In this disarmingly courageous and hilarious talk, Toby Walker exposes the obstacles facing the transgender community when it comes to accessing complete and competent medical care- in the hope of ‘making being trans less fatal and more fun’.
Toby Walker is a trans journalist and LGBTI+ Liberation Officer for Westminster University, where he is in his final year of study. He is particularly interested in intersectionality in news and the politics of who sees what.

I’ve been questioning being FtM for a few years now, but every time I get certain of myself being trans my mind thinks I will regret coming out and that I’m not trans. This has caused me dozens of breakdowns and I don’t know what to do anymore.

you don't need to be certain! if you feel like you want to express or present yourself differently, make those changes and let the labels and pronouns come later. if you're able to, maybe talk to a close friend about your feelings. stay strong :)

Someone recently told me it is the moments of gender euphoria that matter? Like does it feel great when you’re not misgendered? Also when my son went to the doctor for T, she told us that starting hormones for people who aren’t sure helps them know. Like if they feel great on T then there are most likely on the right path. Some non binary or non conforming choose to have top surgery but not do hormones. Whatever you decide is the right thing. Whatever makes you feel good about yourself or more comfortable in your own skin. Also if you think your trans, come out as trans but then change your mind that is ok too!!! This is your life no one gets to decide what is right for you!!! Check out some good counselors, that work with gender dysphoria. Good luck, be easy with yourself. 💗💕💗

@sonoki82 I didn't come out as a FTM until I was in my 50s even though I knew as a toddler I was meant to be male. I'm 61 niw and my life is so much more fulfilled. I consider people like you to be nothing more than uneducated bullies.

I'm terrified of going to my doctor because I dont have "the narrative". I was a typical girly little girl who then became a bit of a tomboy and less comfy with girly things, and then realised I was a trans guy at 16. I dont have the "proof" that doctors want, despite the fact that I've been consistently feeling like this for 3 years and the thought of never being truly seen as a man makes me want to be dead. So yeah, this is why I'm terrified to talk to my doctor, or anyone tbh, about it

I guess you could say that I have some controversial opinions on the subject of gender identity and trans people however this makes so much sense, we should strive to make sure everyone has access to healthcare and can feel comfortable in themselves.

I'm a trans guy, and a med student. I trully hope through the next generations we could actually change the medical field for the better, until we don't have to justify our transness with stories, or where we don't have to worry about not being able to get medical help because we might be denied from the doctor's office.

I’m trans and from Wales and, as he said, we have to travel to London to get any care, and that is HOURS away. I’m closeted, but I have a friend who has to take time off of college just to make check up appointments that are hours away. It’s ridiculous.

Always remember you are your doctor's CUSTOMER. It's no different from shopping at a store. If it's not a match, just FIND A DIFFERENT DOCTOR.

I would phrase this concept a bit differently ... You are not only the doctor’s customer, but you are also the doctor’s BOSS. For both reasons, all doctors should treat each of their patients with respect.

It's a complicated matter. Trans people have a right to be considered with respect in society. But a doctor, in his job, is not held by society's movements. As a doctor, you cannot, for practical reasons, "switch genders" on documents, because of healthcare reasons. For example: Blood donnors. Biologicaly men people can give more blood. Female less. If a biologicaly woman person considers himself a male, it's all well and good, i'm all for it, but that does not mean the doctor can take more blood. It would be dangerous for that person's health. Same thing goes for surgery and anesthesia for example. It can be quite dangerous (lethal) to miscalculate the dose, and biological gender is a part of that calculation. So yes, doctors when interacting with patients should respect them and adress them in the proper manner, but that is a social matter, not a medical one. In medecine, men and women are cared for in different manners because their biology is not the same, and applying one to another is dangerous. Oh and Omar, Socialized healthcare makes me able to pay 10 to maximum 50 euros for an emergency room check-in and all medical attention I need when I break an arm. And I work, so I'm not a "vermin" for society. Everybody takes benefits in such a system.

Adylom Lomlom I agree as far as completely changing documents, but the doctor could at least write a note to use the right name and pronouns and file that with the documents. As for the switching doctors thing, that can be tricky because you can’t guarantee that another doctor will be a good match either. Unfortunately, most doctors are inexperienced and don’t handle things correctly when it comes to trans healthcare. My doctor was ready to give me testosterone hrt before I even had my hormone levels checked by an endocrinologist, and that’s really dangerous. He just didn’t know better because he hadn’t had any trans patients before, so it slipped his mind.

Being trans isn't a mental illness but it is a medical condition that needs treatment. If it wasn't, then hormones and surgeries wouldn't be available on the NHS. And it's very important that they are. So I hope this guy doesn't mean he wants it demedicalised completely

Basically anything that can require medical treatment is a medical condition. You don’t have to view being trans that way, but I do.

i’m trans and i believe it’s okay for it to be classed medically as a mental illness. it may sound like an insult, but if they view it more as something that is limiting you or needs to be fixed, rather than just a 'hobby' or phase, that’s when it gets recognized as something that needs proper attention. if it’s (gender dysphoria) classed as a mental thing, like depression etc, then the doctors need to 'fix' it, by giving therapy and hormones and surgery etc. it needs to be seen as an illness because then there’s reason to help people transition and lessen their dysphoria etc. i get that 'mental disorder' can be used as an insult, but it’s what gets it recognized as a medical thing rather than a phase. no one wants it and that’s why it should be given attention

From what I understand, this is kind of how it works in japan...possibly some other places...and it seems to be what works for them. So I see your point. The only real issue with gender dysphoria being classed as an illness though is - as you point out - the stigma of it being an illness, but also the fact that then it's dependent on the doctor and whoever is defining the official symptoms - and it's almost definitely going to be cis people making the decisions - to define transness. And as he kind of touches on, if the doctor decides you're not "trans enough" based on the list made by cis people, then you won't get the help you need. I like the model we currently have in Texas, where it's informed consent - meaning you can go to a doctor who can prescribe hormones, describe that you're feeling dysphoria (or that you feel more comfortable when coded as another gender), and then get the prescription based on discussion of the results you want to achieve with HRT, assuming you're in good health. This way, drugs like testosterone can still be regulated and there is a chance that the appointment at least can be covered by insurance, and that the therapy will be a legitimate medical expense, but it gives more power to the person who is ACTUALLY trans.

Thank you for saying this, I couldn't have put it better myself. As a trans guy, I actually find that people are much more understanding and sympathetic when I explain my perspective with gender dysphoria as a mental condition. All these people that want the negative stigma removed from "mental illness" that go on to deny gender dysphoria of being a disorder is just adding to that stigma. It's not a bad thing to have it labelled as such, and in my opinion it would be better that way. That's just me though. I worry for young people who think they have gender dysphoria or say theyre trans but lack dysphoria and decide to transition anyway, only to regret it while also harming their bodies.

Thats like saying a doctor doesnt know how to treat schizophrenia unless he is schizophrenic. There are a lot of gender clinics and child psychologists being FORCED to affirm gender transition regardless of the circumstances. Doctors and experts have been attacked and fired for not affirming all cases. Trans activists are also changing the medical manuals to deny that being trans is a disorder, which begs the question, why do you want medical assistance if its not a disorder. Trans activists want everyone affirmed even at a young age and given medical treatment but I think its a recipe for disaster. There was a study around 80% of kids who say they wanted to transition as a child, grew out of it by the time they reached adulthood. I think asking trans people to treat trans people is also a recipe for disaster.

Ajskdldk I swear this is so crazy lil story..: I came out to my mom at the doctors because I went there for a limphnode I was v anxious about.. my doctor just stopped me and said..'has your anxiety gotten any better?' And we were talking about things that bothered me, etc etc then it lead to me saying I was trans lol

Im trans girl and ive been rejected by both biological genders either causse im tall and or have a fake genitalia (cause of my srs) and i could understand them for being nice about the situation and not being rude like transphobic people and im also insecure because of it :( but hey im still best friends with them.

Im a trans man, and id disagree with the “being trans isn’t a mental disorder” because A. Gender dysphoria is quite similar to body dysmorphia and that is a metal disorder in which you, in layman’s terms, hate your body. Honestly they are so similar the only difference is dysphoria goes away when you transition and body dysmorphia has to go away on its own B. If being trans is not considered a mental disorder insurance companies will stop, in lack of a better word, insuring our transitions. They will make us pay full price for hormones and surgery. I don’t know if you’re aware of this but hormones are very expensive C. The way trans peoples brains are set up are typically more similar to the gender they identify with than the one assigned at birth. However, it is still, in a case of a transman, a female brain. Making myself consider this more so a mental illness than a “feeling” or a “i just know” kind of thing However, this is my opinion and I would be interested in hearing other trans peoples opinions on why is isnt a mental illness

AlexThatItalian It's proven not to be a "female brain" in trans men case, and vice versa for trans women. Not by structure, nor by response to stimuli, and surely not in psychology. Debate's up if it'd be a "male" brain or intersexed brain, but its surely not a "female" one. "" used because there's no such thing as male or female brains, just statistical groupings.

There's a difference between a mental illness and a neurological condition. Mental illness is more of a side-effect, and it can be temporary.

Gender dysphoria itself isnt a mental illness. It is only if the feeling is si strong you cant live everyday life. When It turns to gender dysphoria dysorder, wich is a mental illness.

Lola K I wasn’t the only guy sadly. I thought it was pretty weird too, but couldn’t turn down the opportunity. I likely wouldn’t do the same today- unless I was specifically discussing male privilege and feminism from the perspective of a trans man who knows how people treat women, if that makes sense.

A doctor thought I had chronic daily migraines because I’m “not the norm” and a teenager. (I’m asexual and agender) me and my dad had a conversation about it later. But I’ve been agender before I got the migraines so XD

I assume he's talking about the UK but it's kind of relatable to the US too. I definitely avoid going to the doctor for things because I know I'll have to out myself and then it's anyone's guess how I'll be treated. So usually when I have an infection or something, I'll just wait it out and hope it'll go away on its own. I do have a nurse practitioner I see who's trans-friendly, but if I need to be seen right away for something it doesn't always happen. We're lucky though in that in many places, we can find people who do "informed consent" for HRT, meaning in most cases we can get a prescription right away. But then of course you face issues with the pharmacy - who will often do everything they can to make it difficult or impossible for you to fill the prescription- and insurance companies, who either don't cover HRT or make it extremely difficult to get coverage (as in, there are a bunch of forms for clinics to fill out and often they just won't). Things have gotten better here (well unless you're going to join the backwards military now), but there's still a ton of progress we need to make. I really want there to be a day when I can be like "oh I have an infection - I'll just hop on over to urgent care and get some antibiotics" instead of "whelp time to take my chances". In any case, I think that our informed consent model is a good balance. It allows for medical help for people who want to physically transition, without the issue of having the mental illness requirement - which in all honesty is kind of just a list that allows cis people to define transness and can PREVENT people from obtaining the care they need.

I'm fortunate enough that my doctor was just like "oh cool". But because phonecalls sometimes get picked up by my parents I have asked them to deadname me and misgender me on the phone. I mean, I also get misgendered unintentionally because... Like... How are the staff supposed to remember? Most people aren't trans, and I don't look like a guy so.... 🤷‍♂️

Depends on the age in my opinion, most children eventually grow out of these behaviours, but if he is a teen then, by all means, talk to him about it. Either way, be supportive.

ask him if that's who he wants to be, see if he enjoys feminine things, ask him if he wants to be a girl. if you hit most of those markers than maybe experiment with letting him present as female at home. if not then just let him be a kid.
