Had an issue with pelvic floor dysfunction a few years back. Painful/burning urination, sex etc. Did pelvic floor therapy and it was a life saver. Never knew it was a thing but I am so thankful for it. Thank you Dr. Mike and Dr. Malik for this interview. Hope it helps a lot of guys that went through or are going through the same thing I did.

Listening to this podcast makes me relieved to learn but also devastated. I had an ex that struggled with ED despite being in his early-mid 30s. He used to smoke and blamed that for part of the issue, but his father also died from a heart attack when he was younger. I always tried to see if he would be willing to talk to a doctor because I remember Doctor Mike mentioning it can be a sign of vascular issues, but he refused and would get upset if I brought it up. I don’t date him anymore and I doubt he’d listen if I sent this to him now. It sucks to know so many people like him are at possible risk and won’t receive help despite all the signs and options available.

Dang, Dr. Mike did such a phenomenal job meditating her answers. He makes sure anything she says that isn't imminently obvious to extract "the bottom line" from is restarted by him and reconfirmed by her agreement/approval. As a scientist, it's very important to learn how to structure your language to be precise and avoid misinterpretation or misinformation (especially these days). I felt she left a lot for Doctor Mike to restate for her to avoid misinforming the audience.

The issue with testing for testosterone, the "normal range" is 300-1200. The range is extreme. This range doesn't even look at age. So 300 for a 20 year old and 300 for a 65 year old are both considered normal.

Yes because age changes the number, not the sensitivity. Meaning the range is pretty accurate for all ages. A 20 year old at 220 and an 80 year old at 220 are both hypogonadal. However what's more important is symptomology in general. You might have one guy (regardless of age) at 290 that feels fine and has no symptoms because his sensitivity is higher than normal. You can also have guys at 350, who are having all the symptoms because thier receptor density or sensitivity are abnormally low. That's part of why the range is so broad to begin with, since a lot of guys with "low normal" are usually highly sensitive, and guys with "high normal" levels are less sensitive, since guys with 400 and 900 are basically indiscernible from each other.

I don't see how you're supposed to build data on this and determine what is actually normal, when doctors are refusing to do testosterone tests and no-one is gathering the data on a population level and your doctor hasn't got your previous checks in your medical records so they can say, 'Look, your T was 500 at 19, 485 at 30, 470 at 45 and this reduction isn't due to a hormonal problem, but because this chart matches up perfectly with your muscle mass loss which I also have the data for.' I really don't think doctors get big data or what it's for yet. There's a company that's edging in on using heart rate to accurately pin down your blood glucose which would democratise continuous blood glucose monitors since it'll be a one time purchase. They can only do that with huge amounts of data, which Dr Mike has said before he's opposed to gathering because the general public can't interpret it. Well... neither can scientists if they aren't allowed to gather it.

@@taylorhillard4868 usually people get ordered a hormone panel through their primary care physician and they ask for symptomology and interpret current T levels in the context of patient history. If T is borderline low, there are lifestyle interventions that can nearly double the testosterone, especially in younger men. Having more sensitive and specific tests and adjusting the ranges for age might be a good thing, but the problem that ‘some people who would benefit from TRT are being missed because of these rigid and outdated reference values’ is just not existent, because even if the T is low - lifestyle first, medications later. And not that many people maximize their T FOR SURE because majority of them are (a third of the male population-and that’s a conservative estimate) sedentary and don’t sleep enough. Drugs are cool but they don’t cure the underlying issue of WHY the T was relatively low. Sorry if it sounds like i made a couple of strawmen here, my point wasn’t really orthogonal to yours but kinda adjacent 😅

Regarding hormon replacement, you should get your most obvious checkpoints in line: Exercise regularly, sleep well, eat healthy, relax. As you get older, you can get your stats checked anyway.

Thanks for doing this episode! It responded to a lot of curiosities I'm having about how to think about my health and have productive conversations with my PCP as I enter my 40s. Regarding your curiosity around the fear of herpes, the way sex ed was taught when I was in middle and high school (in the late 1990s in the state of Virginia), STIs were essentially discussed in two buckets. In the first bucket were the STIs presented as having awful symptoms but were curable. The second bucket were the STIs that were presented as infections you had to deal with for the rest of your life: HIV/AIDS, HPV, and herpes. And when herpes is lumped in with AIDS (especially in the 90s), that particular STI has a high fear factor. It was an approach that created a lot of anxiety around sex and sexuality--unless you were in a cis het relationship with one human and were focused on procreation, in which case sex was AOK.

I had an accident in bed a few years ago. Ended up pinching a nerve in the spot where my leg and groin meet and also jamming my pelvic floor. Hoped it would go away and it slowly got better (I couldn't have sex/masturbate or stand for long periods) but it was slow and after a few months I tried to get help. Went to the Urology group and they told me it was in my head. Second urologist did a ultrasound and found nothing and didnt seem to know what to do. I ended up talking to that urologist about PT and it worked! Was able to stand for longer and be sexually active.

Here my story with testosterone as someone from europe, where the doctors are REALLY against TRT: i had t-levels of 280ng/dl as a 25yo, which is in the „normal“ range here (240-920ng/dl at the time i was tested the first time). I had all the symptoms of low T, but that didnt matter because i was barely normal, for a 90yo! With lifestyle changes, 8.5-10h of sleep every nights (i really slept that much, had 10-12h of time in bed EVERY day), better food, etc, i managed 320ng/dl on second test, 360ng on third test, 340ng on 4th test. I felt miserable. In the end, turned out i had a brain tumor which prevented me from getting over 30min of combined REM and deep sleep. Now first test 6months after tumor removal i had 540ng/dl and i feel like a different human. I wonder how much was caused by the testosterone and how much from the tumor itself.

When I was a kid I was vomiting daily due to stress. Family took me to a "doctor" and they gave me a placebo and told me I had to sit after I took it. And drink lots of water, and sit still, and eat after taking the pill. And the drugs worked! The placebo is what it took to convince me to make a lifestyle choice. Maybe the tanning bed, and BS pills are what it takes to also make the lifestyle change?
