Not only are Ethel’s lines extra mean, they’re also very specific. You’ll note that for pretty much everyone else, including other NPCs, the insults come from the generic pool. Ethel meanwhile can make insults that are not only specific to the target, but using information she should have no way of knowing. Some of her insults prick at the origin characters’ deepest secrets (Wyll and Astarion in particular). It strongly implies Ethel is able to passively read peoples’ memories somehow.

Their lore is all over the place and they are not well liked exactly because 5e made them so bland.
