IMPORTANT 2023 UPDATE re: IUD DURATION OF USE- US FDA now approves 52mg LNG IUS (Mirena, Liletta) for 8 YEARS use for contraception. When treatment of heavy menstrual bleeding is the primary indication, best results are usually seen in the first 5 years. Kyleena (19.5mg) is still 5 years. See video #35 for info on copper IUD extended use.

hey, i’m an american girl too. almost 16, from oklahoma, _was_ thinking about having sex but now with the total abortion ban here and the supreme court about to overturn roe, that’s gone out the window. i’m beyond devastated, i’m terrified.

@@weldchickn2270 im in the same boat. Im 19 and am getting an iud because of the recent abortion laws. Since theres a chance of still gettting pregnant with an IUD, I was going to track my ovulation cycle to see when Im the least fertile, and have "intercourse" (that term is weird) at that time for extra protection. You dont have to do that with an iud, but it will make me feel a bit better. If you do decide to do it, please use condoms, and maybe talk to your family about getting birth control. If they wont let you, say it is to help lessen cramping and period flow or acne (if you have any of those)

Thank you so much for taking the time to record and post those videos. I'm about to have an IUD inserted, so I started searching and read some pretty scaring reviews of some people who had terrible side effects and I was so worried the same would happen to me. I have never been on birth control before and my OB/GYN prescribed Kyleena for me. Thanks for explaining the process in detail. Hope you continue making these videos, they are truly helpful.

In scientific studies looking at use of anaesthetic gels, sprays or injections, they unfortunately have NOT been shown to significantly decrease the degree of discomfort at insertion. -Dr. Dervaitis

I'm getting mine in tomorrow, I'm also from Ontario! My period was supposed to start yesterday but it still hasn't, but I'm taking those pills. I hope this won't turn my doctor away from doing it, getting an appointment is COVID is awful

Got my iud today and it was not nearly as bad as I thought. Just two deep cramps. So far today though the constant cramping has been the most annoying thing. Hopeful that it goes away after a few days.

There are not any standard of care guidelines on using “relaxants”, but there are some promising new options for pain relief in the form of inhalational anaesthetics (these are somewhat new to Canada, and were not available at the time I recorded this video). I plan to do a future video on this topic so stay tuned!

I am getting mine tomorrow and I’m very nervous but I’m hoping this will be a big change for my life. May 2019 I got the nexplannon bar in my arm and since then I’ve had a terrible increase in my anxiety and I’ve developed awful stomach problems. Mostly nausea and indigestion. Most doctors I’ve seen say they are related to the nexplannon but both started the day after I had it put in. So I finally made the appointment to get it removed and try the IUD. I’m hoping this helps me get back to normal life!

Thank you for the in depth info. I recently had mine inserted with my gynecologist and when I asked about a follow-up, they said they weren't doing them because of COVID. I am the type of person to get anxious if I have something new like this, so would you recommend scheduling a checkup with my primary provider and do you know if they do ultrasounds?

Hey Dr.D am really interested in an IUD, after watching your previous videos im leaning towards a hormonal iud, however what do you recommend the mylena vs the kyrena? And which would be better for someone who have had very negative side effects from the combination type pills of birth control.

Hello- I mostly recommend Kyleena for patients who: -have not had children (smaller insertion device and easier to insert in these patients) OR -are looking for a lower dose hormonal option. NOTE: the hormone exposure from an IUD is VERY different compared to the oral birth control pill. Please also see videos: 10, 11, & 14 for more information on this particular topic. -Dr. Dervaitis

Doctor, I had Mirena insertion one month back due to endometrial Hyperplasia, but even after the insertion, I am bleeding heavily and continuously (almost 1 month now, the flow has come down with medication only). I just want to know if this is also a normal condition and should I be more patient for a couple of months or if you suspect it is not working for my body? Would really appreciate it if you could please answer this.

Getting my IUD this week, my doctor recommended no sexual intercourse for 2 weeks prior and to be on my period. I followed both recommendations so I’m hoping for a smooth insertion😊 will update y’all soon👍

@@Cece_1444 so it hurt really bad for a little bit after insertion. And then during my first period after insertion my uterus was having severe cramps (like stabbing pain) and it turns out my IUD was being pushed out by my body, so I ended up ripping it out myself.

I'm on my second IUD and I do love it! The initial insertion was a bit painful because of the cramping. I was just wondering if it's normal to have bleeding immediately after sex? I've noticed that it does seem to happen relatively consistently.

Hi Love your videos so simple and straight forward. I had an IUD inserted on Thursday. I have this feeling afterwards of pain down there. It only hurts when l walk. I have slight spotting. I don’t have cramping in my abdomen area. Is this pain normal? If so, how long should this pain last? I don’t want to take a lot of pain killers, it’s not good for the liver. Thanks.

I’m educating myself again because the first time I got an iud I got Skyla and it was painful but it was a good thing for me :) I was just scared of the IUD traveling like all the people on reddit say will happen that’s why I got it removed

Thank you so much for saying it's OK to remain on the pill until the follow up visit. I was going to message my doctor about that because she didn't tell me if it was OK to do that. She just said to use backup contraception for at least one week after insertion, but I wasn't sure if it was OK to continue on the pill or if she just meant condoms. I wasn't sure if it was OK to be on two hormonal birth control methods at the same time. Thank you!

Hi Doc. Got a copper IUD inserted almost 2.5 weeks ago. Recently i have been experiencing some really bad cramps however my menstruation isnt due for about 1.5 weeks. Is this normal? It also feels a little bit dry & irritated ‘down there’. Should i contact my gyn or is this just normal?

I currently have Nexplanon and I’m considering getting a hormonal IUD. Would the two cancel each other out or would it be better to have both? I’m in a new relationship too.

It would not be recommended to use both (usually it’s one or the other). Remember that neither the Nexplanon nor the IUD protect against STI’s, so always also use condoms for STI prevention with a new partner! -Dr. Dervaitis

I’m 52 and am scheduled for a Mirena in 3 weeks due to hormonal perimenopause. That is, heavy bleeding accompanied by painful cramping. All tests have been done to rule out any issues. I’ve never had children and my gyno has advised me that she will freeze my cervix prior to insertion. Is this common practice? Thanks so much for your informative videos!!

Some practitioners choose to freeze the cervix prior to insertion. This is not my practice personally (as there is not sufficient evidence in the literature to show that this actually improves pain significantly, and there can be done discomfort from the injection of the freezing). Some have tried freezing sprays and gels to the cervix, but there is also little evidence to show that this improves the discomfort associated with insertion. -Dr. Dervaitis

I had that question about mentrual cup use in the first 20 days. It seems logical to me to avoid putting something up there during the "window for infection," but my doctor was unconcerned so long as it isn't in the 48 hours post placement. She was very good at explaining everything you are too, before and then after so it wasn't her just brushing me off or anything. It's up for me to decide, I guess. I don't know if I can manage my period without my cup (or disk rather) anymore, to be completely honest, I'm looking forward to it hopefully being completely gone.

Yes, it is an individual choice. Admittedly, there is not any study evidence to prove harm from tampon or menstrual cup use after IUD insert (there is not actually even evidence to prove harm within the first 48 hours) My recommendation about avoiding tampons/ cups for 3 weeks is based simply on what we do know about the window for risk of infection (for which there is evidence). -Dr. Dervaitis

It might depend on the kind of IUD, and where you are in your cycle.... but most gynecologists would say “no”. However, because there is a risk of expulsion or undetected perforation (in either of these situations the IUD would not actually be within the uterus doing it’s job...) I advise all of my patients to use back up contraception until they see me for their one month check up (and we are reassured that the IUD has remained in proper position). -Dr. Dervaitis

Hello- Glad you are finding the videos helpful. It’s not absolutely necessary to be on your period for an IUD insertion. Some clinics schedule it this way because: 1) it’s a way to be certain that the patient is not pregnant at the time of insertion, (although there are other ways of determining this, with by history or lab test) 2) the insertion may be somewhat easier during menses because of changes to the cervix and the cervical canal is a bit more open due to the menstrual blood). Check with your doctor/clinic to find out their policy on this issue. -Dr. Dervaitis

No consistent evidence to support a decrease in libido with IUD use (very low levels of hormone in the blood stream, and the majority of women still ovulate with the hormonal IUD and so natural hormone cycling is not altered). -Dr. Dervaitis

Thanks for the info! I just got my copper iud placed on June 2nd, my experience after placement was horrible I cramped really bad to the point I was crying. I’ve had heavy discharge mostly clear with a little bit of brown spotting but I get random sharp cramps still, will this continue? I’ve also been spotting a lot and had a little bit of dark blood, is this something I need to worry about? Sorry for the tmi, my gyno didn’t really explain this to me.

@@ashy2709 hi Ashy, I actually removed mine 3 months after because the pain for me was unbearable. Turns out they ordered me an iud that was too big for my uterus, according to my doctor she said I had a small uterus and the bigger ones are usually placed in females that had babies already and I haven’t so maybe that’s why it hurt so bad. Anyways I’m off birth control completely now I did go back on the pill for a little bit but decided to get off it. I would make sure to ask your Gyno if it’s the spotting continues, what I read is that it normal to have in between spotting until your body adjusts to the IUD but I’m no expert. Best of luck! (:

@@missjosy34 Hi! I wanted to say that maybe the Kyleena IUD is something for you. It’s hormonal, but the smaller sister of the Mirena. I went and got it this week, didn’t have a bad experience with the insertion. Plus I don’t experience any cramping. My period even went away after a few days already. I know it’s really personal. But def with your situation, it can be a better option to get the Kyleena.

Hi Sarah- There would not be any different effect on the IUD (or you) based on frequency of intercourse. Obviously, more frequent intercourse increases the chance of pregnancy, so it’s good that you are using such an effective contraceptive method! (A reminder that if you are ever with a new partner in the future that condoms are necessary for STI prevention.) -Dr. Dervaitis

So it's not necessary to refrain from checking for the strings too soon after insertion? I was concerned whether I should wait for like a week before I put my fingers or anything near my cervix, for fear of causing an infection!

Hi! I’m 18 and have experienced increased anxiety and depression from being on the birth control pill. I would like something long term as I want to avoid getting pregnant at this age. Would you suggest getting another form of birth control before the IUD? I read that If you’re under the age of 20 it may not stay in place. Any suggestions?

Hi Makayla- There is a small chance that an IUD might fall out (average around 5%), but it is not necessarily more likely if you are under 20. Anyone in need of very effective contraception (regardless of their age) is a candidate for an IUD (if they don’t have contraindications- there are very few, see video 8). If you are considering an IUD, I’d recommend that you watch videos 10, 11, and 14 (which talk about the issue of hormones, the big difference between hormone exposure in the pill versus the IUD, and also compares the copper and the hormonal IUD). -Dr. Dervaitis

There have been studies to look at this issue. They found that taking pain relievers before the procedure did NOT work any better than taking them afterwards (as needed). -Dr. Dervaitis

Some clinics recommend insertion while the patient is actually menstruating- insertion might be slightly easier (because the cervical canal is a bit more open due to the menstrual blood) and the inserter can be certain that the patient isn’t pregnant. However, an IUD can be inserted at any time, as long as there is some sort of assurance that the patient is not pregnant (either based on clinical history or a negative pregnancy test). -Dr. Dervaitis

I just got my Hormonal IUD inserted a week ago for very heavy periods. It's been a week and 3 days ago I started bleeding not extremely heavy, but is not light or spotting either. I bleed all day and night -_- and it scares me to bleed so much. I've already been having abnormal bleeding for over 4 months where I've had only 2 weeks break. Is it normal to bleed a lil more than just light?

Is it normal to not be getting a period after four months of the iud being in place? No chance of pregnancy but I always had light periods and prior to the iud insertion I was on LoLo birth control which stopped my period

20-30% of hormonal (not copper) IUD users may stop having periods. The chance of pregnancy with an IUD is 2-8/1000- if you suddenly stop having periods and think you might be pregnant, do a pregnancy test &/or see your doctor. Patients who previously had very light cycles may be more likely to not have a cycle with the hormonal IUD. -Dr. Dervaitis

Hello- Cramping and pain in the “adjustment phase” after insertion can vary from patient to patient (much like menstrual cramping can be different from one person to the next...) The discomfort is usually similar to menstrual cramping. Some women do experience lower back discomfort with menses. This sort of pain is also a possibility after IUD insertion (but less common). If your pain is severe, or not improving, I would recommend that you see your doctor for assessment. -Dr. Dervaitis

hey! I am getting my Kyleena IUD inserted today. I have a fear of throwing up and people say they have thrown up because of the pain after. My doctor prescribed me with 800mg of ibuprofen, I took a pill the night before to soften it, and I am on the heaviest day of my period. I am still dreading it because I have heard people have thrown up after, and I am scared that I am going to.

It is rare to vomit after an IUD insertion. It is not usually because of pain, but because of what is called a “vasovagal reaction”- stretching of receptors in the cervix trigger it, the heart rate slows and blood pressure drops (briefly) and that’s what causes the nausea. Hello! I know this was a year ago but you’re comment is EXACTLY what I am looking for. I’m getting mine (Kyleena) inserted in 3 days. I’ve done so much research and watched so many videos I may be about certified to insert one myself 🤣 anyways, also major fear of throwing up. More so than the pain. How was your experience? I also heard 800 mg of ibuprofen helps but I am afraid to take the cervix softener (misprostol) because I’ve heard it can actually cause nausea/abdominal distress and I’m too afraid to risk that. I need to try to get off these damn pills though. I’m so tired of them after years and years . Thank you!!!!

Hi Dr Dervaitis! Is there a lot of bleeding going on during the actual insertion? For the 2-3 weeks following insertion there is a lot of cramping and spotting, but would there be an estimated time of when a "period" comes after the IUD is inserted?

There is some bleeding associated with insertion but not usually heavy bleeding. A period may or may not come on schedule after insertion, it can sometimes be difficult to distinguish period bleeding from “adjustment phase” bleeding in the few weeks following insertion. -Dr. Dervaitis

If it’s a hormonal IUD, some women (up to 30%) stop having a regular menstrual cycle (it can take several months for this to happen). However, because of the known 2-8/1000 chance of pregnancy with an IUD, if it’s the first time you’ve missed a cycle, the safest thing would be to confirm that you are not pregnant (either with an at home test or at the lab, via your doctor). -Dr. Dervaitis

I just got my IUD today and I felt so comfortable and knowledgeable because I had been watching all of your videos prior as they really shaped my decision to switch from the pill to the hormonal IUD so I really can’t thank you enough! At my appointment, my doctor told me to abstain from sex for at least two weeks or to use condoms, I’m in a long term monogamous relationship and neither of us like using condoms, other than the increased failure rate when comparing condom use to the withdrawal method, is there any other potential risks (possible infection) to using the withdrawal method for these two weeks?

The hormonal IUD is not effective until 7 days after insertion (unless inserted during day 1-7 of the menstrual cycle) and so patients are advised to use backup contraception. Condoms have an 18% chance of pregnancy, withdrawal has a 26% chance of pregnancy. The copper IUD is effective immediately. Both IUD’s have a small chance of falling out after insertion (for thus reason, I advise my own patients to use back up contraception until they see me for their follow up visit when I check that the IUD is still in proper position). I’ve not seen this side effect and there is not any evidence to suggest that there would be isolated inflammatory vaginal changes after insertion. However, infection can occur in 0.5% of IUD users, and is common in the first 3 weeks after insertion. If you are concerned about infection (other symptoms include abdominal pain, fever, foul discharge) see your doctor! -Dr. Dervaitis

Had my paragard inserted about an hour ago. There was one big severe cramp when it was inserted but that was it. Doctor said that it is effective immediately, but I am concerned for infection. How long should me and my partner wait before having sex. Also by doctor did not schedule a 1 month follow up, should I be worried or push for a follow up?

The paragard is effective immediately. Because of the small chance of expulsion, I advise my own patients to use back up contraception until I see them for their one month check. Not all doctors schedule a check. If you’re able to feel the strings (see video #27) then you can be fairly confident that it is where it should be and protecting you. You can be sexually actively immediately. The infection risk is less than 1%. If you’re with a new partner, it’s still important to use condoms for prevention of sexually transmitted infections. -Dr. Dervaitis

I’ve had the kyleena in for just over a week now and so far I hate it and it’s still super painful to the point it stops me in my tracks. Ten x beyond anything I’ve experienced as far as regular period cramps. I’ve felt bloated as a tick since as well a headache nearly every day and uncontrollable crying teii ok nights in a row 3 days after insertion. The most concerning however bc it was a brand new issue to me that happen the day after was a swelling in my left foot/ankle causing the foot to feel thick and numb. It went away after about a day or two and it’s back again today and idk why.

So sorry to hear your experience has been bad with Kyleena. I had mine placed on tuesday. Didn’t feel pain during insertion and had mild cramps. But nothing severe. It had been 5 days now and my period stopped, plus I got no cramps. I think you should make an appointment to check if everything is normal. It sounds like something isn’t right. Maybe your body doesn’t like the new hormone (levonorgestrel).

Hi Dr. I have had Merina inserted on 16th Jan 2023 . I was diagnosed with adenomyosis 4 months ago and was advised to have Merina . I had spotting the day before my surgery ( Hysteroscopy, curettage and Merina) . Had light bleeding for few days but today have dark red almost black bleeding . Good question Kay! Answer= NO. An IUD can be inserted into the uterus regardless of whether it is tilted back (retroverted), forward (anteverted) or in the middle (midverted). The doctor examines the patient prior to insertion to determine which direction to insert. -Dr. Dervaitis

If inserted within days 1-7 of the menstrual cycle, Mirena is effective immediately. If inserted outside of this window, it takes up to one week to work to prevent pregnancy (and it has advised to use back up contraception in the first week after insertion). -Dr. Dervaitis

Ok here, my experience wasn’t that bad, it didn’t hurt but when it was about to get into the cervix it was really uncomfortable, and then when it’s done you’re going to feel like cramps but there’s no pain, at least in my experience. I really recommend you that before you go, like one hour before eat something, and bring a pad cause it’s going to be a lot of spotting and of course don’t go alone because if you start feeling bad you can have at least someone that can help you. And that’s all, it’s not going to be painful so there’s no need to be scared just relax and think about something else while getting it.

hi, so I just got my IUD yesterday, and after the insertion (or before I don’t remember) they put some orange-yellow cream stuff on my cervix which I’m not sure what that is for. Today, I was seeing if I could feel the string and I noticed the inside is very dry and almost clumpy feeling which I guess is from whatever they put in there. Is this normal?

Sometimes we use what’s called Monsel’s solution (ferrous subsulphate), it helps to coagulate any bleeding related to the insertion procedure (it’s an orange/mustard colour). When it mixes with blood it clumps and can look black (and sometimes like a coffee ground consistency). If you are concerned, talk to your doctor. -Dr. Dervaitis

The discharge should resolve in a few days. Vaginal dryness is not typically a post insertion complaint. -Dr. Dervaitis

Does a Mirena cause or increase risk of breast cancer or any gyn cancer?

No consistent evidence to link the IUD to breast cancer. Note, there IS however evidence that the hormonal IUD decreases risk of uterine cancer, and also new evidence to suggest decreased risk of ovarian cancer. -Dr. Dervaitis

My mother is 52 and she is a CVST patient and her periods are not normal sometimes she have clots and have really bad cramp..so my gynecologist has suggested marina IUD for her... So what should we do about it? Please can you respond as soon as you can...

I hope this isn’t crude but every thing I’ve googled or searched for has been very vague and...demure almost I would say, without a very direct/clear answer. Can my partner regularly finish/ejaculate inside me once I’ve had my IUD for a month ? We have both been tested and are free of stds and are monogamous. Thank you for making these videos, my gynocologist is very very nice and makes me comfortable but sometimes I forget details of what she said during our appointments and this helps reassure me.

Hi- Nothing crude at all about your question, it is frequently asked. The answer is yes- if there’s not a concern for STI’s then it is not necessary to use condoms concurrently and ejaculation is fine. The chance of pregnancy in this scenario is 2-8/1000 (depending on the IUD). For couples that aren’t comfortable with that very small risk of pregnancy, they may choose to use additional contraception (eg, condoms) or to practice withdrawl. Speaking generally, the “adjustment phase” bleeding sometimes can last longer than 4-6 weeks (sometimes it lasts even a few months). I’ve been getting super duper nauseous during sex, not after, during. Like 10 seconds in. The only position that I know of that doesn’t make me nauseous is when I’m on top. none of my Doctors really even know why, i think it’s my iud. I got it about a year ago and it started happening in February of this year. Do u have any ideas or any information I can take with me to the doctors to give them some sort of idea of what’s wrong with me?

There is not any evidence to link this sort of a reaction/ side effect to IUD use, nor do I know of any other obvious explanation to explain this (…although some patients can be sensitive to pressure applied to the cervix, sometimes causing a vasovagal response during IUD insertion. Vasovagal episodes can be associated with nausea. I am speculating, but possibly with deep penetration the pressure on the cervix might also trigger such a response?) -Dr. Dervaitis

It varies from patient to patient. It could be a few minutes, a few hours, a few days… rarely would it last weeks. Most cramps are treated adequately with an over the counter anti-inflammatory like Ibuprofen. -Dr. Dervaitis
