That's a first for me. Never witnessed an O2 sensor blowout. Also, this video is a a good lesson on how screwed we are with all these in-tank fuel pump/filter/sender assemblies.

Not too often we see a toyota on the channel ! Over 400,000 kms+ a simple inexpensive repair I have a 2015 sierra (70,000 kms) that has required a bunch of expensive repairs ( oil pump,high pressure fuel pump) wish I had bought a toyota

Not unless the fuel filter breaks apart internally and allows the dirt through. There is no bypass in the filter and it simply restricts to the point of fuel starvation.

Was your diagnosis based on the look of the fuel filter?

Combination of both banks running lean and the fact the fuel filter looked like it was never replaced.

Which engine and what have you done so far? Is the misfire on a specific cylinder or one bank?

Unfortunately, replacing the fuel filter did not help with the leaks, but thanks to this the car drove much better))) mileage 328,000km. Today the scanner showed a misfire on the first cylinder and a lean mixture. p0171, p0301. Looks like I'm at the beginning of a very interesting journey🕵️😬😱😭

didnt realize you can use compression fittings on metal fuel lines i know brake lines you cant

Definitely not on a brake line but I've used them on fuel lines and transmission cooler lines.
