OMG I WISH the medical community would get with the program. I have been trying and trying for 4 years to figure out why I have inflammation and dyspareunia. Oh I WISH the OB/GYNs KNEW to test for bacterial vaginosis. I went through FOUR rounds of testing and NO ONE even mentioned it, nor did they mention ureaplasma/mycoplasma testing. I had to find out MYSELF. Unfortunately, the tests came back negative. That's when I stumbled across AV. Now I'm having trouble getting one of the doctor-gods to order the test.

I had had a vaginal hysterectomy with prolapse repair of bladder and colon back in July of this year. The surgeon removed my cervix as well so I have a vaginal cuff. At my 6 week post op appt he examined me and said all was well. Roughly 2 weeks after my post op appointment I begin to smell like fish. I was diagnosed with BV. I was prescribed metro gel. I completed the course. But then it came right back after 3 days. So I phoned the doctors office and assured me another round would do the trick. A few days after starting the second round, I noticed a rash all over me. I wasn’t able to do the last dose of the gel in fear my allergic reaction would worsen. Again around day 3 (after I stopped) the smell came back. Desperate, I searched online and came across boric acid suppositories. I began using those for 21 days straight. Again, after around day 3 the smell came back. So I phoned the doctors office. They did a exam, and prescribed clyndomyocin. I completed that round and again it came back. Called the doctors again, she then said to use metro gel for 2 days per week for 16 weeks as a “maintenance” protocol. She paired that with boric acid to be taken nightly. I started that and then began to have burning and big chunks of cottage cheese looking discharge. It was awful. So I had to stop the metro gel protocol. I then decided to leave it alone all together but it got worse. I had cramping pain along with lower back pain and nausea so I ended up going to the ER as I can’t see my new OBGYN until February 24th because we moved 13 hours away. They did a pelvic ultrasound along with a vaginal ultrasound and Pap smear and exam while in the ER. He did check for BV and all std’s. All looked well however he said I have BV and prescribed me clyndomyocine again. I’m at a complete loss. Prior to my hysterectomy I’ve had one case of BV and one case of a yeast infection. I haven’t started the prescription because I’m afraid it will cause C-diff. Can you make any suggestions? I’m truly at a complete loss. I haven’t been the same since my surgery. I just turned 44 as well. Not sure if hormones are contributing to this. Thank you doc! I just found you on KZbin. Hope to see your channel grow.

Thank you for you comments and sorry to hear you've had a rough time. BV can be troubling. Sometimes the best treatment for BV is just oral probiotics (stronger the tablet the better) and natural live unflavoured yogurt. Follow the 16 tips to prevent BV and all together this should help control BV. Should hopefully work quite quickly in 5 days or so. If not then use the antibiotics then use the probiotics etc. Let me know how you get on. Kind regards Dr Bret Palmer

I was diagnosed with aerobic vaginitis last year.i catch it repeatedly even after using clindamycin cream for two weeks sometimes 4 weeks at a time

I assume you have discussed prevention methods with your doc. Also advise people to watch 16 tips to prevent BV video which can help change the environment of the vagina, which is what is needed to treat AV. Let me know how you get on. Kind regards Dr Bret Palmer

Cream is best once a night for 7 nights.... assuming no allergies.

My question is if they do regular testing for bv? Even if you have aerobic vaginitis, would the regular bv test come back positive anyway for gardnerella?

Hi Erikka, good question, thank you. Best way to test for BV is to go to a sexual health clinic, be formally examined and a sample taken and looked under a microscope. BV is then diagnosed on the appearance of the findings. This is usually the best way. Some send away tests may look for certain bacteria and then assume it is BV. Hope this helps. @@DrBretPalmer ok I have one more question! Do you believe that BV can live in rayon fabrics such as bathing suit bottoms? Like if you contaminated it, could you reinfect yourself?

No, as they could be removed by washing. However man-made fabrics don't allow the vagina to breath.... you need fresh air around thd vagina. So breathable fabrics like cotton are best. Hope this is helpful Dr Bret Palmer

Hi Doctor, I was diagnosed with desquamative inflammatory vaginitis. Is this the same or different? I’ve been struggling so much and have an appointment with a specialist shortly. Any advice for helping to balance things back out? I want to get back to a normal life again.

Hi there Michelle. That can be difficult to treat as well as diagnose. But at least you know its DIV. Treatment tends to be local vaginal clindamycin usually 1st and/or vaginal corticosteroids 2nd. Depending on what's seen down the microscope and then replacement with probiotics.... see my 16 tips to prevent BV. Ideally you are seeing a sexual health doc, or a gyne doc with a special interest in sexual health. Please let me know how you get on.

@@DrBretPalmer Hi Doctor, thank you for your response. I did do vaginal clindamycin (3 weeks) and hydrocortisone (2 weeks) it improved but never fully went away and I still have burning and dyspareunia. Although my ph has never gone above 4.5 is bizarre to me. Specialist was a bit of a bust, his focus was more pelvic prolapse and urinary issues. I was diagnosed by somebody at Planned Parenthood actually in January, after seeing five doctors! Yay PP 🙌 I reached out and have an appointment coming up with a new Gyno who said she treats DIV - she seems interested in sexual health. I have been taking renew life probiotics 90 million daily, a lot of natural herbs i.e. turmeric,l lysine, etc. On an anti inflammatory diet. Started to also see a certified Acupuncturist and TCM herbs. I also did an Evvy test recently so hoping that gives me a clearer picture. How do you feel about Evvy tests? I was also prescribed vaginal estrogen by specialist in a small amount but to be honest I’m a little scared of it and haven’t started to use. Do you feel vaginal estrogen is helpful for restoring tissue? After the medication merry go round in the six months I really wanna make sure I’m taking the right kind of medicines!

Gynos typically diagnose DIV when they don’t know what the cause is. Don’t settle for it. Take a Juno, Evvy, or microgen dx test to find out the cause. And if it is AV the antibiotic you need highly depends on what bacteria it is. Clindamycin doesn’t treat e. Faecalis but augmentin does for example.

What brand of probiotic do you like best? I had Cervical Cryotherapy a year ago and have been dealing with copious amount of yellow discharge and sore vagina ever since. Every single test my ob has ran comes back negative. Wondering if I need to specifically ask for Aerobic vaginitis testing? I've also had follow up pap and no longer have HPV and no more Cin 2/3.

Hi Misty, Thank you for your question. Does the discharge small? E.g. a fishy type smell? Also have you tested for Chlamydia, gonorrhoea and mycoplasma genitalium? If all clear then any probiotic can help, the stronger the better, brand not so important. Some people eat fresh unpasteurised sauerkraut which also helps. I would also advise to watch the videos on genital hygiene and 16 tips to prevent BV. Let me know how it all goes. Kind regards Dr Bret Palmer

@@DrBretPalmer It doesn't smell, I have been tested for Chlamydia, gonorrhea and mycoplasma all came back negative. I only wash down there with warm water and a wash rag. I never had this issue until after cryotherapy.

Thank you for that. Whichever probiotics you use go for the one with the highest number of cultures. In the UK there are a couple of makes which have over 100 million cultures per tablet. Good luck and let us know how it goes. Dr Bret Palmer

Hi I’ve just found you and I’m in the Uk. I’ve been on a very long journey with Av, I have Alloscardovia omnicolens bacteria. Do you know much about this? I’ve been trying to repopulate without taking antibiotics but it’s not working for me. Any help would be much appreciated

Also if a lab has identified it they could culture it to grow and see what antibiotics it is sensitive to. But if no symptoms then you could ask why did they test for it as not an STI.

Hi doctor, I did vaginal swab culture and sensitivity and first got entrococcus and klebsiella species, then went on Cefuroxime and a few weeks later Amoxyclav, did another test and found "gram positive cocci+", but my doctor refuses to diagnose it as AV since "he never heard of it", do you think it's the entrococcus? Since I didn't get any antibiotics for it..

I have been having an abnormal discharge and experience soreness in my vagina since 5 months now but doctors refuse to acknowledge the Entrococcus saying "it will go away on its own", what do you advice please? My country has no sexual health clinic..

Hi there, Thank you for your question, a very difficult question. Without looking under a microscope it is difficult to say. Entrococcus is difficult to treat as they are usually resistant to many types of antibiotic. Some times taking probiotics can help reduce the symptoms. If you take antibiotics, then you will need a sensitivity report to see which antibiotic would be most effective. To answer your question, then yes, entrococcus faecalis can cause/contribute to AV. @keshneeg972 L. rhamonus may help reduce entrococcus if my memory serves me right.... usually mixed in with other types.

This was very informative doctor! I've been struggling with ecoli AV since the past 3 months. However augmentin hasn't given any relief. Ive been applying candid (clotrimazole) since the past few weeks but to no avail. Vaginal probiotic suppositories aren't available in my country. So I've been taking oral probiotics containing several lactobacillus strains. Will oral probiotics work ( would they take a long time if so otherwise what should i do. Most of the antibiotics are resistant. Thank you!!

Always worth a try. Could try natural, live, unflavoured, bio yoghurt, and place some directly into the vagina, sometimes works.

I would do a Juno, microgen dx or Evvy test. Then do the free consultation. Then find a doctor to prescribe what was advised. Because the tests doctors run aren’t sensitive enough and don’t provide the full picture of what’s going on.

@@juliezimmerman1435 already did all those tests. My ureaplasma levels were literally super miniscule both times i did them. Either way I lesrned my issue is muscular-tight pelvic floor mimicking uti symptoms

@juliezimmerman1435 depends on what country you are on. Most postal tests just test for random bacteria humans carry anyway. See a doctor, get a consultation first and then test appropriately.
