Any time multiple helm stations exist the potential for serious problems leading to accidents also exist. Maybe a deadman style switch to limit the functionality of wing helms is one potential solution but in the end their is no engineering substitute for crew safety knowledge/standards.

What prohibited the builder from making a safe size storage area for the wing station control's. Seems like you should be able to close the wing station with the control's in neutral on a yacht that size. This kind of mistake should never have been able to occur when the solution is as simple as making the housing larger or shorter throttle control's.

I was a surveyor for over 20 years and i have witnessed the consequences of many debilitating and fatal injuries, i really feel for you in this instance, my heart goes out to all the people involved. I am surprised that this has never been addressed by Class or Flag state???

Real ships have this. Station interlocks are a well-established technology, mandated by all classification authorities. The "Superyacht" industry skates around the regulations by flagging in weird places and lying about tonnage and cargo.

Thank you. These videos are never fun, but I know they are very helpful for safety. I have worked on US Navy flight decks with all the dangers. There was always lots of safety training. Witnessing accidents in real time and training films always kept safety on my mind. Reading and reviewing accident reports are also very useful. This how we can help keep these tragedies to a minimum.

1) I can’t believe that such a design defect is allowed to exist! 2) Knowing that it exists, while the engine is powered on, there should be someone responsible at all times supervising those controls - it’s like leaving a loaded gun just sitting out where anyone could pick it up and hurt themselves or others. Good of you to put this up. - For about $100 worth of foregone fiberglass work ( or, as another has already pointed out, about the same value in shorter control handles ) one person has died and another been injured. This is inexcusable, and neither your yacht nor the JeMaSa should have been released by their respective builders/construction managers or certified until the problem was fixed.

I was working on M/Y Hadia (ex M/Y Jemasa).Since the accident, minor modifications have been made, such as lowering the throttles and gear levers in order to close the wing stations without having to change the positions of the levers and a pin to maintain the the wing stations opened. The biggest changes concern the procedures to moor the ship, each crew member must remain at their station to avoid the displacements near the controls and both engines are switched off once the mooring and spring lines are in place.

So next time they will blame you. The ship should be seized by the authorities until they demonstrate a full interlock system. Only one console should be able to command the engines and rudders. Wing stations need to have Safety Redundancy procedures BUILT-IN to the ships console system. Several possibilities could have easily been implemented and prevented this traject accident. 1. Redesign the structure itself to handle closure when throttles are in neutral, or for that matter REQUIRE that the throttles are in neutral when closing the wing station. 2. When the Wing Control station is CLOSED the system will automatically SHUT DOWN control to that wing station regardless of whether a manual transfer has occurred or not. 3. Prevent the wing station from being physically closed until CONTROL is manually transferred back to the HELM or to the other wing station. 4. Docking sensors that tell the ship's controls you are in a docking situation. These sensors would be on each of the cleats on the boat itself. Also, cameras can be used like in cars to detect the location of the yacht on a dock or mooring and prevent full throttles in either direction from being engaged when lines are connected to the dock and the boat. 5. Require someone to physically be attending to the controls at all times, with a LOCK mechanism similar on a tender, that if it comes detached automatically TURNS OFF the wing station. 6. Along with #5 there needs to be a Maritime Policy Requirement that states someone MUST remain in control of the WIng Station at all times unless it has been authorized and manualy transfered to another station. Items 2 & 3 above can easily be PROGRAMMED into the system of all Yachts and would have prevented this tragedy from ever occurring in the first place. All 4 items should be STANDARD REQUIRED IMPLEMENTATIONS on all YACHTS of this class and highly recommended in smaller classes.

The written report notes that since it was published, several unofficial reports of similar incidents have been received, but no injury or damage occurred. How unfortunate that nobody in the yachting industry thought to use the confidential reporting system CHIRP to alert the authorities so that action could have been taken that might have avoided this tragedy.

I deeply saddened of the death of such a young woman. I pray for her family. I love your videos, I could see you were deeply moved when it comes to the loss of life. Why wasn't there communications between the deck hand and master. I'm not sure how one can assume it was OK to stow the control station without verifying the bridge had resuming control first. This shows the yacht builder did a poor job in designing the control station and inclosure.

On-board the vessels I have been on we always had a person at the station and in control until the master or the Officer of the Deck properly relieved him/her. This would have greatly reduced the possibility of this happening and also allowed quicker responses to any other issues.

Well it would not prevent the situation as the levers need to be put to full throttle prior to close the bridge control. But it would prevent an accidental closing which would put engines in gear.

@@Andrew-wv7qp it’s a good point anyway, but it would be easy to disengage the controls by a switch when closing the bridge. But you need to lower the levers anyway before closing the bridge. So discipline is best to avoid accidents, check who is in control and the captain is in command.

I’ve only watched a few of your videos tonight because I have been watching eSysman SuperYachts this week…YT led me to your channel. It’s not even like I need to know about this accident but it was the way you presented it…with great sensitivity..and I felt out of respect for this young girl that I needed to hear what happened and spend a moment extending my prayers to her saddened family …and to the deck hand as I, like others, hope he is able to recover from this awful accident, bith mentally and emotionally. Even it the wing stations had a light on them to warn the wing station is energized and in control, could of made a difference. Simple rules with the crew, only allowing the captain or designated person who is operating the wing station to touch it while it’s open, may have prevented it. Your right though, engineering the problem away would be much better then crew procedures to deal with an extra hazard.

I would retrofit a Deadman switch into the console and whomever is there would have it attached to their person like small personal watercraft like a jet ski. If a person leaves then the console becomes inoperative or kills the engines. Whomever is supposed to be operating the wing station should not leave until it is disabled by the bridge and if using a Deadman system they should not be allowed to leave either; both a fireable offense.

How about a requirement or a mechanism that would prevent a wing station from being closed if it is still in control of the ship. Shouldn't the panel have some very clear lights, signs, bells, indications that the station still has control over the ship? I think that would help prevent this kind of incident in the future.

I once had something like this on a Feretti where the cover of the side controls slammed down pushing the throttle forward. The boat jumped forward pulling a cleat out of the dock but fortunately no one was hurt. There were many things I didn't like about that boat and this one was at the top of the list.

Could those controls have an interlock system engineered into the system so when the entire assembly is close it would automatically return control to the master inside the wheelhouse? Michael- I get what you are saying, and I agree. They should have some kind of electronic disconnect in the event someone inadvertantly closes the wing station. If the controls were electronic, a disconnect switch on the door should be easy. But, As I see it these are fuel driven engines with mechanical controls. If this were a car, the shut-off or disconnect would have to be on the battery lines but diesel engines are the norm on boats and I don't know why the manufacturers couldn't come up with a similar disconnect. Perhaps it is an option you have to pay extra in order to get it. Similar to how, on the Boeing 737 MAX, the airlines had to pay extra to get a stupid warning light that could have saved the lives of 700 people in 2 accidents. Personally, I think the fault lays in the design that the door wouldn't close with the levers in the upright position. Perhaps the designer thought this was safer and prevented the wing station from being forgotten by making the doors easy to close?

The control should have a physical lockout when unattended. Just about all aircraft have a 'gust lock' which when installed locks the controls and in small aircraft the lock covers the igniton so you can't even start the aircraft before removing the lock. Or even a simple kill switch installed on the wing station to isolate it when the captain walks away would have prevented this.

Even flashing red lights showing that the station was ACTIVE would greatly help. The problem with the setup is that the "master" control is in the bridge and the wings are slaves. So you have to leave the station to revert control at the main helm. For obvious reasons you can't CLOSE the station without going to the bridge first and then go back out. The problem with EMO switches is regulators want to tie more and more into them when all they REALLY need is an EMO switch that kills the wing station. (and needs to be reset when you want to operate the station from startup) Of course that means people will use the lockout like an on/off. But yes, a poor design.

A recommended redesign of the module model of the side wing manoeuvre controls, to install a human fail-safe feature to stop physical intervention. I think the master of the ship should be in control at all times with the ship engineer whilst the vessel goes through shut down procedures. Had a summer job working on a large yacht when i was in college. I vividly remember the the First Mate drilling into us trainees the extreme danger that ropes posed during docking procedures. Made it very clear to us that it was a matter of life and limb, that we had to watch what we were doing and where we were placing your hands and feet at all times. Also, that unexpected wave action or mistakes in ship handling could result in broken/snapped lines, so we also had to have our heads on a swivel and be aware of everything happening around us, because if a line snapped, you did not want to be in the path of a rebounding line as it could be deadly. Civilians or those not engaged in mooring activities SHOULD NOT be in the vicinity when a vessel is mooring or unmooring. Just in case.

same thing with winches when offroading, stay away! when winching we tie a wieghted tarp mid point on the winch cable, this greatly reduces snap back and whipping if the cable were to part.

Tristan, would it be possible for the stations to be locked (possibly with a separate key which is only held by the Master/officer) in the open position for the duration that they are being used? Or perhaps a key to activate a safety feature, together with a toggle which is positioned next to the throttle, but on the closing side of the station, so that if the station is accidentally closed, then as soon as the toggle is hit, the engines immediately shut down. Obviously the toggle would have to be some gizmo (like a lever) that can be raised and lowered. So basically: unless the key for the safety feature for the toggle is activated, the station can't be used, nor can power be transferred until the safety feature key has been turned off.

Just an idea. The safest way is to make the throttle handles removable and when they are remove it disables that wing station. Then you have to take them to the bridge or another wing station to make it active. Maybe electronically keyed with a chip or something like that. Be sure to order a spare set and keep it under lock and key.

I would expect a multi-million dollar yacht to have the simplest of fail-safe systems, a $1 switch on the door. If the door is not fully open, the station is locked out. If the station is currently selected as master and the door begins to close (even slightly), the station is disconnected and locked out.

I have also Captained a yacht where the wing stations were compromised by style. This is an unspoken problem in yachting, where style is everything. Far too many times style is placed over safety and functionality.. Often times the yachts designer is a well known designer / stylist/ architect…who has little to no knowledge of operating a yacht at sea, and are a times very reluctant to accept the fact that at times it is essential to consider function over form.

Sadly it takes a loss of life to make serious changes across all industries hopefully the cruise industry will recognise this as a potential risk and change the design by changing the rules.

Someone did not follow procedures ,he should be dealt with . every one should not pay a price ,I'm a builder to do every thing by the book is so impractical , you better have a lot of money , and time ,, every one should drive at 40 KLM max also then.. ,Life is not Safe and life is not fair , i disagree with your assessment ,,Australia ..

Absolutely, 1000% agree that stowable wing stations need to have the ability to close while the throttles are in neutral. I work in aerospace and one of the design considerations we try to build into aircraft is the inability to do something incorrectly or unsafely. There is a Japanese term, "poka-yoke", which means mistake-proofing, that we consider wherever practical. I would like to see similar engineering standards in use for yacht design and manufacture as we use for aerospace.

A $0.50 micro switch on the wing station is all you need. If closed it can disable the station. Same as a microwave door, but wired opposite .

How about all command stations requiring a key be present to operate. The captain simply pulls the key from the station to deactivate it and inserts it at the main helm to shut the engines down.

There should be an engine shut off switch placed on the wing stations so if the wing stations were not fully opened the engines wold automatically shut down preventing this sort of negligence. If the wing station were attempted to close the throttle settings would not matter because the engines would die. If control were already set to the bridge then you could shut the wing station without incident and position the throttles into any setting. When opening the wing stations the throttle settings of the bridge should be transmitted to the wing stations moving the throttles to match the bridge throttle settings, before control could be set to the wing stations. If we can have motorized faders on recording mixing boards via midi control then it should be easy enough to have powered throttle quadrants and safety shutdown fail safes.

Absolutely love your show and what you do. At 40 years old, I look up to you and what you have achieved in your career. I'm actually working to become a licensed builder. I don't have much time left before I can take the test and I'm very excited! The point I wanted to get across is many people use these better help and things like that. I'm not knocking people getting help for mental health. The only thing that I am concerned about is the database that gets accrued and people's medical information that gets shared and turned into a currency. I'm not sure if that's what goes on here but it makes me wonder about the old mental health facilities many years ago that would that people and electrocute people in the gains of mental health. It's just food for thought. This company came out of nowhere and now it has millions and millions of people's mental health data. We all know that data is the new paperback dollar. Or the metal pound! Keep in mind I'm a dumb American, smart enough to at least Dodge the obesity problem! Also I have struggled with addiction and I totally totally respect anybody that's willing to get mental health help. I think it's very important. I also believe that the younger generations don't get taught accountability with their friends and family. Having an accountability partner you can trust and talk with us much better than having a doctor you pay money to. A friend will know who you are a family member knows who you are. But it's just food for thought. I always say if you have mental health issues going to a 12-step program that's anonymous. Love your show and I love what you're doing! I think you treat people the way that any manager should treat people. You make a good boss and maybe one day I'll the accredited enough and perhaps can apply for crew on your ship! Proper action: move wing-station in, walk past, open wing station back up again. Do not touch the throttle or any controls until you have confirmed that engines are stopped and docking is complete. Stupid mistake.

To rectify that problem without to much cost the controls that are used needed to be lowered by 40mm which can be done by making the control box black top recessed or the unit top sloped down at the back to move the control levels down. They look like they are electrode controls not cable.

Maybe a quick fix for the throttle handles is to have them attached with a quick disconnect where a press of a "detach button" drops the levers down out of the way while leaving the station in neutral when closing!

As an engineer (not marine), I would call that a design flaw regardless of ship category--wing stations should be able to be closed no matter the status of the control(s) configuration or physically locked from closing if active, thus precluding that particular accident initiator.

A very tragic accident indeed. I find it quite shocking that this design is found on several yachts. It must be some of the designers that have realised that an accident is waiting to happen. It is so easy to fix this, either to lock the lid open when the bridge wing is selected or main engines are running or one can mould a hump in GRP where the handles are . Both these options can be retrofitted easily. It just requires some cable, a couple of solenoids, some 316L material

I would suggest that they install a flashing strobe that would indicate that the station is active. This would reduce the chance of someone messing with the controls.

Very sad story and especially since this was completely avoidable. As you stated, this was an accident waiting to happen. I have no idea why this is designed this way, but even then it would not have been difficult to add a switch that needs to be activated before you can put the wing station in the stowed position. That switch would automatically shut down all the propulsion, preventing a predictable accident to happen. Now this girl had to pay with her life before action was taken, because am pretty sure that they are re-designing this wing station after the accident happened. Thanks for sharing this accident report, after all accident reports are there to learn from it by explaining what happened, so future accidents can be prevented.

Cheaper than rebuilding the yacht would be introducing a wing control procedure. A sign saying-Do not close wing control until it is de-activated. Confirm control is at main helm before closing this unit. Check this box to show you understand this action,& sign.

It is hard for me to comprehend how a crew member whether it be the captain or whomever to leave an active wing station unattended. Personally, I find this to be a serious design flaw but that is just my opinion, and everyone has one. It is heartbreaking to listen to your review of this accident and my heart goes out to the young lady that lost her life and to the deckhand that suffered injuries to his leg. My condolences to her parents and loved ones. Thank you for your report on this and on your other videos as I watch them all. God Bless!

It is surely so sad to know that a girl was injured and lost her life for a some ignorance... May her soul rest in peace.. I pray to Lord that nothing should ever happen again on any boats.. God bless her.. I had tears in my eyes for that little soul..

Tristan, so very sad to hear about that story. Wouldn't it be a modification on the yacht builder just simply give a larger access for the Wing station to close? How far down would they have to move the swing mechanism for the controls, 1 in or one and a half inches? Seems to me like it would be a simple fix and that yacht builders would implement that with no discussion. Could you imagine if that happened to a high performance high speed shot as for instance a mangusta? The mooring line snap and cut a dock hand in I understand the whole concept of naval architecture and design but that is something that would not be cosmetically noticeable.

I think it is also a problem that you can not deselect control from the levers. As far as I know you can only asume command. When I train bridge crews (usualy military) I alwys tell them not to leave the station as long as crontole is actvie...

The logical design requirement would be that all functionality of the stations port and starboard should be cut off automatically when the stations are moved from their operating position. So simply, if you want to close the stations, you would first need to unlock it and with that all controls are made numb and the station can be closed safely.
