also rinse with salt water a few times a day, thats really important. another reason why implants dont look as good as your denture yet is it takes months for your muscles in your mouth to adapt to its new surrounding. i had the same problem with my denture at the start as i did with my bridge, i thought both looked to big for my month but over time it looks better everyday

I was partial to the look of my dentures, as well. I stated that up front to Dr. Golpa and staff. They tried their best to mimic the dentures, took a mold and everything. The one thing that makes them look so much different is the bone loss on the bottom. Were it not for that, they would appear very similar. My face looked much fuller with my dentures. Maybe someday I can have that corrected but for now i'm just thankful for everything I have. Link to my denture dentist in description box.

Very true. Sometimes our expectations can leave us unprepared for real situations that will come up. I also wonder if loose screws will be an ongoing issue. Doesn't seem like it should.

You speak of pressure at the beginning of this video when the prosthesis is being placed. For those of you having this treatment done, if you experience similar sensations, bells and alarms should be firing. Likely what has happened, is that during the impression stage, insufficient steps have been taken to ensure the transfer of the replica implants to the models for processing has been done accurately, and one or more are not in the exact place as they would be in your mouth. Then when the titanium substructure is milled in lab, it is done to perfectly match the implant replicas in the model, which are no longer in the exact same location as they would be in your mouth. This causes the sensation of pressure in your mouth, creating unintentional and undesired stress on the implants and the screws that hold the prosthesis in your mouth, possibly leading to the breakage you have experienced. You should have a PASSIVE fit! Secondly Id just like to touch briefly on acrylic VS Zirconia/Porcelain as my knowledge on the latter option is fairly limited. Acrylic has it's advantages: it is extremely easy to repair and its cost is reasonable, when designed properly with adequate thickness it does provide a sufficient platform to build a successful prosthesis. That being said, I believe it was mentioned in another comment thread that acrylic is porous. This is absolutely correct, denture design is of utmost importance in fixed cases to allow the patient to easily clean the denture. This is why the gum portion of the denture cannot be stippled for a more natural look, but even with a smooth surface it is up to the patient to be extremely diligent with cleaning (water picking twice daily and hygienist cleanings sometimes as often as every 3 months in extreme cases), good for you for doing so! If not cleaned properly it is quite common to see tissue inflammation on the gums where the acrylic contacts them. Zirconia/Porcelain is a much more bio compatible material and therefor the ideal material for this treatment option. It however also has its woes: cost is the most obvious deterrent, it is VERY expensive and more difficult to repair than acrylic. Also if you have chosen this treatment option you will know that with an upper and lower Zirconium plate, it is like banging two rocks together and the old saying "something's gotta give" certainly applies. My recommendation: if your willing to spend the money and go for zirconia, put it towards the upper denture, that way you get the aesthetic advantages on the arch people see the most but oppose it with a softer material, like acrylic and just have it "re treaded" when you burn through the teeth. The last thing you want to go through after this long process is an implant failure. I hope this comment helps anyone considering this treatment option, implant retained dentures are leaps and bounds better than traditional dentures but at the end of the day they are nowhere near as good as your natural teeth were and come with their fair share of complications. Good luck out there!!

Hello, I watch you all the time. you have been an inspiration to me. my question to you is how did you know the issue was a screw vs an implant fail? 3 mos ago I got an all on 6 and recently felt some looseness on one side. went to prosthodontist and he tighten screw so I feel fine now but he also referred me to my surgeon to make sure. My surgeon did a ct scan and all while everything looks good he said the ultimate test is to take off the over denture and check to ensure the implants did not fail. if this is revealed in the test he ethically can not put back on the over denture and said he will have to replace with a removable denture until the healing is complete. I opted to move this test out for a few weeks as it is depressing to think of wearing the denture again while I am not currently feeling any issues. any advice or insight is appreciated. Thanks

Wow, sorry that happened to you. That's good advice for all of us. I know you've talked about this part of the dental saga before; that although there have been phenomenal strides made in reconstructive dentistry and dentures, they will never (never) function as well as your healthy, natural teeth. It helps to come to terms with that and do the best you can. I wonder if the screw tightening will be part of the regular maintenance now?

I'm very sorry this happened to you. Unfortunately, I'm not surprised. This is exactly the reason my dentist doesn't trust the All-on-4 system. Conventional wisdom always held that you need at least 6 (lower) or 8 (upper) implants to secure a full bridge. Yes, with 4 it can hold, but the biting forces are such, that there is enormous strain on each abutment. And that kind of strain causes things to break or come loose. Normally, abutment screws shouldn't come lose after only a few months...

Those are really only used once in a while. I keep them on hand for stubborn food that won't come out with normal rinsing, which is rare.... so no, that wouldn't have caused it. It's scary but i'm sure, as with everything else, there's a simple solution. I'm calling around first thing Monday morning to see if I can get it taken care of! Thanks ;-)

I think it's great you have a healthy attitude and approach. Anything invasive/surgical always runs with it the chance of complications. It'd be silly to think everything will come out perfect, every time. The sky isn't falling, you've got an issue, you're learning, your body is still adapting, it's still relatively new, and you're still learning how to chew with them. I know you're anxious but relax my dear, you'll be fine. You may need a few adjustments to find your baseline, & that's okay.

Oh and I forgot to mention that the 8 implants in the treatment option are 2 stage implants and not immediate load such as the ones Kristi in this video have. I thought at first I wanted the benefit of instant permanent teeth via immediate load implants but not after doing my research. I like the idea of giving the implant and bone time to integrate without a load being placed upon them. Less chance for failure. A temporary denture is used until it's time to place the perm arch on implants
@kalinjax

From everything i've gathered online, the way in which the implants are placed using this system makes up for the fact that there are only 4 implants. I don't have all of my back molars and I can only assume that is not to stress the implants. But no, I don't see why the screw should be coming loose so soon. Only thing I can think of is that i'm grinding them in my sleep because it sure can't be anything i'm doing during the day...

there are a lot of things held by screws or bolts that become loose.. an example is your car's wheels.. you get your new tires installed and they tell you to come back in 100 miles or so and have the bolts retightened.. fasteners that support live loads usually need to 'seat in' and be retightened over time. is probably not a problem with the implants themselves other than needing to be retightened. kind of surprised you weren't informed of this very common situation (with fasteners in general)

Hey WillJay8130 i believe she has traditional implants from what i can see and following her journey. I'm a dental implant consultant i see LOTS of failed mini implant cases. It take at least 3 mini to equal one traditional makesure to do your research. Also if you're suffering from bone loss you should call us 855-544-5089! It's a bit more than 800 per arch because the procedure and check up are involved and surgery but there's financing and rather invest in something permanent than temporary like mini implants.

If I recall from some videos I watched about implants, the screws are tightened using a small torque wrench to prevent over/under tightening. I wonder if it's possible to purchase an identical tool as used by the dentist? It's easy enough to use it as it has a little wire that points to a scale of how my foot pounds of torque is being applied. Sounds more complicated than it is... and a spouse could easily perform it. Heck you could probably do it in the bathroom mirror.

3:45 there can be problems with all-on-4 denture bridge implant hybrids. 4:15 plenty issues, wasn't prepared for the kind of pain, or that a molar would break off, or that a year in my whole bridge was on brink of falling out. out of state dentist can pose expensive hurdles. chewing sticky gum is likely tougher than apples on dental systems. but it sounds like it was under strain. like what ryan says: You speak of pressure at the beginning of this video when the prosthesis is being placed. For those of you having this treatment done, if you experience similar sensations, bells and alarms should be firing. ... This causes the sensation of pressure in your mouth, creating unintentional and undesired stress on the implants and the screws that hold the prosthesis in your mouth, possibly leading to the breakage you have experienced. hey no problem.. you should really look into supplement drinks, its great for the bone and stops bone loss, you can even get supplement pills. try to stay away from eating hard food for the first 4- 6month until the bone fuses together the supplements will also help this. i pay 100$ every 3months for a cleaning. i live in canada, but i also get all my dental work done at a dental university which cuts the cost by 75%.

It is not the dream we expect and there are problems, I have had to fly back to Costa Rica every 5 months because something is always happening. Now more is known. The reason for not having dentures is to prevent bone loss. No matter what the expenses for all of it has run 35,000, under what USA people pay. There is pain and responsibilities for the patient and a mature attitude helps, but still these are not for everyone. They look beautiful. I cannot chew like I used to with natural teeth like when a raw carrot salad did me in. Apples are a challenge. It is likely due to full body osteoarthritis. The challenges are always being addressed but there will be a time that I cannot keep up with it. My prosthedontist is USA trained which has been a plus.

@@kalinjax I had 2 implants on top with snap on teeth. & a bar on 4 on bottom, So I am in London & it would have cost at least 8k. to upgrade, its about half in Hungary & Roumania so I went to this guy in Roumania & his price was about £4.5K to add 3 implants on top & new teeth top & bottom. (I didn't like the bar much I guess it was ok at first but it got lose & food goes under, it would do for a nightmare,) So he did the implants on top & when it came to fitting teeth I found out he was going to glue them to the abutments & bar wit temporary glue which I have now & am supposed to go back after a month for permanent glue. I thought screwed on was the only way.
@kalinjax
@kalinjax 5 жыл бұрын
I have never heard of such a thing and can’t imagine there is a glue on this planet that would permanently secure a dental appliance to an implant. You need to get to a local prosthodontist straight away to assess the situation and see if it is even possible to screw them in at this point. This is the risk in traveling to different countries for dental work. It is cheaper but the regulations are different, the tools are different, the appliances are different, techniques, etc. So you need to know exactly what sort of work was really done and what the next steps should be but I honestly have never heard of gluing anything to an implant. Good luck and please keep me posted.
