In the game you choose a group to be a part of and represent your group by challenging opponents to become #1.

make it so instead of coming back to life after a bit, they die after a bit, so they have to either use an item to come back to life, or their friends have to make it to a hospital in time. also there could be a lower level part of town with run down hospitals that give negative stat effects when your revived. but dying fully makes you lose your items like in da hood so ppl actually try to get revived instead of dying off and respawning with their stuff
