Corrective jaw surgery, or orthognathic surgery, is a group of procedures performed to correct dentofacial irregularities, most commonly manifested as misalignments of the jaws. These deformities not only cause malocclusion or bad bite, but also create problems in the temporomandibular joint - the TMJ- and the airway, resulting in difficulties chewing, swallowing, speaking and breathing. While the surgery is performed to correct functional problems, patient's appearance may be dramatically improved as a result.
Open bite is a condition where the upper and lower front teeth do not touch when the mouth is closed or at rest. An open bite can lead to a number of oral health conditions including tooth wear, tooth breakage and TMJ disorders. It may also cause speech problems known as ‘lisping’ in some individuals. Open bite surgery involves removing some of the bone of the upper jaw to move it to a new position. Once the jaws are aligned, plates and screws are used to secure the bones in place.
Protruding lower jaw is corrected in a procedure called mandibular setback surgery. The tooth-bearing portion of the lower jaw is separated from its base and moved backward for proper alignment.
In a similar way, receding lower jaw, or “weak chin”, is corrected with mandibular ADVANCEMENT surgery. In this case, the tooth-bearing portion of the lower jaw is repositioned FORWARD.
Orthognathic surgeries are commonly performed in combination with orthodontic treatments and may take several years to complete.

I had this surgery done 19 years ago due to a massive underbyte. The worst part about it was recovery and getting used to a new appearance. Eventhoug my surgery thank god went good, i have to get used to my new appearance, new expressions, smile and also get used to the implants. Your appearance will change for the better. For anyone scared about the surgery: I just had this surgery (to correct my 10mm overbite) last week Wednesday and they put you to sleep and trust me when I say you can’t feel a thing. You’re very swollen afterwards and drugged up when you’re in the hospital but they make it so the pain is bearable. I am still swollen now and in pain but it’s worth it in the long run.

My 14yo son just had this done yesterday in Australia for his overbite. Braces stayed on during surgery Top jaw moved back 8mm Bottom jaw required to be moved forward 4mm to achieve best alignment. 4 wisdom teeth removed Chin bone cut and re- positioned. Bone grafts from same site. Had it all done at once in a private hospital. Total Cost in private hospital was about $28k Braces about $8k No health insurance Hardly any pain as plenty of meds in hospital and to take home. One night stay in hospital Lots of swelling, but not as bad as I expected. can only have liquid food through a straw / syringe for 6 weeks. Can only open mouth enough to fit a straw in as the braces have temporary elastics at the back to keep his jaw shut to heal. Ice packs on cheeks for a week or more to reduce swelling. No sports for 2 months Was able to talk / mumble within 24 hours. He is doing really well, a bit weak and tired but minimal pain, mainly just a lot of discomfort.

Hi, friends! So I had this surgery almost two years ago. I had what my surgeon called "the worst case of underbite he's ever seen", and an open bite too. If you follow your doctor's instructions, recovery is incredibly chill. There are painful moments, but you will probably get medication prescribed to you. Also ask your doctor about wiring your mouth shut. There are more modern techniques that don't require it (I, for example, didn't get mine wired). Having only liquid food is not that bad, unless you have soup or something haha. Biting for the first few times can be kind of painful and weird, because you jaw muscles kinda forget what they are doing. There's also a chance you might still be numb. My chin is still numb to this day, but mostly because my surgery was very aggressive, since my case was severe. Most cases heal sooner. What I do recommend is a bit unusual: Therapy. It's very, very weird to see your new face, especially when your swelling starts to go down and you look different every day. It's very uncanny, to say the least. I am still getting used to it, but I don't regret the surgery at all. I stopped snoring, I stopped choking for no reason, my posture has gotten better (surpringly), and my breathing has gotten so much better that I finally get why people like scented candles! The health benefits alone overshadow everything else.

un disclosed recovery is doing well :) it’s a little slow and just today i got the elastics that hold my jaw shut replaced with slightly weaker elastics. i don’t have any pain but in case you plan on getting this surgery, you should note that due to swelling and having a plastic tube down your throat during surgery, you might have to deal with a sore throat and maybe achy ears for a few days afterwards. but right now, 13 days after my surgery, i can tell you there is no pain and i am not taking anymore meds. i also can’t feel the metal connector at all. it’s worth mentioning that jaw surgery is different for everyone but this has been my experience so far :)

Val Salas thank you so, so much. I’m 16, and have a pretty serious underbite. Not as serious to the point I look like a bulldog, but it’s only fixable with surgery (so says my orthodontist). I’ve never been able to bite correctly my whole life, and I’m eager to get this surgery done ASAP! I was told I’d get it done once I stopped growing, which is 18. I ask how recovery went because it seems like quite a restructuring of the jaw. I just want to get this done and be able to smile. Thank you again for your reply!

It's not like you have to watch them do the surgery. If you are in the hands of a good maxillofacial surgeon who has a reputation, the probability of something going wrong is probably less than 1% in even the most extreme cases. Some of these guys are so fucking good at what they do and they make a boatload of cash per year, literally. While I would never get something like a Lefort III osteotomy, I would be fairly confident in a doctor performing a lefort I osteotomy on me. Healing time isn't that long, like 30 days before you can eat hard foods, but it can absolutey change your looks entirely. For a lot of people, a lefort osteotomy, if they're recessed facially, can elevate them considerably.

In 2006, I was 17-18 years old. I had lower jaw reconstruction sugurey to bring my jaw foward a few mm. I was innitaly soppose to have top and bottom reconstructive surgery but once in recovery room I was told they only did the bottom as they felt I didn't need the top portion done. Also during this operation the surgeon removed 3/4 fully impacted wisdom teeth. The 3rd molar on the right was left in because they needed that tooth to act as stabilizer for the screws in the jaw until the new bone grew back, then they'd remove it. My jaw was wired shut. At the time I had braces on my teeth and my bite wouldn't allign properly with out this particular operation. I was told I could "possibly" lose sensation/feeling in my lower chin lip and lower gums and teeth. However I permanently lost feeling in Lower lip, Chin, teeth & gums. After 10 years nothing has changed other than every now and then I'll get a burning or stinging or pins and needles sensation in certain areas of my chin & lip from time to time. It's annoying but I've learned to live with it. I'm not sure what happened other than the The mandibular inferior alveolar nerve got severed or stretched or pulled or damaged. Even the orthodontist at the time had no clue what happened nor did the surgeon. I was never told it could be permanent. It became a love/hate procedure while the results of my bite and facial construction looked amazing I honestly don't think it was worth losing the feeling to half of my face. Sometimes I'll get this like surge of tickling sensation through a nerve and I badly want to scratch it only to find I can't locate where the itch was coming from then on days I'll have a dull aching feeling in a nerve. Some days it will pulsate like a heart beat some ticklish some tender to the touch. Some days I feel fine some days in a ton of pain. Some days I feel the wind on my face other days I can't. Some days I scratch my chin or lip and get a rush of the ticlkes followed by a few seconds of pins and needles then back to no feeling. Sometimes I scratch I'll feel a mini orgasm in my chin and lip and the nerves will travel to my nose and other parts of my face and make that area start to itch. Since this operation I'm e extremely sensitive to cold water or things touching my chin or bottom lip. At times it's very painful times its feels nice. It's such a complicated thing to explain. I wish I had a way for my nerve to heal but after 10 going on 11 years I've come to terms I'll end up like this for the rest of my life. I wish I knew how to feel in my jaw teeth lip and chin. If any dentist or doctor or orthodontist or anyone in general is reading this and have any information they can provide to me about anything going on with me please let me know. Also what's the probability this nerve can be healed or saved if at all?

Man, your story resonates with me, as I am possibly about to be forced to live out a version of it myself. Ever since my adult teeth came in, I've had an open bite. It only worsened as I got older and into middle school due to my severely impacted wisdom teeth. I had braces from about 9 to 13 years old. This arduous period of dental treatment mostly corrected my exaggerated crowded upper teeth but left me with a retainer to wear on the honor system and a full set of wisdom teeth still driving at all of my teeth. Naturally, being the stupid short-sighted kid I was, I barely wore my retainer after having put up with the misery of metal wire and bracket braces and rubber bands for years; pain from the pressure, food getting stuck in them constantly, etc. I was free! So I thought. Where it used to be my right canine that was thrust up high, now my front teeth and left incisor are forming an orthodontic-repudiating enamel-coated rocket, being forced upwards as my wisdom teeth drive the right incisor and left canine behind them. I didn't get my wisdom teeth until one and a half years ago, and i am halfway through 26 years old. I thought the worst of my troubles would be over with my wisdom teeth removed. Wrong again. From then until now, it has been as though the removal of my wisdom teeth only yanked the anchors for my upper teeth. and they have bullied my three front teeth so high up to the point it has somewhat been affecting my ability to close my mouth with being fearful of aggravating the condition. If those teeth were tectonic plates, in the past 12 hours I've been experiencing an earth quake. The three front teeth are just floating now like an island, and I can't speak normally without feeling them move. I am afraid that I will be faced with a choice like yours soon. I've been told I'd have to do it before and shrugged it off, figuring I'd get a second opinion somewhere that was less gory sounding. This video was not comforting. I haven't been able to afford insurance or out of pocket on my own, and times aren't what they used to be for my parents' finances. I am grateful I had what I did get when times were better. Not everyone is that lucky to begin with, and a good part of my problem is my fault for not wearing the ugly and uncomfortable retainer, but I also cast a bit of blame on my orthodontist for not delegating heaps more priority on removing my wisdom teeth much earlier. I am about to pay dearly for it now. I am sorry that your procedure robbed you of your facial feeling. No one deserves that. I hope that they devise a repair for it soon. Until then, stay strong my friend. -Jake

@MrDeantheBean14
@MrDeantheBean14 5 жыл бұрын
the bones are surgically broken in a neat and clean order. after the doctors adjust the jaw to where it is suppose to be placed, they screw small titanium brackets into the two spirited pieces of bone in order for the bone to heal perfectly back together. I was told I had a 3% chance of having an infection due to the metal brackets. its very rare it occurs but can be fixed with medication. I was also given the option of having another operation to remove the brackets after the bone has been healed 100% to further reduce the infection possibility. I hope this answered your question!@@empirical_blade6926

I officially started getting stuff done to my teeth at 16. Spacer, jaw expander and so forth till we hit braces. Fast forward 5 years, I finally get my braces off and get retainers done. 6 months after my Ortho tells me he notices my teeth shifting again. My upcoming appointment is next week, but I just woke up this morning and my Mom noticed they've moved. I think I need to have this done... Crap.

I've had an expander on at age 9. Then again at 14-15. I'm almost done my second expander as of now and then I'll need a retainer to maintain the top jaw's shape. And then once I'm 18 I'll need the jaw surgery for the underbite. :( Oh and then I've heard the Dentist's want to give me braces afterwards but I think that's BS and they just want money from me. My parents had to get them out of making me get braces TWICE! WTH?!?!

I used to have an open bite (5mm) and most orthodontists had suggested extractions since I couldn't afford surgery. So I opted for a camouflage technique instead, osseous genioplasty to fix my protruding chin, 4 premolar extractions (wisdom teeth had already been extracted), lingual braces with tads for 2 years. I now have wire attachments behind my front teeth and wear a retainer for 12 hours a day and a tongue crib to better help me position my tongue. It has been worth it.

when I was suppose to get braces at 13, I went to a few different orthodontists. But one of them said they wanted to break my jaw and do this reconstructive surgery, because my jaw grows down instead of out. At 13 that sounded scary, I was still growing, and I didn't want to do it. Also because it was cost a crap ton more money. But the way it looks now like I don't have a jaw, and it's just my face connected to my neck doesn't look the best. Sometimes I think it'll be okay because I do exercises to tighten the muscles so it DOES look like I have somewhat of a jaw. But still sometimes I think about it, and wonder what it be like.
@emerald4812

It is still possible to do a reconstructive surgery after childhood! Usually they do it at around 18-19. If you have the money for it now, and still want it, I suggest you go back to a surgeon to talk about this.

Your situation is very similar to mine I was 13 when my orthodontist suggested I get corrective jaw surgery or I would have problems down the line. But at that young age it sounds so scary you really aren’t thinking too much about the future. Now that I have sleep apnea and other problems that come along with it I wonder if it would be worth the trouble to get the surgery now as an adult.

Hey! So I had a tiny underbite, misaligned jaw and long chin and had my jaw setback, chin trimmed 3 days ago. Unlike most people, because my immune system was so good, I had zero complications, side effects and pain that I’m ready to be discharged now. I was able to speak right after surgery even and my recovery’s been 3-4 times faster than normal people. So the recovery varies from person to person. I’m a little excited and nervous whether there’ll be any change in my appearance though. Looking forward to it. Glad I had it done for now. Also about diet, I’m put on a liquid diet for two weeks but my appetite didn’t go away, I haven’t been hungry or nauseous at all that I’ve been able to eat more mushy, variety of food compared to other people. The only annoying part is getting bunch of antibiotics shots 4 times a day. That hurts. Oh I had tubes connected from my jaw to drain out blood and fluid, the third day was the worst when my face felt so heavy and numb, I felt fatigued overall but it got better after removing the tubes and a getting a good sleep.

what i've learned from the video: 1- open bite is a condition where upper and lower front teeth don't touch when at rest 2- open bite can lead to tooth wear, tooth breakdown, tmj disorders and speech problems 3- open bite reconstruction surgery involves removing some of the bone of the upper jaw and move it to new position plates and screws are used once the jaws are aligned to secure it in position 4- protruding lower jaw is corrected by mandibular setback surgery. tooth bearing portion of lower jaw is separated from its base and moved backward for proper alignment 5- orthognathic surgeries are made along with orthodontic treatment and take several years to complete

Just had underbite double jaw surgery yesterday at around 6 AM. I was unconscious, didn't even realize I had the surgery. One of the surgeons before the surgery said I was going to feel like a trainwreck afterwards. I was hoping he was exaggerating, but not gonna lie, I felt like fucking shit :/ I didn't realize it at the time, but it was pretty bad. The worst thing was the swelling. I thought the first day in the hospital would be only temporary and the worst day, but here a day after, and right now the swelling gotten even worse. And you're right, I still don't feel like myself with my underbite before, honestly, but one of the surgeons showed me the before and after pics and the afterpic looked pretty good. I just hope the swelling would go down quickly and I would like to be able to confidently smile more. I had my braces on since I was 12, and I'm now 19. I am also worried about keeping my mouth clean after eating, cause I find it pretty hard to open up my swollen mouth, but I have to, or else I'll get an infection :/ The good thing about this whole experience were the nurses and other assistants who helped me with water, food, bathroom, etc. They were very nice. I'm hoping I won't regret this in the end, honestly. It might be wrong, but I felt unlucky to have an abnormal jaw and have to get surgery, unlike other people... I was also bullied for it, at least in middle school and first year of freshmen year of high school. Now my jaw has gotten better, but I figured I had to take the surgery since it was planned ever since I got braces.

It’s a life changing experience but personally I do not regret it one bit. The outcome is amazing.

I may be getting this surgery to bring my lower jaw forward. I have severe overjet where my front teeth protrude horizontally over my bottom teeth, but an orthodontist said its because my lower jaw is too far back and that my upper jaw is in a good position relative to the rest of my face. I'm 18 currently and haven't been able to get braces as a child, so I'm not sure whether to go with the surgery option or the option where i get two premolars removed and my front teeth pushed back.

My dad had this surgery done in the early 90s for orthodontic reasons. I had an implant surgically installed over 10 years ago and jaw surgery was discussed but I didn't choose it in the end. I was in the middle of college at the time so it would have been a crazy time to recover from a reshaped jaw.

I’m 15 and went to an orthodontist just 2 days ago who said I was a candidate for this surgery and said I have a class 2 overbite. I convinced my parents to have this along with a rhinoplasty when I’m 18. I always knew I had a class 2 overbite just needed the words of a medical professional to convince my parents, hopefully I’ll go to the surgeon the orthodontist recommended pretty soon to have a surgery plan and determine when the pre-surgery orthodontic treatment will start. I’m actually so stoked even for the braces part, the only issue is my teeth are perfectly straight now as I had braces 3 years ago and to fit my new face they’ll have to be changed.

@GK-ie6dd
@GK-ie6dd Жыл бұрын
Well I’m still getting the rhinoplasty but not the jaw surgery. I went to another orthodontist and they basically said that it wouldn’t be worth the hassle considering that my overbite isn’t so noticeable. So now I’m getting a chin implant instead which is much less invasive considering they don’t move your actual jaw bones. Of course this’ll all still happen when I’m 18 so not yet.

Is there any way to like, move the teeth further back in your jaw? Like I would want my lower jaw to be moved forward or just stay how it is, but my bottom teeth need to be moved further back. As it is right now, to bite with all my molar grooves aligned and for my bottom incisors to clear the top ones, my jaw has to go back not an uncomfortable amount back but still further back then I'd like to. If I were to keep my jaw where I'd like, my bottom incisors hit the top ones and prevent me from closing enough to have the molars touch. I know when you're resting you don't usually have your teeth touching but I would really rather have my lower jaw further forward while keeping the teeth further back. For both comfort and cosmetics I guess. If it were only cosmetics then I could just get a chin extension basically but as it stands I already hate how my teeth align. And this is after braces. Sigh. My fucking mouth and jaw are awful. Joint issues, frequent biting of the back of my cheek or whatever it decides next, bite issues, it's not symmetrical, had to have a retainer to widen my upper palate, can't do either of the rolled r's that aren't the English one (not rolled) and the tap with the tip of your tongue, my top teeth aren't centred with the lip frenulum and my bottom teeth aren't aligned with either of them because of a molar that never erupted and had to be pulled which left a big gap and meant my teeth shifted, and the last molar wasn't brought forward until years later with braces and then a couple more years until that wisdom tooth erupted to take the extra place, and the top teeth shifted after I had a forsus spring to move the bottom ones forward which in turn moved the top a little back (on that one side, so both jaws' teeth basically rotated opposite clockwises), and just UGH this turned into a stupid rant about my stupid-ass mouth and jaw. Fuck this shit.
