Bikini Bottom may not have a dedicated pet hospital, but it may be linked up with their main hospital. Even if it doesn't, Dr. Gill Gilliam proved in "Once Bitten" (the episode where Gary starts biting people and Patrick spreads mass hysteria about Gary's bite turning people into essentially zombies) that he is knowledgeable on veterinarian care because he correctly knows Gary has "Grouchy Snailitis" and knows Gary's bite doesn't turn people into zombies. He also removes Gary's splinter like a pro, so idk I think he knows a think or two about pets even if there may not be a pet hospital, so it makes sense for him to be at Squidward's about a dying pet if someone called.

Considering his name is “Fancyson” what if it’s less about him giving up his dreams to be rich but it’s possible he’s inherited his fortunes. What if he picked on Squidward because even though he puts on an air of superiority he finds something about Squidward interesting enough to badger and harass him, going at great lengths to waste his inherited fortune. Maybe Squilliam is some passionless person, always having been given anything he ever wanted his entire life, yet he has no talents of his own. Squidward, who isn’t rich but has a home by just having his single fast food job. He isn’t the greatest at the clarinet but he never gives up. Fancyson can’t tread him down no matter how he tries by flexing the only thing that makes him stand out, his inherited money. Squidward WORKED for what he has and though it may not be a mass fortune, he still managed what Squilliam may not be able to. He feels like a failure living a lie and continues to keep up this one sided rivalry with Squidward.

Said ward IS a good artist, the whole point of his character is that his hubris and pride gets in the way. Because when he doesn’t care about his art, he does extremely well. So, for the Pet Hospital thing, Doctor Fish is also a real veterinarian. He shows his certification in "Once Bitten," which proves he's even an expert in Snails. Also in the episode "I was a Teenage Gary" the same fish even shows up in the Vetrinarian vehicle, which also doubles as a snail car, and gives SpongeBob the medication for Gary. And actually for the first time in 2016 we actually see the Pet Hospital or as it should be called "Vet" in the episode "Pineapple Invasion", with the same Doctor/Vet Fish. Also see the Pet Hospital in the Episode called "Say Aw!" from Season 13.

I'm pretty sure the "Squidward Torture" episodes were an inside joke geared towards the original character artist who came up with the look for the show, Michael Todd White. Todd White being the lead character artist/background designer from 1999 to 2000, abandoning the show in favor of seeking a career in painting. (Worth mentioning that Todd White plays Clarinet on top of being a painter. He also knows Martial Arts, I know Squidward doesn't do Karate. But maybe the idea for SpongeBob's Karate came from Todd White). So whenever they make fun of or abuse Squidward, they're likely taking shots at Todd White for leaving the SpongeBob gravy Train early.

Squilliam wanted to show Squidward that squidward can't live without him and that his life is miserable alone. Because Squilliam knew he can't live without squidward either. But squidward's pride and ego made him choose the hard way to become a famous musician and artist. He even started to be a cashier for (probably) minimum wage at the krusty krab to get financially independant from Squilliam. Squidward underestimated his idea gravely because he thought this would be easy. This was a big mistake that made him lose his beautiful long hair. The only thing that kept his mental from being the cynical being that he now is.
