Widower seeking accountability after home birth tragedy

A Safford, AZ man shared his story of loss with ABC15 after his wife and son died in childbirth. The state health department is now trying to revoke the license of the midwife who attended their homebirth. Sarah Kankiewicz is scheduled for an informal settlement conference with Arizona Department of Health Services representatives on Wednesday. She was the midwife attending the home birth of Jordan Terry, 28, an emergency room nurse from Safford. Jordan was expecting her third child and her due date was on December 2023.

@abfranchi What does this have to do with anti-vax? Also, I’m a nurse and almost no nurse I know is anti-vax. A very few refused the new COVID vaccine initially when it was mandated for healthcare workers and quit, or were fired. Turns out they had legitimate reasons to be concerns. Did you know 600,000 people worldwide developed Bell’s palsy within 30 days after the vaccine? I know someone like that and she now has a permanently paralyzed face. What you don’t realize is that we’ve been getting vaccinated for years with other vaccines and that we have legitimate reasons for having concerns about this one. I’m so thankful it’s not mandated for my job anymore, because the last booster caused memory issues for me. Having said that, this nurse should have known better and the midwife should have refused to assist and told her to go to the hospital. They’re both at fault.

She already had 2 high risk births in the hospital and the children and her survived. Common sense would have been to give birth in the hospital again after two c-sections. Midwives never went to medical school, are not doctors and are not trained for high risk births.

Registered Nurse midwives are highly trained. Lay midwives are trained with experience but not formally medically trained. A registered nurse midwife usually works with providers and hospitals, and has a back up plan. If I were having a baby after to c-sections I would get an all clear from an Obstetrician before I attempted VBAC and I would NOT do it at home. That is a big risk and a midwife redflag. She should have known better.

You can have a vb after a c-section. It can be more risky or complicated for some women but there have also been many successful vbacs. Everyone needs to know their own risks, their medical history and make informed decisions.

I ruptured after my second c section, with my third baby. Thank God I was already in the hospital, I had a feeling I needed to go despite not being in labor. I ruptured within an hour of arriving. It was real close, but we both made it. I thank God every day for whatever made me go to the ER and thank the doctors who took me straight to the OR.

@@20PINKluvr they patched me up but warned me not to get pregnant again. Hubby was going to get snipped but the month before his appt, we had a condom split and boom…our 4th was on his way (a week before ovulation mind you, he was a real surprise). Needless to say, I went to a high risk OBGYN who did frequent ultrasounds and I was on bed rest the last 6 months. My OB took him a month early and while he did have a short stay in the NICU, he was healthy and fine. Since I was already open, I made sure they tied my tubes- cut, fried, stitched, clamped. The pregnancy was a breeze but we all worried, my husband most of all. He still feels guilty and that was 20 years ago. But short answer is, yes, it’s possible to have a successful pregnancy following a rupture but not recommended, especially if you can’t stay off your feet. But I love my little surprise guy so much, I can’t imagine life any other way. On a side note, I was diagnosed with Elhers Danlos after his birth and found out many women with this condition are at high risk of rupture, especially if they’ve had a previous C-section but it can happen spontaneously even without one.

Considering the US has an abnormally high maternal mortality rate you're not really guaranteed quality healthcare anywhere here, even in a hospital. Especially in a red state. TX is one of the worst states to give birth in. Women's healthcare in the US is a disgrace. It's an embarrassment how badly we treat female patients. Especially patients who are POC.

Most women choose home birth because they have previously experienced a traumatic birth at the hospital. It's not just about having some hippy dippy experience, it's about not experiencing coercion, freedom of movement, avoiding interventions that cause issues, etc. Many hospital interventions are unnecessary and dangerous. The problem is that this midwife was negligent. Women do not normally die from home births.

Therefore you don't get proper care even in hospital. Many women have lost their life because of nurses not checking on a patient properly after a c-section. @DolceAnz because the uterus has been cut to get the baby out once....the c section cuts through layers of abdominal muscle and the uterus to retrieve the baby. Having your uterus cut can increase your chances of uterine rupture in a subsequent birth, which is a medical emergency and can cause death if not treated immediately. Often women who have had a c section are advised to not try a vaginal delivery again bc of this risk.

I don't know what her complications on her previous births were, but I was warned on my first child not to try going through labor--after a bout of Braxton-Hicks, they hospitalized me and took the baby that night because the contractions had gotten more serious, plus he was full-term so there was no reason to put it off. The first pregnancy was just complicated enough that I didn't even consider not having a c-section with my second, would have been horrified of trying it at home. I feel really bad for the husband, but that was sooooo risky.

Right? This makes absolutely no sense. She had two very high risk deliveries that occurred in the hospital, so…..3rd time’s the charm? Roll the dice on that tiebreaker? So not just one, but two people with medical backgrounds looked at the previous dangerous deliveries and said ‘chyup let’s do this!’ Yes, a scar represents the stronger part of any injury, however, the surrounding tissue is that much more susceptible to rupture. Along with so many countless complications that a midwife is just incredibly unprepared for and this is just a recipe for disaster and tragedy. I believe the midwife is just a complete fraud who should absolutely lose her license, but where’s the due diligence on this ER nurse’s part? There’s absolutely no chance she wasn’t intelligent enough to know to check the credentials and rating on this person, but also to know enough that having *A HOME DELIVERY UNDER THE CHRISTMAS TREE AND STOCKINGS* was just grossly negligent and terribly irresponsible and dangerous on her part. I don’t care what this midwife promised, she was educated in the medical field and knew enough physicians that would have advised her on the correct steps to take, even if she chose to ignore them.

It really surprises me how many people don't realize how dangerous giving birth is just in general even at a hospital. So, I've never understood why anyone would take those chances and choose to have home births. There's so many things that can go wrong in an instant without professional doctors or nurses there to help. Just imagine how long it takes EMS just to arrive. The husband said that the midwife told him everything he wanted to hear. That’s not what medical people are supposed to do. They’re supposed to tell you how it is, not what you want to hear. Homebirth is not scary. If she had high risk pregnancies, I’m sure at some point during her prenatal care, she was informed of the risks of her home birthing plan. My mom was a labor and delivery nurse, one of her coworkers decided to have a VBAC , my mother ( a very experienced LD nurse tried to tell her how risky it was, her coworker insisted on doing it anyway, her uterus ruptured, they lost the baby, had to do an emergency hysterectomy and the mom almost died as well, I am a nurse, I’ve had 2 C-Sections, I would rather be alive and have my 2 sons than try to have a VBAC ( and she had it at home!) even if it had not been a VBAC, there is a reason women started having babies in a hospital, so sad for this family.

@@cynthiamacias2611 well one of my grandmothers lost 3 out of her 4 home birthed babies ( my mother was the only one who survived) my other grandmother lost 2 of her home birthed babies ( that was how it was done back then) there is a reason women started having babies in a hospital, I worked as a nurse for 43 years, my 93 year old mother was a labor and delivery nurse for decades she and I both agreed we have never seen a c-section that wasn’t needed, most people who say they do too many c-sections don’t actually know how it is first hand, just what they read somewhere online…. And this lady OBVIOUSLY needed one, she and her baby died, It would have been better for her to have had one even if she thought it might not have been necessary than for her and her baby to both die, leaving her husband a widower and her other 2 children without a mother…

Yer I think both parents where psychologically compromised. Maybe ptsd from their passed high risk pregnancy. All the trauma and suffering in the hospital birth probably influenced their decided to do this as an alternative. The mom/nurse might have had a bit of an ego and thought she and the midwife had enough medival knowledge and experience to handle any complications. Even when things took a turn south they waited to long to call for the ambulance the mom didnt want to "give up" and go to the hospital. I hope the dad makes it through this and be there for rhe child/kids he still has. I fail to see the validity of a lawsuit here. They made the choice to move forward with this midwife, and anyone wanting to do a VBAC is very high risk and should not do a home birth. Sounds like a lack of research on the midwife (you can't just go off someone's answers). This could have been totally prevented. Home births should not be permitted for high risk women, for these exact reasons. A repeat C-section is a small price to pay for getting to stay alive and have a healthy, living baby.

Oh ffs she was a RN she knew what the risks were and that no physician would ever advise a VBAC except under extreme monitoring and caution. I always wanted a homebirth until I did my L&D rotation in nursing school and saw two births that did not go well. Yea I really hate to victim blame, but she had the education to know what an extreme risk she was taking. The midwife should also be held accountable for taking on a homebirth client with her history. In all honesty the only party without blame here IMO is the father- he may not have known the true risk his wife was undertaking if he has no medical background. Although I don’t agree with it, most husbands/ non-birthing partners only learn about birth from what their partners tell them. I’m 20 weeks and planning a water birth at home. I’m not high risk and I have a midwife, doula, and medical assistants who will be at my birth. It’s possible to have a home birth after c section births. She needed to research and find a better midwife. I do have to say I most likely would have been better if she was in a hospital and had a midwife there. Midwife’s also debate on taking you on if you’re high risk. Plans or not I know that if something goes wrong the people around me will pull the plug and take me and my child to a hospital.

Mine baby flipped also and once they got in there by emergency c section she was not moving and had the umbilical cord tightly around her neck. It was horrible when they took her out and no crying for what seemed like an eternity. I praise the one nurse who woke my limp baby up (my sister saw everything) and luckily baby was fine, we survived because we were in the hospital...no complications. She's now a 31 yr old beauty!!❤

I experienced a uterine rupture 1 day before my scheduled c-section. I had one contraction that triggered the rupture. My daughter and I definitely almost died. The pain was like being tortured alive and I could t even lift my voice to speak, only whispering. This happened because I was bleeding eternally lost so much blood and on the brink of death.. I, too, wanted a VBAC but no doctor would entertain it with 2 previous c-sections. I was mad about that, but I’m grateful for ambulance folks who rushed me to the hospital. I fed for him. My sister finally had a successful pregnancy after numerous miscarriages. She went into labor and during the delivery of my niece, my sister had a heart attack due to a pregnancy condition that went undiagnosed. They did not have a defibrillator near and I lost my big sister and niece that day in the hospital. Even after her death, the hospital (southern regional in Atlanta) couldn’t get anything right. They didn’t release her body for weeks before finally my mom got ahold of someone high enough up that apologized and released her and my niece. Then, after the funeral home picked her up they realized that my niece wasn’t with her and had to go back to get Augustina. We wanted them buried together but because of the delay of releasing her and her baby they had to be cremated. The whole ordeal was traumatic.

I knew a expectant mother that wanted vbac - I told her husband and dad to call 911 if they were concerned about anything - the mother was taken to hospital and at that point begged for epidural but it was almost time for baby to be born . It all worked out - they were fine . But what a day ! She was always a little disturbed that her husband and dad called 911 but also said how grateful she was . She didn’t have the c section as baby was born quickly once she got to hospital but so risky ! Another relative had her baby in hospital and started hemorrhaging on the delivery table after her baby was delivered and the docs saved her of course . I don’t know the details - but wow - this case sounds so risky - midwife living so far and the couple just remained at the house and didn’t call the ambulance?

They started doing it years later but I would never risk a home birth after a C-section with complications and not in the hospital but I have knowledge and experience now that I didn't when I was young nurse. I have heard of this happening multiple times with death to the mother, child, or both. I have had all of my children in a hospital. My first birth was "normal." My second birth, however, was very traumatic and happened in a matter of seconds. I will always be grateful that i was surrounded by hospital personnel and all equipment, surgical needs, etc.

I asked my high risk dr if I could try for a VBAC. He got about 2 inches from my face and said, "Do you want to die?" He went on to say he was there so women in my condition don't have to take unnecessary risks. At the time I thought it was a little extreme, and the birth was awful (different delivery doctor) but I'm here to write this and so is my child. It's so important to speak with doctors. Midwives are important to many in the birthing community but not more so than experienced doctors. So sorry for this tragic loss, but the midwife may not have seen this coming. A doctor more likely would have looked harder at all the risks involved.

Dont mistaken me, she's still wrong, but stuff like her previous (and several) investigations and FIVE civil fines and other things attached to her licensing are public so had they done their due diligence and thoroughly searched, it wouild've added an extra layer of safety/prevention. When he stated "she gave me all the right answers," sounds like they googled someone, asked a few basic questions, she probably had a reasonably low price, told them whatever sounded good and they just said okay. I really wanted a v-bac, but was denied by my doctor and couldn't find a different one because of my insurance. I would've done it in the hospital, but he absolutely refused. (Dumb I know, but I was really in denial about being in real labor, was sure it was Braxton-Hicks because my water didn't break immediately like the first time) Thank God he denied it, because during the second c-section, right as they got to my uterus, it completely ripped open before the scalpel ever touched it and I needed wayyy more blood than normal. We live about 25 minutes from the hospital we needed to be at, or any hospital for that matter. I'm sorry, but a healthy mom and baby is more important than any other desire. A home v-bac SHOULD NOT BE LEGAL IN US.

This happened to a friend of mine, attempt VBAC, in a hospital setting, just feet from an operating room. Hers was the only life that could be saved after the rupture. 😞

I heard of Hosipital Staff abusing patients but okay. (Not that all of them are bad, but still) There is not a 100 percent chance that you will be safe just because you are at a hospital.

There should be a bit of personal accountability here. They knew there was a chance of rupture of the uterus during contractions and birth due to scar tissue, besides previous birth complications. common sense should have told them it was too risky to try at home, it's even very risky in hospital. But she was more concerned with the experience she wanted than worried about risks.

At one time there were no options. Elders gave birth at home because professional services were unavailable and infant mortality was higher. Always trying to improve.

I'm so done with the medical incompetence in Arizona...I have lived here for over 10 years and the stories I have heard.....pathetic...is there some kind of medical school for dummies that these people all go to....or maybe it's a situation where the doctors who graduate bottom of their class flock here for some reason...we need to raise our standards...the fact that this woman didn't have her license revoked sooner and even now it's being debated?...utterly ridiculous...she should be on trial for reckless homicide...medical incompetence does NOT get a free pass...

I'm from California and many of the people I knew growing up did flock to Arizona. One entire family aunts uncles and all moved there. They weren't the smartest of people... and I do feel for the locals of whatever community they moved into.

I have had 3 c sections. I was told after 19 years and 2 c sections they allowed a "natural" birth when I got pregnant with my 3rd daughter. It was not something that was allowed back when I had my second daughter I was told I had to do a c section the risk were too high. So when I was told I could try for a natural birth, I opted out. I knew it was dangerous and being 8 years sober, I knew I would go through with another c section with no pain medication because it was safer for me and my baby. I don't get these home births. Please stop ladies. Its too dangerous and having a doctor and staff knowing you and your past health concerns even if none, a hospital is the safest way to go. Stop playing with you and your babies lives. It is not worth the risk.

I give birth at home for all my babies in Arizona. Home birth is very normal here. But i am low risk. A VBAC is very high risk. I have several friends who have had successful home birth VBAC, however i would give birth in a birth center attached to a hospital with doctors if i had a VBAC. The risks are so high. I feel bad for the family. 😢💔

I also gave birth at home. I was high risk only due to my age. My first two were birthed at birthing centers. I would always recommend qualified midwives. But same, I was monitored as all of the midwives monitor pregnancies in this area do. I did not have issues like preeclampsia, high blood pressure, previous c-sections, diabetes, etc.
