SPOILER (Calamity mod Revengeance Difficulty mode's actual Inspiration/Reference) It is actually inspired from MGRR secret difficulty mode, the Revengeance mode. To Unlock Revengeance mode, you need to beat the game on Hard difficulty, and then Very Hard difficulty, or you can do the following: On the main title screen, enter the Konami code ( Up, Up, Down, Down, Left, Right, Left, Right, O/B, X/A, Start ), then Revengeance mode will be made available as a difficulty setting.

On the topic of the revengeance reference. It could be specifically referencing the "Revengeance" Difficulty mode in metal gear rising revengeance.
