissolves over up to over a year it basically self tapers. I’ve had SEVERAL patients successfully do this minimal to NO withdrawal symptoms and if they do have withdrawal symptoms they can be treated with drugs like clonidine.

I am not sure how this will help me in my condition. 18 lumbar procedures, 4 fusions, 1 done twice, RFA, Spinal cord stimulator that when installed after the 6 day trial went upside down into legs and heels. They waited 5.5 months to reposition electrodes and could get one out. I told them that before #2. It is not close to the spine and one side doesn't work. No one told me except my GP. They got paid and stopped trying electronic programs. Great! 20 hrs of ketamine shows my max tolerance, but no long term effects. Dr's won't replace bone on bone elbow because I am on Medicare. I goes downhill from there. 1986 is turn lumbar muscles into scar tissue. I was told this would happen as I did hard work. Regular steroids epidurals last 1 1/2 days, but give complete relief. Someone messed up! I am approaching 65 and can mow my yard or walk to my mailbox. Sad, but I have little faith from most in the medical profession who have had paper cuts. Do they replace spines yet on 6'3" patients? Or does anyone remove stuck electrodes from the middle or top. I could. The rest are scarrt and have no gumption to figure out new angles. They screwed up and fraid they will make it worse. Like the 2023 video... I will get no response! Just go away and have a happy life! HOW! I "HAD" things I wanted to do, but they (Dr's, Scientists, FDA) are wimps that can't think outside the box! The box has money in it and if a couple of positive results, great) But how / why did they move electrodes from a 60-70% positive trial somewhere else and get it upside down. Crack heads!

I have diabetes neuropathy and have lots of chrinic pain in my lower legs. My A1C is great nowadays and my health is better than ever but the damage is already done from neglecting my diabetes as a rebellious teenager. These meds tho are amazing and the high is pretty awesome and helps you sleep m definitely recommend it before a hot bath or shower after a long day. The only good thing about having chrinic pain is that it gets you some pretty awesome pain killers. Im not advocating for abusing it dont get me wrong. Im just saying when you do take it, it feels awesome.

This is the worse medicine ever!!! The only way when they can be helpful is when someone stoping with heroin!!!! And only for five days, not more!!! I stop them last time in the private clinic, and i make the same mistake again, i started taking very small doses, like 0.4… Then i started to panic that im addicted again, and i continue… Now im taking 20-24mg every day, from few times!!! Thank but it's not for Chronic pain you can use it off Lable like that Painkillers for Chronic Pain are as follows vicodin percocet for The Long acting around the Clock medicine Oxycotin 80mgs 2 x a day or M.S. Contin which is Morphine Sulfate 15mgs etc Fentanyl patches in some cases I been on them they work well The Reason I don't like Suboxone is I'm severely Allergic to it my gums and throat would get swollen then I would get hives of course every body is different but Buprenorphine much like Methadone is to treate Addiction to opiates can suboxone help with chronic pain yes Methadone can also do that
