Don't need reading the answer to the quiz. As you well know, it isn't a clot! Doesn't exist apical thrombus without apical akinesia. Most likely this 2DTTE shows an apical mass of uncertain nature. Cheers. .

it is highly unlikely to be a thombus without a regional wall motion abnormalities , it might be a secondary tumour unknown where it is coming from

This is certainly a mass, not likely a thrombus. With the symptoms the patient has, could it be a tumour with seeding or embolus to the brain?
