Dr Marhur: Every question you asked Dr Jimenez-Shahed and every answer by Dr Jimenez-Shahed is intensely relevant to the issues encountered as I have started the DBS process. I have undergone Cognitive/Psych testing and MRI imaging so that brain atlas mapping information can be overlaid with the MRI imaging of my own brain. The brain surgery plan is to install one lead and install the second lead at a second surgery so that the second surgery will benefit from issues learned from the first surgery.

Thanks for your comment. You raise an interesting reason to consider multiple surgeries in the process of starting DBS treatment. Of course, this decision will involve the preference of your care team and your insurance coverage, but there can be advantages to this approach, and these are worth considering. I had the Boston Scientific DBS device implanted 3 months ago, at the recommendation of my movement disorders specialist at Johns Hopkins in Baltimore. Best decision I’ve ever made.

Has DBS ever been studied in patients with Cerebral Palsy?

The Foundation's staff, who monitor this channel, are not familiar with research regarding Cerebral Palsy, but it appears that DBS has been researched as a treatment for dystonia associated with Cerebral Palsy: thejns.org/view/journals/j-neurosurg/139/3/article-p605.xml.

I am tremor dominate on the right hand ... it hasn't spread to the left hand much 5 years after diagnosis ... it may be masked by meds... use around 13 or 14 Prolopa capsules over 24 hr period... would I get both sides done for DBS ...Ontario

We don't offer individual medical advice, but many people have DBS leads placed in both sides of the brain to help treat symptoms. The decision about the best path for you would be part of the DBS screening process with your care team. Tremor is, of course, not the only symptom with which DBS can help, so that is one element to consider. One reason a single-sided surgery may be preferable for those with unilateral symptoms is that it would decrease the length of surgery and the number of incisions, but again, your care team would have to evaluate the likely benefits and the possible risks inherent to your individual circumstances.
