top left shows he's actually injured and suffering from debuffs from injuries, so he either used them all on a game or had a friend injure him to heal himself. the black legs means they're destroyed completely, red rest of tje body means injured, by patching himself up they went from destroyed to injured. The panting is because he's really hurt.
