You stated you have been getting a lot of slack for your recommendation of medical grade skin products. Please, do not change anything you have said or done in the past! Your honesty and the great reviews you have makes you a trustworthy physician that is desperately needed. I follow your advice as much as I can, and see fantastic results. I wish I lived near you for further results from the services you perform. Also, to your credit, you have videos and even news casts of less pricier products in drug stores. In addition, I am quite sure I am not in the majority of subscribers that follows you (African American who now lives in Hillbilly Hell on the East Coast born and raised in New York!) So in conclusion, stay who you are and continue recommending what works! You are the Best!

In a prior video, you indicated the seven most important active ingredients to look for in skincare products are: 1. Growth Factors 2. Vitamin A Derivatives 3. AHA 4 . Sunscreen 5. Peptides 6. Antioxidants 7. Humectants Besides, prescription retinoids (i.e.tretinoin, adapalene, tazarotene) are any of these 7 ingredients available as prescription (compounding) drugs? P.S. Some lower costs products that contain growth factors and peptides include Acure Radically Whipped Night Cream ($20), Instree EGF Repair Cream ($17), Klairs Blue Calming Cream ($21), and Drunk Elephant Protini Polypeptide Cream ($68).

I got two free samples given to me: One says "Pure Hyaluronic Acid Serum" and the other one says "Bio Brightening C Serum". Should I wash my face before putting one of them on? I've heard a dermatologist say she doesn't wash her body except for her armpits and private area (and I think face, though I'm not sure), because washing compromises the acid mantle and causes epidermal water loss via evaporation. A lot of washes mess with the pH, so she just doesn't wash. Maybe she just washes her lips and eye area and nostrils? What should I do?

Thanks for yet another informative video...Retinoids are known for their anti ageing skin turnover potential but it also helps in reducing sebum but as we age the sebum production decreases naturally leading to dry skin...then wouldn't retinoid exacerbate the dryness and hence ageing?
@DrStefaniKappel
@DrStefaniKappel 5 жыл бұрын
+Alice dsouza I’m glad you bring this up. Sebaceous glads increase in number and size as we get older. This is why the pores get larger as we get older. Retinoids decrease sebaceous glad activity to make the pores smaller, the texture better and the skin more youthful. Remember sebum and oil are bad for the skin. You skin gets hydrated by water absorption and vitamins/anti-oxidants. Does this make sense? Looks at kids skin. Before puberty the sebaceous glands are tiny and inactive. Once puberty hits and the hormones increase, the sebaceous glands increase in size and activity and they get blackheads, visible pores and acne.

There is the think: I’ve start to use PAOLA’s Choice Retinol 1% 10 months ago , but it’s starts to grow many fine hairs in my face . Six weeks ago I switch in Tretinoic Acid ( 0,02 Tretinoic acid + 4% glycolic acid + polivinil A. 0,2%) from Difa Cooper ( RetinCare) , but still have my fine hair on... I’m 45 years old, Caucasian , dark blond and almost no hair on my body. Can you tell me, please if is some associations between hair grow and Retinol / Retinoid Acid? Thank you! Waiting for your new video !!!👋🏻

If skin cells under a microscope present differently for a healthy versus unhealthy person, can you describe what a doctor would see in both cases, and how it differs?
@DrStefaniKappel
@DrStefaniKappel 4 жыл бұрын
Under the microscope healthy skin cells are plump, round, have small nuclei and show a clear gradient of maturation from the bottom basal layer up to to the top stratum corneum. Unhealthy, older, damaged skin cells have big nuclei with a hi nuclei to cytoplasmic ratio, not well organized cell maturation from bottom to top. Further, the elastin fibers are curled up and fragmented vs healthy elastin fibers are thick and straight. Collagen is skinny and sparse within the extracellular matrix vs healthy collagen is pink, thick and chunky. The dermal epidermal junction is flatter in sun damaged older skin and more undulated (which we call rete ridges) more pronounced in younger healthier skin. I love that you wanted to know, and I’ll post an Instagram story just for you. 🤓

Why do medical grade skincare products contain fragrance and a bunch of ingredients that cause irritation ? I used is clinical and my skin was never worse. Now I’m using fit glow (an organic brand that is sunflower oil based) and my skin has never been better. But you say oils are terrible for skin. I’m so confused ! I love your videos and I want to try skin medica or another brand you recommend but I’m just honestly super confused.
@DrStefaniKappel
@DrStefaniKappel 5 жыл бұрын
I'm hoping that I can clarify this confusion. The only Is Clinical product that I've ever recommended is the eye serum. "organic" and "natural" may be misleading. Poison Ivy is "natural and "organic" so just keep that in mind. Oils are one of the worst things that you can put on your face. Check out my KZbin video on this subject for the sake of time. In an ideal world all products would be fragrance free but that is simply not possible. Even "fragrance free" is a fragrance (in fact, that was a question that comes up on our derm boards). Oils as a main ingredient is bad but its ok to have some oils listed on the inactive ingredients as long as its not the mail ingredient. Oils dilate pores, cause blackheads and cause textural irregularities. Sometimes skin can get irritated at first as its getting healthier after switching to medical grade. This occurs a lot with the ZO skin health line. Weak, dehydrated skin gets red and irritated but once you work thru that period the skin becomes stronger, more hydrated and tolerant. Skincare is A LOT like fitness. Think of it as skin fitness. When someone is out of shape they get sore the first few times at the gym but as they stick to it they build muscle, look better and eventually work out without being sore. What if that same person who was unfit refused to go back to the gym after being so sore the next day they couldn't walk. Same thing as fragrance. Its sad to not use a great skincare product because of something trivial like fragrance unless you have a legit allergy to it then that is a different story. Not using a powerful skincare product with health benefits bc of a fragrance is like saying I don't want to ever work out at the gym because the gym is cold and I might get sick and catch a cold. So instead you never work out to achieve optimal health. I'm trying to draw analogies bc this is a difficult topic to understand.
@flowergirl8887
@flowergirl8887 5 жыл бұрын
unfortunally I am going to be unsubscribing from your channel.The fact that you think it is okay to have fragrance in products makes me question all of the other advise you have given. I think you are so hung up on so called medical grade skincare, that you don't' care if it has fragrance. even other Dermatologists recommend fragrance free products because fragrance can be irritating and cause inflammation which causes collagen loss. I wish you all the best.
@Kayyy8585
@Kayyy8585 5 жыл бұрын
Youngr by Stefani Kappel MD I understand what you’re staying and that’s a great analogy because I’m a personal trainer so I get it , haha ! However, I used medical grade skincare for months and had way more blackheads and weirdness than when using my organic oil based fit glow products. I’ve been using fit glow for months now and my skin is gorgeous. I rarely if ever breakout. So how is it that you say oils are terrible for the skin? How about looking at skincare like diet - one diet works for one person and one diet might not, ya know ? Everyone’s skin micro biome is different. I would be willing to give medical grade skincare another try with the products you suggest but I’m a little weary because fragrance in fact does cause irritation for many many people including myself, as I have flare ups of mild rosacea when using anything with fragrance in it. There is no place for perfume on the skin ! There’s no point ! Like .... why ? And I just have a hard time reasoning with the idea that there is “Only one good way for every person”. It’s like saying everyone should be vegan. Or everyone should eat meat. The truth is everyBODY is different. Skin is no different in my opinion. Obviously my skin loves my organic sunflower oil based products, and I constantly get asked what I use on my skin. I love products so I really do want to try the ones you recommend and probably will eventually but what makes the products you recommend better than IS clinical ? A dermatologist told me that IS Clinical was the “best”. I used the cleansing complex and pro heal serum for months and it was not good.
@Kayyy8585
@Kayyy8585 5 жыл бұрын
Also ! I just read the ingredients in one of your recommended hydrators (Alastin nectar) and it contains soybean oil. Soybean oil is pore clogging. Why is this used in this product if oils aren’t good for the skin ?
@DrStefaniKappel
@DrStefaniKappel 5 жыл бұрын
rose I’m sorry you feel that way. Yes, in a perfect world every skincare product would be fragrance free. I agree with you 100%. However, avoiding all fragrance at the expense of really effective ingredients is like not going to the gym to work out because it’s air conditioned and you might catch a cold. So you miss out on all the benefits of a skincare line because of fragrance. I respect your opinion and am only here to help. I met with the formulaters if Alastin last week and there has not been one reported incidence of sensitivity to any of their products. Yet people using these products have improvement in their skin that is life changing. Plus they don’t have to wear make up...which has fragrance.

Medical grade skincare means nothing. Its costly and has many added fragrance or essential oils in it that your skin does not need, stick to a basic moisturizer and a sunscreen. Good skin is all about how you are managing your life. Stress, sleep, exercise, healthy eating.

+Casey Conner With all due respect you should be following a beauty blogger. Not a board certified dermatologist if that is what you believe. I’m here to share knowledge and educate. If you don’t want that then you’re entitled to your own beliefs. I see patients everyday who have had their skin transformed. Even followers who have never seen me in person but use the products I recommend on Instagram send before and afters with an improvement in in their skin. They can finally go without make up. Medical grade has technology that non-medical grade lacks. I see it every day and the transformation in people’s skin is proof itself. I’m a firm believer that a healthy lifestyle is also an important component to skin health which is why I made this video.
