This plane crash was pretty much forgotten by this whole world or the news about it weren’t really widespread. I remember reading about this plane crash in Wikipedia then I watched the FDR animation of this in 2018. I thought “I’m one of the few people in this world who knows about this plane crash.” It’s amazing that Air Crash Investigation made an episode on this plane crash. Perhaps, soon after the release of this episode, this accident would be well-known again. We’ll never forget about this accident. Rest in peace to those who died in there. It’s really tragic to think that most of the passengers were flying to Madang to attend their children’s university graduation ceremony that weekend.

Cause: Captain unintentionally reducing engine power beyond idle mid-flight causing dual engine overspeed and loss of power. The crew had to make an emergency ditching but the river was to rocky. 28 of the 32 people on board died in the crash. Rest in Peace

This happened because the pilots accidentally set both engines into beta range mode (thrust reverse mode) while in the air. The pilots were meant to set both engines to an idle mode to slow down their over-speeding plane, but they accidentally moved the throttle all the way back to beta range mode. The engine thurst reverse was not what brought down the plane but it caused both engines to exceed their rotation speed limit, therefore causing them to damage themselves and eventually fail. It’s sad to think that the crash landing would’ve been less fatal if the pilots didn’t shut down the engine that could still help the hydraulic and electrical systems of the plane to work. Only 4 out of 32 people survived (2 pilots, 1 passenger, 1 flight attendant).

On 13 October 2011, Airlines PNG Flight 1600, a Dash 8 regional aircraft on a domestic flight from Lae to Madang, Papua New Guinea, crash-landed in a forested area near the mouth of the Gogol River, after losing all engine power. Only four of the 32 people on board survived. [1](2] It was the deadliest plane crash in the history of Papa New Guinea. 3] The subsequent investigation found that the flight crew had inadvertently retarded the power levers below the lowest position allowable in flight (known as flight idle), causing both propellers to overspeed and leading to a complete loss of engine power. A 'beta lockout' mechanism that would have prevented the overspeed even in case of erroneous power lever setting was available but not installed on the accident aircraft. Installation of such mechanism became subsequently mandatory on all DHC-8 aircraft

Pilots screwed up. Both were distracted and allowed the props to overspeed while on descend. Captain panicked and slammed the power levels passed idle without identifying the problem. Further instead of ditching in the sea like they said they would do the captain attempted a controlled landing, No procedures/checklist were reviewed. Result - landing gears were not lowered, flaps not set for controlled landing (which would have further reduced speed). Further fuel was not dumped and all who died were burnt beyond recognition. My sister was to booked on the flight to go see her son graduate but missed the flight. RIP olgeta

Apparently they could have squeezed 6 minutes of flight time out.. I don't understand how they were 16 miles away from Madang airport IF they had to turn around to crash at the mouth of Gogol river... Which is less than 10 miles away from the airport according to google maps... which means that if they followed procedure and got the extra 6 minutes, at 120Kts would have given them approx 13 miles ... then they could have, in theory, made the airport?... 16 miles out would have put them closer to Mindjim river?.. Dont get it.
