Luke wrote this song all by himself. Its a masterpiece of songwriting. Absolutely love it. He is writing to himself. As I understand the "Take my hand" part is to take care of yourself. I love the part "Just as your loosing yourself again, look in the mirror". I should have done that many times myself...but many times its not "you" that looks back.
