It’s hard to advise anything without seeing a current X-ray, but hopefully, she’s a candidate for an implant bridge to replace the four missing teeth. If she has enough bone for implants, most likely a four tooth bridge on two implants.

Dr. McFadden I thank you for your honesty. I have been listening to and watching the videos of another dentist about implants and I thought they were being honest but I have never heard them discuss bone removal. It is really unbelievable the greed and misinformation associated with all on 4.

Can you please answer the dentists who have ask about classes & teaching this technique? Im curious why they don’t know though, is this not taught in dental school?

I have given a few classes but, honestly, the technique is so advanced that the only people who can even begin to start this learning curve are those dentists (mostly surgical Prosthodontists) who have done dozens of large cases. I will release a video soon that demonstrates why it's so difficult which causes the treatment to be very rare. I literally just finished a surgery and I feel like a ran two marathon. The surgery is difficult but the real limiting factor that limits these treatments to Prosthodontic practices is the immediate (day of surgery) fabrication of the FP-1 (implant bridge temporary). If you haven't seen my "Why Some Dentists are so Much Better than others" please see link below. It probably sounds elitist and I don't mean for it to be that way. It's a really time consuming, delicate, precise surgery that most dentists (especially Oral Surgeons) do not have the patience to perform. A Prosthodontist must be involved for the entire day so it's also a logistic nightmare for that office. We have three Prosthodontists here. I hope this explains some of the reasons...

I first watched this video 2 years ago. I was ready to go with all on 4 because of all the positive videos. You stopped me from making that bad decision. btw, I still haven't made a decision.

What about patients that have some form of facial neuralgia pain such as Atypical trigeminal neuralgia or atypical odontalgia that was caused by a dental prcedure or dental injection. I have been told that inplants would be asking for more nerve pain. Not sure if you are familiar with how many people suffer lifelong pain from nerve issues that started with dental procedure but would like to know if you have any knowledge of this. Most dentists either claim ignorance on the subject or just do not want to admit that dental procedures can go terribly wrong and cause people to suffer from pain and or numbness the rest of there life

I am aware of these complications. Fortunately, I have never had a patient suffer any of these life-changing outcomes. I would certainly proceed with caution. I would need A LOT more information to advise.

I live in upstate ny so obviously it wouldn't be Feasible to travel to Texas for treatment. is there any dentist in the northeast that you could recommend?

Try as I have, I have not yet found another dentist in that area who follows the same protocol as we do. But with the information you glean from many of my videos, hopefully, you can interview some prosthodontists and know which questions to ask and be sure to ask to see lips retracted photos of their work. I had a dentist recommend all on 4 for me because I have 5 teeth that need implants in my upper jaw. The problem I had with his recommendation was that all my front teeth are in excellent condition, except for a chipped front tooth. They are strong and healthy and perfectly aligned, yet he wants to extract them all and do all on 4 because I need implants in my back teeth. I said absolutely not. He seemed annoyed that I couldn't see the benefit of his suggestion. He even sent his assistant to try and convince me.🤔 Is it common to extract good teeth for this procedure?

Because you’re only paying for each phase of treatment AS IT IS DONE, and not the entire fee for ALL treatment which, in some cases may ultimately take 12-18 months to complete. In other words, you’re not paying 12-18 months in advance for treatment that is yet to be done.

Will the Interdental gingiva (papilla) eventually fill in the gaps if a zirconium roundhouse 12 tooth with no pink and 7 full size implants that were all bone grafted is screwed into place after wearing an interim denture for about 8 months while bone grafts healed?

They are most likely as large and full as they are going to be. Some minor improvement may happen if the permanent has more pressure on the tissue than the temporary.

Hello Dr / thank you for your upload / I am going through a terrible stage in my life - as I was assaulted last year which resulted in my upper two front teeth removed - and the lower four front are not in great shape as they were also impacted / so I will have to get them removed most likely in the coming years / it has destroyed my smile and ability to vocalise like I used to - which has shattered my life - and sense of well being - I have been diagnosed with severe periodontitis- I am wondering do gums have to be completely pink to receive a form of treatment?? Or can it still be possibly not to get a lasting effective result while still having poor gum health / as I think it may also be hereditary as I cannot really remember ever having pink gums - well maybe only as an infant to adolescent but even then I am not sure if i had great gum health - it has been about 18 months since teeth were removed - and i am also worried about the length of time it has been and damage done by denture - which i have been wearing for about 10 -12 months .. i have a partial denture for my front two upper which attach by hooks on my canines . but i sincerely hope to get optimal treatment to bring back my bite force and ability to vocalise - do you think there will be a possible avenue / that is not on the extreme end of costing / thank you for your time - i hope to hear back from you

43 years old. On permanent disability. I only have around $16,500 saved, with 805 credit score. The problem is my income is only $963 a month. Had to pull all but 4 on bottom front teeth removed between age 25-30 my teeth started massively failing. Root canals with crowns were at least $1700 each. My only option, according to my money hungry dentist wanted to do a lot of root canal/crowns. He could have done fillings I'm sure on several of them. I ended up caving in and had to have them pulled. I don't leave the house hardly ever. I wish someone would run into me while walking down the road so I could be "deleted". I haven't had a life since my early 20s. Is there any hope for me?! By the way I live in Southern Illinois, so I'm 11 and a half hours at least away from you. I have headaches, and jaw pain everyday. All those teeth being pulled I just know is what caused my trigeminal neuralgia, which is so painful it's known as a suicide syndrome from the pain. Also, I have tried two sets of cheap dentures and I could not wear those things. I couldn't talk right, and they hurt so bad, fell out etc. and I also had to have at least 4 Alveoloplasty procedures years ago while trying to wear those basic dentures. Little splinter pieces of jawbone would poke through my gum. I haven't had to have one of those in many years. I was told about 6 months ago that I had a good amount of bone left from a dentist.

Yes. With bone level implants we use straight abutments 95% of the time. 5% of the time we must use angled abutments because of bone anatomy. However, when you don't remove bone as in the all-on-4 process you can successfully use an angled abutment when needed.

Hi, Dr McFadden, So I checked out some implant dentists in Brighton, UK, where I live. They quote 'from $40,000'...so I am just wondering if you can quote a similarly rough figure to see if I have enough money to come out to Dallas? Basically dentistry is still a bit of a Wild West scenario, with many not very ethical, not very skilled implant dentists offering substandard care. But for your videos I too would have gone down the 'All on Four' route, which is the standard offering in Brighton, [and indeed in London]. Judging by your videos I imagine you are swamped by people from all over the US [and indeed the world] trying to find your level of skill, ethics and knowledge where they live-which must be rather frustrating! It depends on the implant. One kind you can avoid the abutments (Tissue level implants) and one kind you can't (bone level implants). I use both. The main thing about "abutments" is to avoid or minimize the use of "angles abutments" which are always used in All-on-4. It's part of what causes the problems with All-on-4. Better surgery without bone removal reduces the need for angled abutments. This is covered in my "Buyer Beware..." videos.

Healthy patients or we bring in a anesthesiologist. They are all long learning curves that most people can learn if you progress slowly and cautiously. IV Conscious sedation is incredibly safe.

such an excellent explanation! As a Sjogren's patient it has been a nightmare. I already have two upper implants and two lowere (one on each side) but my upper teeth are failiing due to the dry mouth. This really answered so many quesstions, especially as I have a very gummy smile and when my front tooth was removed the guyms receeded a bit. Looking forward to setting up a consultation.

I might have complications with tooth 31(root canal treated) and I was wondering if instead of doing a retreatment and new crown, if you would ever suggest doing an implant for 31? I think most of your restorations only include 24 teeth instead of 28, and I wanted to know your opinion on keeping 28, with implants

Yes. If that’s a tooth you need for chewing/there’s an opposing tooth above it, absolutely. We do single tooth replacements all the time!

@@davidmcfadden7927 yes I remove but all I am saying is your animated presentation is too much presenting bone removal. I remove just enough not to show transition of real gums plus gum prosthesis when patient is doing full smile or like gnarling smile that is why pre photographs are in a full smile and or sort of doing gnarling smile as one of the guides of course ct scan etc should be taken in consideration also. How much bone removal has no definite measurement it will all depend on how you think it will all end up pre surgery. So if you plan with end in mind you can come up with FP1, FP2 and FP3.

I will say I have gone through a brutal ordeal after an almost fatal car crash and a previous dentist that totally left me in dire straights after his treatments. Thank God I found Dr. McFadden. Interesting video. I am trying to decide between bridge implants which are 3 bridges each on 2 implants or do the all in 6 procedure. My understanding is that if one or two implants fail with the all in 6 you should be okay. If one implant fails with a bridge then you would need to replace that section but at least it is only one section. Also, bridges do not need to be removed ever year to be cleaned and I have a lower price quote to do the bridges by 6K. Looking for any advice as it is a hard decision and I plan on doing next week.

We no longer do separate bridges. There are several reasons why which I believe would only muddy the waters. I'm glad you're doing your research but I believe you are missing the biggest point about the differences between implant bridges and all on 6. You will have massive bone removal with all on 6. You will not have any bone removal with implant bridges. This is the main reason to opt for implant bridges. Your precious bone is worth far more than $6,000.

Very very informative, educational Thank you. I wish you were here in 🇨🇦. My second bout with implants in 10yrs. I am scheduled to have bone grafting on upper for an all on 4 denture. My first in 2014 completely failed due to lack of bone. . I don;t believe there are any specialist here in Canada that will do implants all on 4 bridges ( Zorconia) but this is fantastic information that I will be speaking with my surgeon about.

Typically, it’s always best to replace it with an implant- unless, it’s the tooth furthest to the back with no opposing tooth to chew against it…

@@danstutzke3387a bridge is another option, given that the adjacent teeth that would support it are healthy enough to support the bridge. The down side to a bridge vs an implant and crown is that eventually that bridge will need to be replaced. The implant would be a more permanent solution.

I agree with your thoughts. I have a few teeth that are bad, and the composite they put on my front tooth came off. I asked the dentist to add more composite or bonding as I called it. Boom! I'm referred to the Implant Place to get implants for $46,000! Why bother going in to get teeth cleaning 2x a year if it's for nothing?!? I'm disgusted. They had a big 3-D Xray of my teeth and 'See where you have an abscess there? It needs out NOW' 'OK should I take antibiotics now?' 'No it's coming out' Makes no sense. Nothing makes sense. I'm not falling for this. I'll glue my own piece of tooth back on if I have to. I call B.S. Thank you for your info. We're not that stupid, although we may look like it lol. I appreciate your videos.

There needs to be protocols in place before any irreversible, invasive, and permanent procedure is made by a dentist/periodontist stating, showing proof that ALL teeth shpuld be extracted. Its devastating to learn something so traumatic did not need to take place. The particular place I am referring to had amazing reviews with alluring photos. Looking back, there were so many red flags....Including telling me he came up with the all on 4 concept. How could I have been so foolish?

To get an opinion on my current mouths health that I can trust. No insurance sucks. Finally bought the best Delta Individual plan but the 12 month wait time for actual repairs is rough. on a managed plan that will begin in september though. Residing in Houston currently so it's not the worst drive I guess! Stay safe~!

Will tissue grafting be enough to 'build' your natural gum or papilla?

Great question! That technique has not been invented, yet. More tissue volume is always better than less but rebuilding a papilla is extremely difficult. We can get about 50% back with the shape of the temporary but that's all.

Can you change all on 4 to a 3 on 6 type bridge with no artificial gums even when bone was shaved?

Before a surgeon cuts the bone away, the patient not only should be informed but also told the ramifications. I had no idea this was part of the all on 4 process until it was too late. I'm devastated because the bulkiness is absolutely awful and I did not know about bridges until afterwards. I am miserable.

Never liked the look of the "all on 4". Just looks like a denture. I like the look of the implant bridges. They look way better. Great video. Thanks for sharing.
