Hey Mike, 911 EMT of almost 5 years and current paramedic student here. Just a quick note on applying the TQ: Pulling the red tab towards the midline of the body should be applied in situations where you are applying the TQ one-handed to your other upper extremity. In situations where you are applying the TQ to a lower extremity or to another patient and you have both hands to work with, pull the red tab away from the midline of the body. This gives you a couple advantages: 1. You give yourself room to work. By pulling away from the midline, you won't be pulling the TQ strap into the pelvic area and/or the person's other extremity, which will make it difficult to wrap around the limb. 2. Pulling away from the midline gives you more leverage to pull the TQ tighter before even turning the windlass. 3. If you tuck the remaining strap across the windlass to keep it from coming loose, pulling away from the midline means the red tab will end up facing the outside portion of the person's limb and will keep it out of the way from other work being done (wound packing, etc.) and mostly safe from accidentally being pulled and thus loosening the TQ. Obviously, this is all situation-dependent, sometimes you just have to work with what you got, but I thought I'd throw in my 2-cents lol love the channel, keep up the great work!

Got Stop the Bleed training at work a few months ago from our local FD (maybe about the time of this video, but only just found you last week). The BHRK/CAT-T combo on the visor is a great combo to compliment that. Great teaching/explanation methods, love the snippets of humor added in - intentional or not. Thanks for all of it! You have one new loyal follower, and I think I'm about to have my brother - possibly my sister - added to the groups soon!

Man your channel is rockin.I learn something new with almost every video.I really dont have the cash to take all of the courses as im on disability for a broken neck and being pre quadriplegic but im recovering very well and im up and walking just gotta take it slow these days.Anyway I try to learn what I can from these videos and several other resources so I do appreciate what you do here and also thank you for your service to this great country.

I remember the olden days: Take off your belt, put a quarter twist and run the tip through the buckle, place it on the injured limb with tip toward the heart, put a stick into it, twist until bleeding stops, run tip around stick and tuck under. Write the letter T and the time applied, in blood, on the victim's forehead.

Thank you for this video. Really great information that’s really understandable. I spend a lot of my time traveling and working in some remote Bay Island’s of Honduras and medical care is never really close by. Great video Mike, I like the pouch and the concept. I will be adopting this in my 100 series Landcruiser and will use a M2 Ratcheting Tourniquet instead of the CAT Tourniquet. I feel it is a equivalent Tourniquet and can be easily self applied easily to arm and leg, there are other reasons I like it but will spare the notes here. My hemorrhagic package will include WoundClot vs Quick Clot as it does not have any long term effects (>8 hours) if left in the body.

You can pack that with sugardine, beatadine and sugar along with gauze with the pressure bandage. This comes from Native Americans packing wounds with honey. I shoe horses for a living and we use it all the time. I have used it on my kids, dog and my self. You make a paste out of the beatadine and sugar.

I used to carry a stop the bleed kit and forgo a simple first aid kit. But in my area of work and play, boo boos are more prominent than a bleed. So I carry a trauma kit but I also carry a simple first aid kit. Also, as a person that suffers from headaches and chronic migraines, I need headache and migraines pills. Those, benadryls and some natural remedy such as maca root, white willow, and kratom are part of my kit.

Very important video. This is a must and the most neglected aspect in our daily life. Not to long ago someone shoot himself in a leg while shopping at a mayor store. Because no one has a tourniquet this person die waiting for the ambulance. Is good to have a tourniquet on your self as an EDC. Specially if you carry a gun. Thanks for sharing. PS. Even if you don't carry a gun.

Great stuff. Another consideration is to apply the tq with the windlass on the inside of the limb (close to torso/crotch) to further minimize the risk of it being bumped loose during cas movement. PMARCHP is current algo, with P being patient safety(if applicable)

Just dropping the hospital/operation perspective of things: in the event of a limb bleed, if a pressure dressing can stop the bleed instead of a tourniquet, then a pressure dressing may be preferred as we really try to restrict tourniquet times to 2-2.5 hrs max including intra-operative tourniquet use during repair to prevent the risks of tissue death, nerve damage, and total limb loss. Like you mentioned, people can do OK with times greater than that but there's serious increase of the risk of permanent limb damage. If you need a tourniquet though, obviously use it- better to have a bum leg and be alive than be whole and dead.

Mike, First, thank you for putting out thought provoking, informative material. I've learned a ton since discovering your channel and hope you continue to put out this high-tier content. Have you tested your hook/loop visor attachment point in simulated hard braking/roll-over? Having been in an 80+mph roll over (seat belts save lives, kids), I can confidently say that almost nothing was found where it was prior to the roll. I had an IFAK in the locked center console and a holstered handgun at appendix (shameless plug for Odin holsters). Those were the ONLY two essentials still stowed where I needed them to be. My self-extrication tool, gloves, tool kit, glove box contents, etc. all disappeared. Definitely made me re-think my cab organization.

Good stuff, I had a car hauler fall on my leg and used my belt for a tourniquet and when I got to the nearest hospital the doc says you shouldnt have done that you could have lost the limb. I said it wouldn't matter if I bled to death before getting there...lol

I am not a combat medic and don't purport to be one. However I have a tremendous amount of medical training and experience particularly in austere medicine and even managed to get a graduate degree in Epidemiology and Community Health.This is a well done video and is quite effective in emphasizing: 1. Just how fast you can start your eternal dirt nap from bleeding out. 2. Just how important a tourniquet ("TQ") can be. In the 1980's during my off time from my bomb squad duties I was an American National Red Cross First Aid (and CPR) instructor. I wish I could show everyone how much attitudes towards TQ's from the established medical community have changed. In summary: If you are the average John or Jane Doe, have a TQ readily accessible in your home or automobile. This video illustrates this very well. The key phrase "readily available" might mean you need more than one placed in strategic locations. If you are a gun totin' civilian, have one on your person and another readily accessible such as in your automobile. Some TQ designs can be awkward to conceal so choose accordingly. Other designs such as the SWAT-T (no financial affiliation) can be simple to conceal. If you are law enforcement have a TQ on your person and a few more in your unit. There is a plethora of TQ holders designed to hang on your duty rig and finding the right one will not be a challenge. If you are military and/or in a combat zone carry a TQ on your person. Depending upon your duties it might be necessary in some instances to carry it concealed. The following statement might elicit a few laughs from the intellectually challenged but this also applies to Navy guys and gals aboard ship or Air Force nose pickers sitting on the ramp waiting to fuel a helo. A mortar shell or IED doesn't know you're rear echelon. If everyone carries a TQ there should be enough locally to handle multiple casualties or casualties with multiple wounds. If your military job is to ballistically reform bad guys (and girls) carry two TQ's on your person. This also applies to law enforcement SWAT or tactical unit operators. If you are Spec Ops and you're going out to Ninja your way into the party, carry four TQ's on your person. The rationale here is that such units are typically small so there will be a limited number of TQ's available. This is especially true if you have sniper and/or recon duties or are in a situation where you operate alone or with one other operator. Be aware that one size doesn't necessarily fit all. If you are a teen or adolescent or are responsible for same or if you are genetically underweight or just went through SERE school or if you are a large land mammal or buffet slayer plan accordingly. Some manufacturers do offer models to fit atypically sized individuals and it may even be necessary to improvise in certain situations. There is nothing wrong with improvisation-manufactured TQ's are definitely newcomers to the medical device landscape. Just Improvise ahead of time if at all possible. And don't forget a few for the dog!
