As seen in Pokémon Origins, Gym Leaders have their teams based on the amount of Badges the player has. I’d imagine the teams for the Kanto gyms in GSC are used for trainers getting their 8th badge from them. They probably don’t have any higher level Pokémon, which is why they all have similar levels.

These gen 2 games are 3 years ahead of the gen 1 games story. Giovanni disbanded team rocket after losing to Red at his gym and disappeared. In the gen 2 games rocket executives try to bring back team rocket and attempt to reach out to Giovanni before getting beat by Gold at the radio tower and ultimately failing. With Blue having lost his role as the champion he became a new gym leader and filled in the vacancy that Giovanni left much like Janine did with Koga going into the elite four.

Pretty sure this is default behaviour. It's due to Gameboy limitations of how many sounds can play at once. Plus you wouldn't be able to hear the cries properly if it played at the same time as the music.

Giovanni was the leader of team rocket while being the secret gym leader for the Viridian gym. He isn’t in the Johto games because he disbanded team rocket after losing to Red in a gym battle and ultimately disappeared. While rocket executives tried to restore team rocket and bring him back three years later they ultimately failed and Giovanni was never reached. He actually has an event in HGSS where they time traveled to the moments leading to Giovanni’s disappearance and establishing Silver, Giovanni’s son, as a character. As for the gym, Blue takes over the gym since he lost his spot as champion to Red.
