IMPORTANT 2023 UPDATE re: IUD DURATION OF USE- US FDA now approves 52mg levonorgestrel IUD (Mirena, Liletta) for 8 YEARS use for contraception. When treatment of heavy menstrual bleeding is the primary indication, best results are usually seen in the first 5 years. Kyleena (19.5mg) is still 5 years. See video #35 for info on copper IUD extended use.

I want to thank you for making these videos, Doctor. I got my kyleena yesterday and I watched a ton of your videos beforehand and it made me feel a lot better. I remembered your insertion video and how it took you about 15 seconds, luckily my gyno was just as quick! It hurt like hell but it’s over now. Thank you for what you do!

I had my first gynecologist appointment today and was suggested and iud without even having spoken for 2 minutes(another dr i feel didnt care). I dont know what came over me but i said yes and had it done today. I was explained pretty much nothing before nor after. I am so grateful I found this channel.

I am so glad I found this video. I had my copper iud put in on Monday and I am a bit worried that I did not get much information so far the cramping comes and goes and the bleeding has stopped for now I was just scared from the reviews and videos I’ve seen online

Its use ful.inserted mine in nov but since feb am in period did scan but all things turned to be normal, was given injection, monthly capsule, and other tablets but no change

Hi Mgallowa- Based on what you’ve told me, I suspect that you’re one of the 30% who will not have a period (or maybe only occasional/ sporadic light bleeding) with a levonorgestrel IUD. That said, I often will tell patients that the first time they miss their period, it doesn’t hurt to do an at home pregnancy test for further reassurance (and especially if there were any concurrent signs of pregnancy like morning sickness/ nausea or breast tenderness...) But thereafter, it’s not necessary to do this sort of test every month (if you continue to NOT have a period). The chance of pregnancy with an IUD is very rare (2/1000 with a LNG IUD). Definitely you should attend your scheduled follow up appointment with your doctor. -Dr. Dervaitis

I've had a Mirena for a year now. At first when i got it, i bled for almost 4 weeks straight, at my check-up my dr didn't express much concern over the bleeding or cramping that I was having. Then for the next 9 months i struggled with bloody discharge for 3 weeks and then one week normal discharge. I was then prescribed Mireille and a clot forming medication to clear that up. I was prescribed the Mireille for 3 cycles and the bloody discharge was only for about a week and a half. But when i went off of the Mireille, within the same month i got a heavy period with heavy cramping. It almost feels worse having the Mirena.

Hi Dr. D, Love your video! Thank you so much for spreading awareness and knowledge on this topic! ❤️ I just have a question: i have a copper IUD insertion in April 2017. Everything is good except for the fact that I have constant spotting (bleeding in between periods) ALL THE TIME. It’s been more than 2 years but it doesn’t seem to stop. I’m really bothered by this, and I don’t know if this is the message that my body’s trying to tell me that it doesn’t like the IUD and want it to be removed. I bleed so often, like 20 days out of 30 days in a month. Is there something wrong? Should I be worried? Should I remove my IUD? I’m 20 and I’ve never been pregnant. I think I have the Mona Lisa mini that’s supposed to last for 5 years. I also locate in Ontario. Please help me with this! Thank you so so much! I really appreciate it! 🙏🏻❤️ Clara.

Hi Clara- Sorry to hear about this spotting, you must be very frustrated! While it’s less common for spotting like this to persist past the first few months, it does sometimes happen. Based on the timeframe you describe, I don’t think that there’s anything to be worried about. However, since the spotting is bothering you, I might suggest that you try switching to a Mirena (or even Kyleena) IUD. Although spotting like this is still possible with the hormonal IUD, the overall bleeding tends to be less (due to the levonorgestrel thinning out the lining of the uterus) and you might have better luck. This spotting is NOT necessarily an indication that you can’t continue to use an IUD. Hope this helps! -Dr. Dervaitis

@@lemnatins1434 No it never really ended. In fact it seems that at times it’s really heavy. I’m getting it removed next week because to me it’s too much. I can’t hang with loosing all this blood.

This is my Day 7 with my Mirena on, day 2-4 were juz fine no cramps and no much spotting, On day 5-7, I having my heavy bleeding, I just called the dr , he said iud takes time to adjust approximately mths to adjust :(

This video was amazing. You were so clear and informative. Also you have a very nice soothing voice! Thank you very much, Dr! I've had an IUD (Kyleena) for a little over 6 months now and I’ve experienced bleeding and spotting nonstop, but it’s finally starting to go away, other than that I’m feeling fine (just very ocasional cramping). Bleeding is super annoying, but patience is the key! I also feel super safe (I don’t worry about getting pregnant anymore) and happy that I no longer have to remember to take the pill. PD: The first few weeks I did feel really nauseous.

Hi Kaylyn- When you have an “IUD change/ replacement” (immediate new IUD insertion after old one is removed) you are immediately protected from pregnancy as long as the old IUD is not overdue for removal (not expired). However, because of the small risk of expulsion after a new IUD insert (ie, risk of the IUD falling out), to be cautious, I ask my patients to use back up contraception until they see me for their one month follow up visit when I check that the IUD has remained in good position. Hi Anna- Measurement of cortisol levels is NOT standard of care practice after IUD insertion. Cushing’s disease is NOT a contraindication to hormonal or copper IUD use. -Dr. Dervaitis

Very informative. I am nervous because I am a week late on my period and I have no clue is it because of pregnancy or this new IUD. My old IUD was 1 month expired before I've gotten an appointment to change it. So I am very confused. I've gotten one positive pregnancy test on a blue dye test, which we all know gives out false positives. So VERY confusing

Hi Sonya- Sometimes bleeding can return after having been without menses (“amenorrhea”) on a levonorgestrel IUD. It’s difficult to predict what the pattern of bleeding would be, moving forward. Any time there’s a significant change in bleeding, I recommend that the patient have a follow up appointment with their doctor, in order to check that the IUD is still in proper position, and to rule out any other causes for the bleeding. Hi Raven- Unfortunately I can’t really answer your question given the information provided. Most IUD’s are to be used for 5 years and not 7 (but I’m not sure exactly which IUD you have, and in some situations the duration of use depends on patient age at time of insertion...) i had paraguard inserted nearly two weeks ago and havent had a day without at least light bleeding (it’s extremely light but nothing too bad) but sometimes it goes medium light for like an hour and that’s it

Glad you found the video helpful! Bleeding during the”adjustment phase” (first 4-6 weeks after insertion) is not generally reflective of future bleeding patterns (or whether there will be need to discontinue the IUD). -Dr. Dervaitis

I just had my iud (Mirena) placed in two weeks ago. I’m still lightly bleeding. Thank you for letting me know this is normal. I forgot to ask my doc about it. I had the copper one years ago. I have PCOS. My husband had a vasectomy done so I don’t need it for bc just to he,p with the PCOS.

Hello- It is definitely possible (and safe) to use a menstrual cup with an IUD. (If you search this topic on Instagram there are a lot of helpful posts with illustrations and tips. The patient just needs to be cautious when manipulating the cup so as not to inadvertently pull on the IUD strings (usually if the cup is placed low enough in the vagina it’s not a problem). Thanks I wanted to see someone ask this since I love cups but am getting my first Mirena inserted next week and my doctor said ‘we don’t have enough evidence to prove if the cup with IUD is a good or bad idea so be careful with removal’

I got my copper IUD inserted on May of last year, so a year and 3 months ago. But the bleeding between periods hasn’t stopped. I’ve never had that problem before the IUD. And I’ve been hesitant to go to the obgyn because of COVID. But I’m just so tired of this, I’ll be making an appointment on Monday for sure! Thank you for the informative video👍🏼

Hi Abrilia- Keep in mind that not every patient has heavier or more painful cycles with a copper IUD, so I would just see how things go. If you’re not happy with how your periods are with the copper IUD, you could later switch to a lower dose levonorgestrel IUD (you might not find as much acne as you had with the transdermal implant). Regardless, you will be protecting yourself from unplanned pregnancy using one of the most effective options available! Hope this helps! -Dr. Dervaitis

Yes it does help! I'll for sure search for that option in my country in case this IUD cause any trouble. I am still bleeding, today got heavier but taking Ibuprofen calms down the cramps. Thank you again for your thoughts!

Some women have irregular or skipped cycles for various other reasons (other conditions or situations that interfere with ovulation). I’d recommend that you see your doctor for further evaluation if you are concerned. If there is any chance you might be pregnant (if you’ve been sexually active since your last normal period), do a pregnancy test. -Dr. Dervaitis

I got mirena, and I had one day of pretty heavy bleeding at first, then it lightened. Now it's very light, and easily ignored. I have light cramps, but nothing as bad as it used to be before the iud. I'm quite happy with mine. I had a period a month straight before getting it.

I have had my second IUD for 2 years and I recently started bleeding consistently throughout the last 2 months. Some days it will stop and then come back. Should I be worried? I currently have an Implanon rod in my arm as contraception, but I feel like it's affecting my mood and anxiety negatively... anyone else experience this? I've used the ParaGard before too but it was expelled from my body twice because the nurse didn't tell me that as a heavy bleeder, using a copper IUD isn't a good idea. I'm thinking of trying the Kyleena due to its lower hormone release. Anyone with anxiety disorder using the Kyleena??

Hello negin Joseph- The amount of progesterone in your bloodstream with depo is far greater than what enters the bloodstream with a levonorgestrel (hormonal) IUD. If you were experiencing any progesterone related side effects on depo, they may decrease or no longer be present. If your cycles were very light (or if you had no cycle) with depo, there is a chance that you may have the same menstrual side effects with the hormonal IUD (although some women with no cycle on depo might go back to having a light cycle with Mirena or Kyleena). -Dr. Dervaitis

I'm on my first month with my Kyllena, I didn't experience any spotting and had my period when it was due, although i noticed some difference. Is it normal t have perfectly normal period? As it was before the insertion?

Hi Camille- Although there is often an adjustment phase of unpredictable bleeding, it doesn’t necessarily happen to everyone . Also, it can take several months before a patient may experience less menstrual bleeding (and a minority of patients may never see a change in their bleeding). So having no bleeding and then a normal period isn’t necessarily cause for concern- but as always, I recommend that you attend your routine post-insertion follow up appointment with your doctor (always a good idea to have at least one check afterwards to ensure the IUD is in proper position). -Dr. Dervaitis

Hi Pallavi- Some unpredictable or “breakthrough” bleeding can happen with an IUD (especially in the first few months) but usually it isn’t heavy. If you continue to have heavy bleeding in between your periods, I’d recommend that you go and see your doctor. -Dr. Dervaitis

Hi kat x- In the first 4-6 weeks, it can sometimes be difficult to distinguish period bleeding from “adjustment phase” bleeding. hello dr had the 5 year copper Iud the other day. was told to keep going with my progesterone only pill for awhile which I have never had a period on. about 3 days after insertion which was very painful I had heavy period like bleeding but I have not had a proper period for about 3 years... is this normal? given I am still on the pill?

Hi Flo- This is a unique situation, so it’s difficult to say what’s “normal”. The heavier period like bleeding may be due to the “adjustment phase” after the IUD insertion, and so you may still have some bleeding (despite continuing on the progesterone only pill). However, if the heavy bleeding persists, I would recommend that you see your doctor. -Dr. Dervaitis

Glad I found your channel and hopefully you can give me some info in regards to my situation. I had the Paraguard inserted in 2008 and went to have it removed in 2014. The midwife that did the removal failed to inform that it broke. My life has been changed physically by this. The broken piece has been embedded in my uterus for 4 years making it a total of 14 years. I’m currently working on trying to have it removed, but I keep getting passed around to different gynecologist. Can you shed some light on any possible long term effects this may cause me in the future? Thanks in advance

A small portion of the copper IUD is not likely to cause any health problems (although could interfere with conceiving if one is trying to get pregnant…) There have been many cases of patients forgetting about their IUD and the IUD staying in place for years past their suggested duration (sometimes decades) without major consequence. Generally it is still recommended to try to remove the IUD (or portion of IUD). -Dr. Dervaitis

I’ve had a Mirena for 3 years and it did help to stop the heaviness/pain of my periods. However, this year I’ve started to have more regular and heavier periods again. Is it possible the hormone that lessen/stops them, stops working so well after a few years?

Hello- Hormonal IUD’s release hormones throughout their lifespan (in the case of Mirena 5 years). It is a consistent daily dose that does not diminish during the recommended duration of use. Sometimes periods can change over time. But for any of my patients using the hormonal IUD, I advise that if they suddenly go from much lighter to heavier periods, they should come in to have the IUD checked (to make sure it’s in proper position). (Bottom line, I can’t provide personalized medical advice over the internet, so I think it would be best for you to see your doctor for a check up.) -Dr. Dervaitis

First Mirena period for me last week added more days and heavy blood loss. My RBC is low beginning to think my dr is crazy. Wish we had dr like you in Northern ON. Thanks for all your videos. Because of your videos June 2 I had my iud placed the next day. Insertion was exactly as you said. Thank you doctor. I sure hope things improve. I have had my copper IUD going on 2 years and I’m starting to get very concerned. I have irregular bleeding that is heavy. I’m getting my period twice a month now. My doctors are no help; every doctor thus far is unsure what’s going on. I’m not sure what to do. But I’m going to just get it removed.

Some patients might experience light bleeding or spotting that extends beyond the initial 4-6 week “adjustment phase”. If things don’t resolve within the first few months, I would recommend that you schedule a follow up appointment with your doctor to discuss the details further and have things checked out. Hi Amanda- Daily bleeding can be normal for up to 4-6 weeks after an IUD insert. I don’t think that a higher dose oral contraceptive pill would be of benefit. An IUD can be removed at any time. -Dr. Dervaitis

Hello- Generally speaking, most pain or discomfort associated with IUD insertion is described more as cramping pain (like period cramps) and in the lower abdomen. Back pain would not be typical. -Dr. Dervaitis

I got mine Mirena IUD 11th of Octouber 2019 by under general anesthesia. I'm so scared! 😭I got epinefrin injection because I'm allergic to penicilin. It was adrenalin on it. Then I feel better. And I have nausea and little pain and I feel dizzy and I been very "high" when I woke up. I got allergic reaction to antibiotic called Zinacef. I got Zinacef infusion on my iv because the stopping infections just in case. Putting intrauterine device went well.

i’ve had the kyleena for almost 8 months and i still have periods that are 10-14 long and my gyno had prescribed me a birth control pill to regulate but i’m still struggling with the long period. any suggestions?

Hi Molly- I agree with your Gynecologist- we often prescribe a few months of the birth control pill (for simultaneous use) to try to help patients with bleeding issues. If that doesn’t work (and having ruled out any “pathological” cause for bleeding) I might recommend switching from a lower dose levonorgestrel IUD like Kyleena, to a higher dose (52 mg) levonorgestrel IUD like Mirena or Liletta- sometimes the additional progesterone may be enough to lessen the bleeding. -Dr. Dervaitis
