This is cutting edge stuff. I'm having a harder time getting my thyroid optimized then I did getting my testosterone done. It was hard enough finding a doctor that knew anything about testosterone much less thyroid.

Powerful information here. I am on combo of NDT & T3- feel 10x better. Dave mentioning the fact of the “last push” in workouts & just general movement resonated fully, that’s what happened with me!

@@leslielamont8032 for me, my T3 was still suboptimal on NDT alone. Still fully locking down dose- but at 25mcg is where my body temp is normal & symptoms disappear.

How would one go about getting a NDT? I’m assuming through a physician, but these days most physicians don’t care about optimal levels of anything. If you’re within range then you’re good to go. What about some of these thyroid glandular supplements from grass fed and finished bovine, is this a decent option?

I’m researching this and it seems the natural versions don’t have a specific amount eg 60mg (1 grain) I think they can work but not as precise as the rx formulas

There are optimal amounts that are recommended for you to feel optimal. I don't mean what doctors or labs tell you. T3 needs to be upper range or slightly over. T4 mid range and tsh means nothing really. Armor I think a has all 4 thyroid hormone 1 2 3 and 4. Certain foods will cause hypothyroidism people or hashi people issues. Thyroid issues will drive you Test down. So you may not have a test issue can be Thyroid issue alone. Stop the Thyroid Madness is a good book that gives you optimal amount and other things. So these guys definitely are on the right track with this approach.

I had really high hashimotos tpo antibodies, which I lowered to close to zero with a carnivore diet. Im still on T4 only, ive tried T3 and NDT both made me hyperthyroid. Increasing my T4 dose just slightly will lead not just to higher body temp, it will make me tired all the time, increase my appetite to unbearable levels and give me brain fog and depression. I think T4 only works great for some people that convert it well to T3. Ive noticed, that blood tests, really are useless when it comes to thyroid. I get them done every few months and Im dialed in now symptoms wise, I feel great and my TSH is fluctuating between 0.5-2 and T3 between 3.5-4.5, T4 stays around 18-20 fluctuating way less. I used to adjust dose based on labs, wince my doctor taught me this, which would always make me hyper or hypo. Going by symptoms is the only way to go.

Great information, thanks! I follow you from Belgium and I appreciate the topics you guys are sharing. I have a high tsh, with symptoms like feeling cold, brain fog, high heart rate, fatigue, I feel terrible, my movements and thoughts are slow and on the op of that I also have a testo crash. My doctor (functional medicine) prescribed me testo (sustanon)/deca and thyroid gland extracts (from pig) it's called erfa thyroid, I have been taking 1/2 tablet (15mg) every morning for 3 days but I feel that it gets worse. I'm going to stop it and ask the doctor for advice, we've already tried cynomel but my condition had gotten worse too. What's curious is that I have symptoms of hypothyroidism but for the moment the treatments are not working and have even made my case worse. I have also been taking hydrocortisone for 3 months (20mg in the morning), without it I won't be able to get up to go to work. Has anyone ever experienced the same thing maybe or is it just me?

Im hypothyroid and low T as I've been through Mae menopause and it's brutal having permenant low T. Suicidal depression thoughts, extreme heaviness loss of all strength motivation brain fog and ed loss of libidoits endless the bad symptoms are life threatening, worst illness I've ever had. I'm on levythyroxine but it only works a little bit and I've complained to my doctor and asking for T3 but I have to do bloods first. The NHS UK is not what it used to be as I buy my own test as can't afford to go to clinic. That cortisone med maybe affecting how the NDT works. It also has to be taken at right time of day. I couldn't work though absolutely no chance as cannot even do basic tasks at home let alone work unable to concentrate as so weak. Hope you get sorted.

@@Henry-uo4xd Thanks for what you shared. I stopped hydrocortisone a few days after I started testosterone (my doctor told me to take it in the morning) . 6 weeks later I have better mood and libido. I still have brain fog and tinnitus but in overall I'm getting better. Hope you'll get better too

Hey Dave have you had any experience with improvements in lipids with thyroid supplementation? My TSH was 6.4 and my doctor here in Japan didn't think there was any issue because my lipids were not too far out of "range." So I got some T4 and T3 and started on 50mcg T4 and 25mcg T3 daily, and over a year my LDL went from 166 to 117. TSH went down to 2.4 and a 30% reduction in LDL without any lipid-modifying medications or diet modifications.

Sounds like you need a little more thyroid medication. 2.4 tsh is still a bit high. 1.0 1.5 would be more optimal and you'll probably feel better

I’ve never had a TSH below 6-7. I was on 5-6 grains of armour. Always in a brain fog when taking thyroid meds.

They respond even better. I always say that testosterone to men is what thyroid is the women. Thyroid is vitally important for a woman's optimization

Thought on NDT vs a combination of T4 and T3. The drawback to NDT is you’re married to the the 1:4 ratio while with combination of T3 and T4 you can tailor to your specific needs

@@keithnicholsmd6896In Europe we can buy Metavive, wich is a product almost like NDT. The good about Metavive is that you can choose Thyroid origin from Pigs or Cows. And they have different T4 to T3 ratios. So you can combine the products to fit your needs for T4 and T3.

I was prescribed trt and thyroid at age 59. I put on a lot of muscle but I also had brain fog. I decided to stop the thyroid after about a month and the brain fog went away. I also felt a little off balance when I would run and change direction. When I stop thyroid that went away as well. I’ll be 62 next month and more muscular than I’ve ever been in my life and I feel great. I’d like to optimize my thyroid but why did it make me feel the brain fog and have the balance issues? And I also tried DHEA too and it caused cystic acne on my hips and shoulders. I stopped the DHEA and the acne finally went away completely but the cysts took a while to actually heal. Why did that happen? 🤪

Yes it does, TSH will drop... but not to zero, so there will be some remaining own production.

Nice video. I was listening to another podcast where a doctor said the blood level of Free T3 is different than tissue level of T3 so it isn’t reliable. Any thoughts on that?

All studies done with thyroid and raising free T3 levels correlates with symptomatic improvement better than TSH or T4. So there is a correlation with serum levels and clinical improvement.

@@keithnicholsmd6896 perhaps that’s why FT3 tends to decrease on T4 monotherapy? I also notice FT3 decreases on T3 monotherapy unless labs are taken right after swallowing the pill. That lends credibility to the notion that combination therapy is better since FT3 does seem to go up with the combo.

@@lv1985aa we explained your situation in the video, and what happens when you take only T4 is that a decreases your thyroid production of T3 and T4 and when you take only T3, it also decreases your thyroid production of T3 and T4. If you just take T3, which has a very short half-life then when that runs out, you don’t have any T4 to convert to T3, so you’re right it will go down. When you take just T4, and if you don’t convert very well to T3, then you’re not going to raise T3 very much by just taking T4. It’s best to take both T3 and T4 together.

Because when you take Cytomel, which is T3, only you're going to shut down your thyroid's production of T4 and T3 and when you do that you're not going to have any T4 to convert into T3 once the cytomel is metabolized. It also has a very short half-life so once the cytomel runs out you don't have any T4 that you produced on your own to be converted into T3.

Please refer to the TRT versus HRT video but nonetheless since you're not symptomatic you don't have to do it. See there's two approaches in one approach you treat symptoms related to a deficiency and the other approach you optimize levels so that you never have symptoms of deficiency and you maintain maximum benefits of the hormone as you age. So there are really two ways to approach it. Wait till you have symptoms treated or prevent ever having symptoms of an efficiency and maximize levels to maximize benefits.

@@keithnicholsmd6896 thanks doc! I had one big symptom which was an emergency room visit with afib in December: they took labs and my TSH had shot up to 6 randomly. Only ever tested it between 2.7 and 3.8 on my own lab work. Figured I would have to get this under control.

@@keithnicholsmd6896 Free T4 - 10.4pmol/L, TSH 1.6mIU/L and Free T3 - 2.5 pmol/L. Symptoms include cold hands feet and overall, don't sweat with any exercise, dry skin on my hands, sleeping issues, feeling overall low and lethargic. Have been on TRT for 8 months thinking that would be the cause but sadly the mental and physical issues are still there. Thoughts?

When you add exogenous thyroid, does that cause your thyroid gland to reduce or shut down your natural production even more? Thanks.

It will decrease your thyroid's production, but it won't shut it down unless you take suppressive doses. But the reason you would take it in the first place is because you're already not making enough. Just like with testosterone.

Wont taking even small amounts of NDT just lower your own production over time, so you end up the same place as before starting supplementing, or even lower ? When taking Progesterone, it works like an add on om top of your own production. Do you think it work in the same way with NDT ?

No, when you stop thyroid, you go back to producing what you normally would've been producing had you not taken thyroid. Just like with testosterone
