It's interesting because both Columbo and Dr. Mayfield do highly performative, strategic explosions. Columbo does it seemingly because he knows it will slip up Dr. Mayfield in some way. Dr. Mayfield does it to conceal how he hides the sutures. Both of them were probably venting real anger (Columbo at the possible death of another man, Dr. Mayfield at feeling trapped), but both of them only show that anger because it suits a larger purpose. Dr. Mayfield is an interesting foil to Columbo - smart, controlled, but sociopathic.

One of the best episodes in my opinion. Nimoy was just so perfectly reprehensible as the murderer, and the twist that the initially intended target ultimately survives makes this a very compelling script. Great acting, great writing, and executed flawlessly.

They should have had Nimoy and Shatner guest star together in an episode, but also framed them together in the background of shots, watching and whispering. At the end of the episode when Shatner and Nimoy get arrested, the Shatner and Nimoy in the background disappear in a column of sparkles.

Wonderful episode and a great cast. Leonard Nimoy, Will Geer, Anne Francis, Victor Millan. This was a rare episode when Columbo got really mad at, and actually hated the murderer. Columbo usually found a redeeming quality about the suspect, but not this time.

Thanks to my father who always watched and loved Columbo right from the start, got me into it when I was a kid in the mid 80's. I feel like LT. Columbo was my uncle ha ha! Peter Falk was amazing! This episode used to be up in full on this channel until they removed it perhaps for copyright reasons. The first time I saw this episode was around 1982 which was the time I worked late night shifts in a hotel kitchen. Fortunately my older brother had just bought a VCR and he would record various episodes on videotape so I could watch them when I was not working. I think he still has those old Columbo recordings on videotape to this day and an old fashioned CRT television and video recorder to play them back on.

A lot of people talking about him slamming the desk. I think he really did get angry for a second. I think it was only because Nimoy's character was laughing that he got so mad. Most of the murderers on the show are at least respectful enough to take their crime seriously, know that they have done is wrong somehow. But Columbo can't stand more than anything a man who is pure evil because it is the diametric opposite to what he stands for. And it is extremely disrespectful to the victim.

Love how Columbo is nauseated by the sight of the first surgery, but is fully locked on Mayfield when he's replacing the heart valve of the victim. It shows that, while he tries to hide it from the criminals, he's fully aware and the one in control all the time.

Could someone please explain this Episode to me. Honestly, I can’t figure it out even with all the evidence - how did the doctor do what with the stitches? And what did he put in the medicine? Those writers were genius.

The doctor used dissolving sutures for the heart operation knowing that once they dissolved, the heart would fail, and thus his murder would be complete. He basically used dye to make the dissolving sutures look like permanent sutures. Once Columbo assured him that upon dying, an autopsy would be performed, the doctor knew he had to replace the dissolving sutures with the correct permanent ones. He then hid the removed dissolving sutures in the lab coat Columbo was wearing as they all barged in and said they would be searching everyone. As for the medicine, I don't know.

SPOILER ALERT: I love every part of this episode except the end; the payoff doesn't seem credible. Couldn't Dr. Mayfield have simply said that Columbo planted that suture in his own O.R. gown? At any time? Or can lab tests prove that those sutures were actually inside of Dr. Heideman?

There's a pattern that I always find in every episode: First, there is never an eyewitness to the crime, so Columbo has to build up his case on circumstantial evidence, and second, almost all criminals are wealthy enough to hire a high-priced lawyer who can tear Columbo's evidence down to shreds. This is a typical case.

Even by the 60s, DNA evidence was advanced enough to test the sutures for trace amounts of the victim's genetic material. Mayfield could claim Columbo put them in Heideman himself, but I doubt that'd hold up in court.

In real life a guilty surgeon would have grabbed that hospital smock the second lt Columbo left his office. Of course in real life, few doctors would ever attempt to kill someone during surgery.
