First I would like to thank you for taking the time to make videos, I've watched all of your videos, they are very informative, I wish I could attend any of your presentations if you do any, even if they are over my head, I've been a dental technician for 19 years (ceramist) and now working with all on 4-6 and pink tissue, for sure the layer bridges look better than an all on 4-6 monolithic or crown bar all on 4-6 but I feel that this cases are more functional than esthetic and layering one of them might compromise the strength of it, and the reduction of bone, well that is just crazy, please keep those videos coming it had been a while since you had one more than 10 minutes, thank you.

@@mcfaddendentalimplantcenter I don't know if you have covered the difference in cost but been through the ringer with damage to my mouth and then a dentist that prepared my mouth for full implants agreeing to do all the work pro Bono due to my situation and cause of my teeth loss. Still, I paid all I had $5,000.00 feeling so blessed for this gift of true confidence and self esteem. Waiting with very badly fitted dentures for 6 months I get that call and on cloud nine I go in and just overwhelmed with anticipation. Then the Dr. Came in and said ok this will be what we need to finish your implants handing me a bill of $35,000+ so gobsmacked I was I just broke down and said you were to do this pro Bono. She said oh no and here is the treatment plan which was altered with all prices and even copied my signature. I had no proof I trusted them. I had no more money but I asked can you please make proper dentures....yes for $500. I still have plate that requires me to buy at least a tube a weak plus powder sealer. Because of my dental condition had already put me in a very fragile state and now I have Agoraphobia, severe depression, severe anxiety and NEVER leave my front door. Yes I am getting help but this hasn't just caused me financial, physical and medical issues but serious mental and emotional issues to the point I now am disabled. So important a smile could be for me to have a life again I just really need to know the bottom line cost. I know I probably will never be able to afford it but I'm hopeful. I have given a few classes but, honestly, the technique is so advanced that the only people who can even begin to start this learning curve are those dentists (mostly surgical Prosthodontists) who have done dozens of large cases. I will release a video soon that demonstrates why it's so difficult which causes the treatment to be very rare. I literally just finished a surgery and I feel like a ran two marathon. The surgery is difficult but the real limiting factor that limits these treatments to Prosthodontic practices is the immediate (day of surgery) fabrication of the FP-1 (implant bridge temporary). If you haven't seen my "Why Some Dentists are so Much Better than others" please see link below. It probably sounds elitist and I don't mean for it to be that way. It's a really time consuming, delicate, precise surgery that most dentists (especially Oral Surgeons) do not have the patience to perform. A Prosthodontist must be involved for the entire day so it's also a logistic nightmare for that office. We have three Prosthodontists here. I hope this explains some of the reasons...

Excellent question, and yes, one does. The All-On-4 would be likely to eventually cause the issues you mention. The implant bridges (as shown here) fit intimately up against the gums without gaps between the prostheses and the gums, therefore, eliminating this space and air flow issue.

Hi, Dr McFadden, So I checked out some implant dentists in Brighton, UK, where I live. They quote 'from $40,000'...so I am just wondering if you can quote a similarly rough figure to see if I have enough money to come out to Dallas? Basically dentistry is still a bit of a Wild West scenario, with many not very ethical, not very skilled implant dentists offering substandard care. But for your videos I too would have gone down the 'All on Four' route, which is the standard offering in Brighton, [and indeed in London]. Judging by your videos I imagine you are swamped by people from all over the US [and indeed the world] trying to find your level of skill, ethics and knowledge where they live-which must be rather frustrating! Dearest Dr McFadden. I’ve written to you several times with thanks and feedback as well as gratitude for all the Frank information you have put into your videos. I’ve appreciated them more than I can honestly articulate I have friends in the dental industry. She , besides being one of my best friends, when my teeth still were like my pearls, but I was beginning to struggle with decalcification due to autoimmune issues as well as long term medication uses was my hygienist. He, owned and operated the local oral maxialfacial lab and was a genuine artist in his craft. They have both retired now however I still have the benefit of instruction as well as referrals to the best of the best of dental physicians in my area. Still,you were my first introduction to the lowdown on what’s e what in the varieties of implants and I will EVER be grateful to you for you generous instruction. I literally shutter at the thought of having made the mistake of choosing 4 on 4 in ignorance. You made pivotal instruction in my health. Fortunately my. Generous husband understood why the implant arches would be the wises choice for me, and with the help of a genuinely stellar oral surgeon, Don Nikchevich and an extremely caring and talented prosthedontist, Blake Mueller, yesterday I had my initial implant surgery. They retained 8 of my strongest, healthiest, most well positioned teeth and places temporary caps on those just to help strengthen my initial approximately 6 month implants while they heal and those will later be replaced with the installation of my permanent implants. My mouth is very sore today, but I have no doubt that we made the proper decisions and chose the proper medicinal technicians. My temporaries, being only temporaries are so lovely, if I had to live with them forever, aesthetically, I wouldn’t complain. This has cost us such a staggering sum of money, it’s difficult to accept. But I really want them to last. I only want to do this once, and the implants are spaced in groups so if God forbid something needs repairing, it won’t be a whole arch. I sincerely wish to thank you for your contributions to my decision making processes. I’m grateful to you from the bottom of my heart. Any comments on maximum segmentation of the bridge versus one full horse shoe bridge? Thinking of future replacements and ease of delivery. What factors are in play and what priority? Strength? Contacts? Replacement?

These are great questions! As my protocol has developed, I have arrived at solid answers to these questions. If the patient has enough bone, I try to place as many as eight implants, per arch (jaw) so that the Zirconium has the fewest number of pontics (missing teeth on the bridge, between the implants). Smaller spans make the Zirconium stronger. With bone volume compromises, we can almost always get six implants in one arch (jaw). So, the first answers is, with eight implants, we can complete the arch in segments, usually three bridge pieces, sometimes four. My second consideration is the length of the teeth (clinical crown length). If the average length of the teeth on the implant bridge is 9 millimeters, or less, the patient can have separate bridges. If the tooth length is 9 millimeters or more, I make the bridge in one piece of either 12 or 14 teeth. With long teeth, the patient can feel air and fluid at the contact points, between the separate bridges. Patients do not like this sensation. They tend to get food impacted in these long contact areas. When they begin to notice the food they begin sucking air through these contact areas and that becomes annoying for them and their family members. I often become friends with my patients. I was out to dinner with a patient and her husband when I heard the sucking noises and called her the next day and said, "Donna, we must fix that!" So, we modified her three pieces into one piece and that fixed the problem. So, like mentioned above, with six implants the bridge will always be one piece. With seven or eight implants, we have options based on all of the above. As strong as Zirconium is, it is not unbreakable, so I want it supported enough to give the patient the best possible chance for long, long term success.

Thommen Medical is the implant manufacturer I mention there. Straumann makes a good implant, but so do a few other companies. I don’t believe they are “the best”. Where are you located ? I will fly to you if I have too. I went to my consolation yesterday and I was not happy at the results I saw. The teeth looked absolutely fabulous but he kept skipping the pictures of the full mouth open pictures until I made him stop show me all the pictures and told him about this video. He then explained that you have to qualify for this type of implant..he said something about f1 f2 f3 and so on, he said if the patient doesn't have enough bone they can't get the implants directly on the gums. I was not comfortable with the explanation and I don't plan on going back. So I'm intrested in knowing where your located and if his explanation is true ?

I had one question, I’m curious on your opinion as there are some conflicting ones out there. I have multiple crowns veneers implants and have to replace some of them now due to excessive grinding , I had them I’m for 7/8 years now but due to grinding the force went too much in my front teeth and a bridge cracked. My doctor told me the best option is to replace all of the upper ones so I can replace the contacts properly and distribute the force, I already have crowns or veneers on all the teeth. My question is which material would you choose, she’s recommending emax which I agree on for the front teeth as it will probably be better but I heard that the zirconium one is stronger so wouldn’t that be a better option on the back teeth to be more durable against grinding. Also do you know any good therapists or solutions agains teeth griding(other than that annoying night guard 😩) Would really appreciate your answer! Thank you

I agree with EMAX front and zirconium back. Nothing else can protect your teeth like a nightguard. Keep wearing it. Sounds like you might be grinding during the day, too.

I was just recently told that all my back teeth would have to be pulled except for a single implant. Is it possible to have this type of implant for just back teeth? The only type of bridge discussed with me have been permanent bridges, which, aside from no longer having a tooth on each side to anchor to, seemed so awfully destructive to any remaining teeth. Don't think it would apply to me now. Only was told I would need implants.

This is what's lead to the destruction - gum recession, bone loss and several caries below the gum line. Despite singular focus on good dental care, I've lost the use of my salivary glands. Still I've used moistening products every moment of every day. I'm disabled and unfortunately the cost equals my yearly pension. I'm devastated trying to imagine how to exist with no back teeth. How long will my front teeth remain stable before shifting and further gum and bone loss will compromised them. One dentist spoke candidly, telling me "tooth replacement is only for the rich". I know firsthand, the overall impact of body and mental health from poor dental health

I am from San Francisco, I personally made the trip to Dallas Tx, to meet Dr. Mc Fadden to start my treatment for a implant bridge for my upper arch. It was an easy flight and found a Airbnb place to stay within walking distance from his offices. His office staff, dental assistances and Staci and Barbara were all so welcoming for me , made me feel like I was a local Texan ! Furthermore Dr. McFadden is so personal and professional and went above and beyond to make me like I was his highest priority case of the day. He thoroughly discusses the treatment plan and options. What makes him stand out is he is “down to earth “ speaks to your level, although he his top of his class in his field and he is HIGHLY intelligent , he had me feeling like we long lost friends and I am so glad that I took the faith in him to come see him in person. Greetings from Northern Ireland, i need to get serious work done on my teeth, going by photos of my mouth alone, 3 different dental implant centres have told me i need all on 4, after watching your posts, im dead against it. I had bad gum disease for a few years over 10 years ago, i worked hard to try and keep as many teeth as possible, unfortunately i have lost a tooth and had 5 extracted over all. Im in a position to get some cosmetic restorative work done, i love the look of the dental bridge 3 on 6 solution, but my jaw bone density is poor. What solution would you recommend?

hello Doctor i am missing almost all of my back teeth upper and lower both sides. My doctor is suggesting 6 implants as option one #s 3,4,13,14, 19, and 30. Option two is 3s 2,4,13,14, 29, 30, and 19. These are individual implants. My question is would it not be better to have an implant bridge on both sides upper and lower? Using two implants on both sides upper and lower for bridges that fill more space for chewing. This would mean i guess 8 implants then. Would option one (the least expensive) allow me to chew food in the back of my mouth?

I'm from western Pennsylvania and went to Pitt Dental School. Howdy fellow Pennsylvanian !! Unfortunately, I only know of a few people in the country who will even talk about doing these. And, most of those will only speak of it and still give the patient all-on-4. So, as much as I would like to make a referral, I do not know anyone. Could I ask do the bridges need to be removed by the dentist to be cleaned? I heard all on 4 or 6 need to be removed every year or so to be cleaned so I just wanted to ask about your bridges? Thanks

Only rarely do we have to remove the bridges. Mainly, when patients have pressure or pain we remove them to inspect the problem. Almost never for hygiene reasons. The All-on-4 offices do this because their design is uncleansable. They have now discovered a way to monetize a design flaw.

I am looking into them now due to having a genetic issue causing my teeth to go bad pretty much regardless of how hard I try. Hello, first of all thanks for your content, however I think a couple of statements can lead to misunderstandings. One of the main difference choosing an all on X or implant bridge is the vertical dimension. it's if the patient had lost maxilar or mandibular bone. Sometimes patients that have been edentulous for a long time have their alveolar bone atrophied leaving only base bone. Remember that the pattern of bone remodeling in the maxilla is inwards and the mandibular is outwards, this can lead to the implants being angulated. Personally, I prefer my rehabilitations to be direct to the platform and not use mini pillars. However, that does not mean that there are no cases in which they should be used. Each patient is different and the case must be studied and planned before performing any action in the mouth. The axis system (or dynamic pillar) is undoubtedly a great tool to carry out our rehabilitations.

The problem with the All-on-Whatever (FP2 and FP3) being recommended 100:1 over implant bridges (FP1) is that 80% of our patients presenting for treatment having some or all of their teeth and good to great bone, Even a few percent of the edentulous patients have very good bone. For these 80+% of patients with good bone, it is absolutely inappropriate to misinform them that they are only candidates for FP2 and FP3. Sadly, this happens in nearly EVERY All-on-4 office EVERY DAY. For the 15-20% of patients with severe bone loss there are still only a few small percent of them where All-on-4 is indicated. Traditional "All-on-4" prostheses cannot be cleaned by the patient, so, again, I would argue that they are not appropriate for 99% of patients with the need for full mouth implant treatment. So, in conclusion, there may be about 1% of the population that have no other options but the traditional "All-on-Whatever" protocol.

I am familiar with that technique. Pros and cons just like every other treatment. In the past three years I have come to believe that on piece is better than multiple pieces (bridges) because we get "cross-arch stabilization", meaning, the the implants on the opposite side are providing stability to their counterparts. Also, screw retention is always better than cement retention. Still, 3-on-6 is far better than All-on-4.

No, it is only removable by your dentist. With that said, removing it for cleaning purposes is not necessary because of the way it fits against your gums. There isn’t that big ledge between the prosthesis and gum tissues for plaque, food and debris to accumulate. It is designed to look, feel, function and CLEAN just like bridges on natural teeth.

I have the zirconium teeth which are great but the gum materail tends to scratch and chip. I have had to go back serveral times to get the gum materail smoothed out.

I am told that due to gum recessing and bone loss that implant bridges cannot be an option on my lower teeth. My only option for my lower teeth is one piece bridge...I am not happy? My next question, is with my upper implant bridges, false gum was added to the back of the bridge, is this for more support of the implants? It is unfortunate, that I found you videos after the fact. Some of my lower teeth were replaced with all on 4 type, now I need the rest of my teeth extracted, I am perplexed about my option? Please clarify???

That's a good start! But: For over 10 years, we (the dental industry) have known that dental cements are toxic to the gums and bone around implants. Offices that do this (use cement instead of abutments and screws) opt to do that because it is much easier and less expensive for them. Not better for you, the patient.

I have crowned teeth that are good after over 30 years. I also have cement retained dismantleable cantilever bridges from implants in both upper canines to the first lateral incisors. This is normal good practice in the UK by a very experienced implant specialist. How else would you do this? Thank you.

@@ColinDyckes You are correct. It is not the same for cement on crowns on natural teeth. Well made crowns will last decades. You’re living proof! It sounds like you’ve found yourself a good dentist. And from your description on the cantilevered implant bridges on the canines, you state that they’re dismantleable (screw retained), which in many cases, such as yours, is a perfectly reasonable treatment.

I am on shopping to fix my teeth. I did get inspected in Jefferson dental. They asked me to pull out nine teeth and a few hours process. Made me scared and concerned. I am in Dallas area. So i decided to watch more and do some research on myself. I don’t want many problems after surgery that will frustrate me. I don’t want to pull out all teeth. Smile and beauty is least important for me. Function and strength. Duration and easy to clean is my top priority. I will do more research and decide

The initial soft tissue healing is 1-2 weeks, and the time we allow for the implants to fully heal/integrate with the bone is a few months. (Anywhere from 2-6 months, depending on various factors that may require longer healing time. )

Hello doctor and thank you for your great video! Very informative. I am a lab tech in California and I do implant bridges and all-on-4s and correct me if I’m wrong but I think that the term all-on-4 just refers to the number of implants used. It still in an implant bridge after all, but with tissue built in? I fully agree that a concave shape for the tissue isn’t as good as a convex design, but sometimes you need to build tissue where it isn’t present, either from removal, trauma, or natural loss due to not wearing a proper prosthesis. Dentures have an average bone loss 1-2 mm per year, more if proper hygiene isn’t followed. Do you actually see a lot of cases where bone is removed? Unless it is needed for gummy smile reasons? If so that is truly insane. Usually we strive to replace bone loss not remove it! And it is a pain staking and time consuming effort that doesn’t always give us the results we want. I also agree that there is a lot of “lazy” dentistry being done, especially if there is tissue present but the extra time is not taken to condition it into a socket shape for an ovate bridge bottom. Not a lot of drs really know how (or want to take the time) to do this step properly and for a technician it is very frustrating as it can totally compromise esthetics and cleansability of the final bridge. But I think the All-on-X name gets a bad rap for this as it can be adjusted by being cutback and layered with porcelain and shaped properly also. Just my 2 cents. Yes, aggressive bone removal is a standard part of the All-on-4 protocol. As soon as an abutment or an implant becomes supra-gingival (above the gums) the prosthesis becomes uncleansible in that area. If all four implants are super gingival and has a con cave tissue surface the whole thing is uncleansible. Thank you for your comment.

Hello Dr I’m convinced you are a truly great dentist. I live on the Gold Coast the southern end of the state of QLD Australia. I’m a 66 year old female. I have only 9 teeth on the bottom & 8 teeth on top. I was seriously considering flying overseas to have the all on four or six. But watching your informative video’s I’ve decided against it. The options you show & speak of are definitely less intrusive & very naturalistic looking. Would you have any recommendations for dentists who practice your type of work. Anywhere in Australia,because seeing as I was willing to travel overseas. I figure travelling around this country is doable,even though it is a massive place. Too bad I’m old..worked hard…single parent…lost my youngest daughter 14 yrs ago and raised all 5 grandchildren. I’ve got dental issues. But put everyone before me. Family and shelter animals were helped instead. Stress and health issues prob have something to do with my caps falling out (neuropathy, severe inflammation, vasculitis and fibromyalgia and anemia, thyroid issues, etc…). I’m tiny but was very tough…pushed myself for 50 yrs. Now I’m financially, emotionally and physically drained. Are there no oral surgeons or dentists who possess empathy any more. I need help. I reread your complaint about my video. You say, "about where 'we' put the implants?". Yes, there are dentists who care more than others but the vast majority of dentists doing All-on-X do not even understand the basic concepts of the protocol. Even if you are one of the "careful" dentists there are major flaws in the treatment planning, informed consent, surgical protocol and resultant prosthesis. I doubt you would have this protocol done in your own mouth if you needed full mouth/ arch implant rehabilitation. To take the time to watch my video, scrutinize it for "untruths" and send me a message means you are at least open to learning something new. I hope you offer your patients the FP1 option as 80% of our new patients have existing teeth and bone thereby making them good candidates for a better outcome. If so, great! If not, I think it's time for some introspection about your practice and ethics.

Can you please explain something to me? I can't believe what happened. I had an implant bridge and it cracked. I had to save for a bit to get the $10,000 to replace it. I assumed he would just make me another bridge. My dentist put in an implant denture. It is awful. I went back and said I would wear the cracked bridge instead of this. He said the change he made is irreversible. I just sat there sobbing. He said he would make another replacement and would not change me more. I just could not stop crying. I didn't understand that this could even happen. Is there a way that what he did is irreversible? He did not cut my gums at all. Is there anything I can do? Thank you so much.

If you had a bridge once, you should be able to have one again. Bridges typically need less room than an implant denture.

Wow.....u are not right. U are mixing concepts. An all on four is an specific technique developed when there is lack of bone in posterior areas an in order to avoid sinus lift or bone graft , 2 implants are tilted . So easy. You just take the technique or not , dont say that the dentist are not carefull placing the implants . Anyhow...it is always good to teach people but do not confuse them more.
