After getting End1, if you press "すた〜と" to begin the puzzle game again, neither the hospital nor the boy will appear in the background (although Ai-Chan will be present on the bottom right). In addition, there's a chance that Kashiwamochies will replace one of the sweets, which is pretty neat Also I just realized this detail in the title. The "Ai" in "Ai to Yuuki to Kashiwamochi" makes it seem like it's a story about love, possibly between the girl and the boy. But it seems like the title is instead referring to the protagonist is named "Ai". Very clever choice to misdirect the audience.
