having a kid in the 1800s and early 1900s was a Miracle. A lot of times kids die being born or shortly after. kids living long time and with low death rate as they do now is the new thing under 100 years

This episode is essentially a lesson on the flow of life. Hughes was removed last episode, while new life was introduced this episode. Unfortunately it was a lesson learned too late for the Elrics

Remember, Ed and Al have a lot of trauma surrounding human life and death. Given their background, It's Not Unusual at all that they would be completely overwhelmed at the idea of being partially responsible for helping deliver a baby.
