Scleral lenses are large contact lenses that fit over the cornea and land on the sclera, the white part of the eye. Scleral contact lenses are often used for keratoconus, high amounts of astigmatism, post-surgical cases, and other corneal diseases including severe dry eye disease. Scleral contacts can even fit on normal eyes as well.
Benefits of scleral lenses include improved eyesight, great comfort, and can treat dry eye and other forms of ocular disease. Scleral lenses are also durable and can last for 1-3 years with good care.

@DoctorEyeHealth I am sorry for this random message/question. I have started to wear monthly soft contact lenses(Silicone Hydrogel). I have been wearing them for about 3 months. I was wondering, is it possible to contract Acanthamoeba keratitis even if your contact lenses never touch "impure"water(tap water, mud, swimpool water etc), and you never swim, shower or bath with them? I red somewhere that despite dilligent cleaning and never letting contact lenses touch "impure" water it can still happen in rare occasions. Thanks very much in advance.

Just wanted to thank you for your content - I watched quite a few videos before my last eye exam. I felt better equipped to ask the questions I had. The doctor decided to try a lesser prescription (I am getting older - very nearsighted with lenses at -10.5 and -11) to help with reading. I was getting very frustrated with my near vision as things about an arm length away were blurry - I felt it was really starting to affect my QOL. But the trial contacts he gave me at -10.0 were just worse all around for me. So I went back and saw his wife and she really listened tried a different contact (still at -10) and WOW! I could not believe how much better it was! I was starting to get really concerned that something was going wrong - I couldn’t believe that by just changing the lenses I don’t feel like my eyes are deteriorating- I dont even need to use reading glasses half as much as I was. Thank you for helping me to advocate for myself and to know there can be so many different lenses out there!!!

This is so encouraging to hear about the new technology. I've been suffering dry eyes last few years as I get older. My usual contact lenses just get dryer and dryer. I have to take them out several times a day to hydrate them just like the scleral lenses, except they do not hold the water as the scleral lenses. I also have to replace them twice as often as they were supposed to be. I will ask my eye specialist for them. If they last more than 1 year, do not dry out my eyes and make me see better, they'd be worth the price! Thank you very much for sharing the information. I see hope now 😊

I've been wearing scleral lenses for the past year , it's changed my life. From wearing glasses for 25 years with every doctor telling me i'll never wear any lense. Having really intense astigmatism and hypertropia. It not only makes my astigmatism magically dissapear but also improves my vision to over 20/20. It is simply amazing.

@@IntegrityMeansAll Hypertropia is a misalignment of the eye or eyes. For brands/doctors i would suggest consulting with a local doctor that supports scleral lenses and going through the process. I would HIGHLY recommend them for dry eyes as their filled with saline that will rest on your eye for the duration of wearing.

I would love these lenses! I have some astigmatism and a lot of Dry Eye! I wore contacts, mono vision, then had the bifocal cataract replacement lens - now I need help. I’m in my 70s - I can still manage pretty much with my bifocal cataract lens, with reading glasses and distance Glasses for tv. The Dry Eye would be my main reason for wanting scleral lenses.

Sclerals, although expensive ($2,800 including the lenses $1,700, fitment, tests) was worth every penny. I used to wear rgps because of my advance case of kc. My right cornea was so steep that the rgp would move out of place. I did get used them after long use, but I always felt they were there. The comfortability is day and night. Holy hell I don’t how I wore the rgps because how uncomfortable they were. The sclerals you don’t even know they’re on. Definitely worth it if you can afford, best investment I ever made.

I've been wearing a scleral lens on my left eye since October & it makes a HUGE difference. I suffer from keratoconus, so the scleral lens helps in a major way. I had a corneal transplant last year on my right eye & just recently for fitted for a scleral on my right eye.

Great question! Yes they work wonders for astigmatism. The lenses essentially negate out the astigmatism caused by the cornea of the eye.

I wear these for driving and seeing distance. Everything is all very clear. It feels like I can see to drive like I once did 40 yrs ago. I didnt realize they help with dry eye though. Thank you for the information.

Wow! These contact lenses sound promising for me. I have never been able to wear contacts because of my astigmatism. Both eye doctors that I have seen tried to fit me with single vision lenses, one for near and one for far, and those never worked either. I'm looking for a new eye doctor now. I'm going to ask about these. Thanks so much for the information!!!

Thank you for and other great video, do you known eye print scleral lenses and if so would you say there is a big difference between eye print and regular scleral lenses?

Amazing question! Eye print sclerals are fantastic but they require you to have a mold taken of the eyes shape/surface. We currently don't use this tech in our office but is something I would like to try myself. Otherwise, scleral lenses have been successfully fit without the use of molding/eyeprint tech for many decades. But people with unique scarring or other surface irregularities may benefit from the eyeprint tech.

I’m in the middle of the process for RPG lenses fitting and I have to admit it’s not easy so far, first set was 9.60 diameter, second set 10.0 each came with different prescriptions, now on to the 3rd set that the specs will be determined based from a video of the eye they made with liquid to see how they could correct the fit in one eye. They suggested that if unsuccessful, I should consider scleral lense for that side and RPG for the right eye. How many tries is normal for a fitting

Yes! I agree, they require a bit more care but still - pretty amazing. There are even some tech companies trying to make AR contact lenses out of scleral lenses.

Hi, I have astigmatism that makes me “unable to wear Regular regular eye contacts” quote on what my eye doctor said. So I have contact lenses that overlap my cornea. Something like the the ones your showcasing. Are they the same ones you’re talking about?

No, I hadn’t heard of them. I have very severe astigmatism to the point the best vision I can get with glasses is 20/50. In addition, I have dry eye disease, cataracts, and abnormal saccades (from a TBI). Do you think sclera lenses could help me? Six months ago, my doctor said that one of my cataracts had reached the point where I qualified for cataract removal. When he finished measuring my eye, he said that eye was NOT a good candidate for cataract surgery and asked me to come back in a year to check the other eye, and dismissed me just like that. So disheartening! I don’t know what to do.

I suffer from really bad dry eyes, but my symptoms are really rare. I only really get dry eyes at night. I wake up with eyes so dry Im pretty sure i've scrapped the epithelial tissue from my eyes and then have eye pain all day. I've worn moisture chamber glasses, eye ointment, prescription eye ointment, autologous eye serum, taped eyes shut, used humidifier, tea tree scrub for eyes. and nothing seems to work. Still wake up with dry eyes. I wonder if I can put some of my autologus serum drops and then sleep with scleral lenses. At this point i'd try that. Edit: if you are taking new patients i'd love to make an appointment sometime next year. Not a lot of knowledgeable people on dry eyes in my area it seems.

I would love your advice on getting a better fit. I have keratoconus and have changed prescription multiple times (about 4 times in 6 months). The contact I have now is the best I’ve had but I still see double at times and always see glare at night. Would you be able to help with a better fit ?

I tried scleral lenses to help treat damage I suffered from SMILE laser eye surgery. Unfortunately, despite the custom fit, they were impossible to tolerate due to my severe dry eyes. They felt like a giant piece of rough plastic stuck in my eye. Indeed the success rate is low, circa 10% in part due to the difficulty with insertion and removal. Additionally, I found the vision with sclerals was poor as my tear film could not form correctly over them. They can help some, but are generally used as a last ditch to aid LASIK damaged eyes.

That is such a sad story. Sorry you have had to deal with all that. True that sclerals themselves can be drying (as with all contact lenses) but they can also protect the eye in cases of severe ocular surface disease. Out of curiosity, what is helping you the most after the SMILE?

Scleral lenses sound promising. Years ago I tried to wear soft contacts, but could not wear them for more than about four hours at a time, so it wasn't worth it. It's important to be aware of how much care reusable contacts require. They have to cleaned and handled with care in order to last, plus if they become uncomfortable during the day, it's not convenient to take them out. The nice thing about glasses is you can just take them off whenever you need a break.

I had contacts that felt like that when I first started wearing them years ago. I went back to my doctor and told him that it felt like a foreign object was in my eye. He said "that's because there is a foreign object in your eye". He was of course joking (I did laugh when i found out he was just messing with me). Ultimately he helped me find a comfortable lens. Maybe there is one out there that will work for you!

So these are recommended for keratacons? I have suffered with this for about 25 years. It has not progressed much since originally diagnosis but still a real pain in the neck.

Over the past many years i have tried most lenses, but my extremely dry eye made it impossible to use tenses. That said I haven’t tried any in the last few years . These might be great, but i am now in my 60s and have my cataracts done, wonder if one can wear these with new cataract lenses. Also have had retinal tear repairs in both eyes. No detachments. Also a vitrectomy in one eye. So a lot there to evaluate if i would be a candidate for these. I still use glasses for reading. Love your blog.

I have had 4 keratectomies and tried every product and procedure on the market. Still, I need to put drops in my eyes several times a night to prevent recurring corneal abrasions. Would scleral lenses work if I wore them to bed at night?

doc i have dry eye symptoms dry, gritty or burning sensation in the eyes, redness, watery or teary eyes and mucus that make the eyes feel "glued shut" after sleeping and at the same time i have retinal detachment symptoms like A sudden increase in floaters (small dark spots or squiggly lines that float across my vision) and i am wondering which one i have dry eye or retinal detachment

I was diagnosed at 53 with a cataract in my right eye. However my eye doc thought I also had keratoconnous as well. The surgeon that was going to do my iol refused to because he couldn't diagnose where the light was actually focusing in my eye. I have however figured out my left eye is with the rule and my right eye where my cataract is oblique. Could these lenses correct keratoconnous enough that I could move forward with my iol placement?

So, I am new to contacts. I am using Purevision 2 monthly, I wear them about 12-13 hours straight, sometimes 15 hours. It doesn't dry out or cause an itch but is it safe for a prolonged period?

On the one hand, I am intrigued. On the other hand I have two different prescriptions I use; one for computers, another for driving. I change out my glasses a lot depending on what I'm doing.

Totally understandable! They do indeed make scleral lenses in multifocal lenses so they can help with all distances (although they are harder to get right). Otherwise people will use contact lenses for distance and then use reading glasses or computer glasses for near tasks.

I rarely wear mine because they irritate my eyes (surface gets dry) and they get cloudy so quickly 😭 We’ve tried different salines, using eyedrops before putting them in, different materials, I use cequa, and eyelid wipes. Nothing seems to work

I know they are such a godsend for a lot of people. I do agree that they are more comfortable than rgp’s. However, I seem to require the movement and tear exchange of rpg’s. A conclusion I came to after trying to wear sclerals past the 10-hour mark.

Doctor, can I ask, I have no issues with my eyes, clearly diet plays a huge factor in looking after your eyes… However I was curious, does using eye drops or any other eye product, say once a week or even once a month support keeping the eyes healthy? I have never used eye drops & have not experience red, dry or itchy eyes. But I just want to maintain my eye health to the maximum. Would it be completely pointless to use any eye product if I have no issues? Or can it help keep my eyes even healthier if I combine a healthy diet along with a eye drop routine more sporadically?

I'm worried that contact lessons are not good. I had cataracts and I asked the question, who had the most successful surgeries. I was very satisfied with him

My eye dr told me about these lenses, she know a dr who makes these but after researching these there are more negative than positive reviews, folks say it’s difficult putting them in and worse trying to get them off. Some say they can only tolerate them for 5-6 hours cause the drying of the eye is awful. For us retired folks $3000 for lenses having to be replaced in 1-2 years is not good. Hey thanks for asking. No, I have not really covered Monocular diplopia. I did do this video awhile ago: kzbin.info/www/bejne/npbbkGetdtpom5Ysi=_92qyxkVJn6GhgWT Most people confuse shadowing in their vision for diplopia. Shadowing is often caused by some form of refractive error such as uncorrected astigmatism or even an irregular tear film from dry eyes. If the diplopia can be corrected by glasses, then it is most likely a refractive cause. Other true causes of monocular diplopia include eye disease and conditions inside of the eye, such as scarring, a wrinkle inside the eye (ERM/macular pucker), dislocated lenses etc. I hope your local eye doctor can help you.

A lot of doctors deem these lenses not "worth it" to fit. Like Dr. Allen said, fitting these lenses is more time consuming, requires special equipment, and special training/experience. But some patients need these lenses so it's a good thing there are specialists who fit these.

I have terrible vision. I have astigmatism, near sighted in one eye, far in the other. A major issue is Ocular Albinism. Does the associated Nystagmus prevent a good fit? My corrected vision has never good, and at 61, my eyes are increasingly dry.

These lenses seem to be an answer to a severe dry eye victim's prayers....however, I also have severe MGD that causes my eyes to close and are hard to reopen. Would these contacts help with these issues?????

I’m not sure where to put my question, but I have red veins in my eyes - bad ones, that are cosmetically so distressful. I have had them for over two decades. As a female, it makes me so self conscious. I want the beautiful white sclera I once had. Nothing I have done has ever “uncongested” the veins. Is there any surgery that shows success? I do not have dry eye per se, just these prominent gross veins (one is particularly terrible, running right to my iris). I’ve been hoping my whole adult life to have this treated. Help?

I have Sjogren's syndrome and got scleral lenses after coming across another video you did. Fitting takes time. I am getting another right lense since this one is slightly off. I have no problem inserting or removing them. I am excited to find out how I will do. Thank you for this channel. Btw when you remove the lenses is the bowls supposed to be empty, since they are filled when I put the lenses in.

Your eye absorbs the liquid, especially with the dryness associated with Sjogren’s. I was told that I’d need to remove and refill the lenses at about midday for similar reasons.

@@canonrock4ever I have SjS for me it helped big time. I was going through almost a box a day in drops, and those are $$$. I find with the scleral lenses, I insert them in the morning, and I'm good to go. By the afternoon, I take them out, put in fresh saline, and I'm good to go again. They aren't a magical cure or anything like that. But they are 10x better than drops. Plus, I use Restatis as well.

I have a lot of eye issues, has several procedures for my detached retinas and now have severe dry eyes. I was fitted for scleral lenses but just can not get them to where they are comfortable to wear. Not once have I had them in and not just wanted to rip my eyeballs out 😔 I tried every songle method out there and can’t get them to work for me, no mstter how much I want them to. I thought there would finally be my joly grail for comfort but I give up. My eyes will always be in pain 😢

My concern is that my dry eye is the worst on the far outsides of the sclera, where the lenses wouldn't cover. The conjunctiva and the area underneath my lower eyelids also get a lot of saponification (soap-like feeling). Regular contacts usually feel decent and make my corneas feel better, but it's the far outsides of the whites of my eyes that still feel the worst, with or without contacts.

This sounds promising. I have severe dry eye and have tried almost everything. Today Im goin in for an "MGR." Really hoping that helps. I have a question about the scleral lenses. I can see how they would help moisturize the eye underneath the surface of the lens, but it doesn't cover the whole surface, and what about the mousture needed for blinking? Wouldnt that still be a problem?

I have an autoimmune condition leading to chronic peripheral keratitis, due to scleral(redness) and corneal(grey/yellowish boundary) inflammation. I have shadowy double vision right now, gradually worsening. Should I consider these lenses?

May depend on the cause of the keratitis, but scleral lenses may help with the double vision/shadow parts in the vision.

I want to try eye contacts but im not sure if to since i do landscaping and there's alot of debris flying like dust and pieces of grass. I have not gone to the doctor yet but i want to know if anything one reconds eye contacts for landscaping and just doing outside activities were you will be exposed to dust and debris of some sort? Or if i can even wear contacts at all?

People can wear reading glasses for near tasks but they also come in multifocal options too - although they are more difficult to fit. Also monovision could work.

I went for a Scleral Lenses consultation for my keratoconus this year. I was quoted a price of $4000 including fitting and was told I would need to have them replaced on a yearly basis.

At this point just go for ICL. In my area premium ICL clinics will cost you around 6-7k €. For both eyes!

Thank you for your continuing information :¬) - not the pleading :¬) have you ever covered "monocular diplopia"? i have this as my main issue but its more of a crisp 2 images and another 2 images of less intensity it can be corrected via my glasses but i still have to move the arms up and down to get the best image sorry if i shouldn't have pleaded for help hehe

Who are still 20/20? I am envious. I was diagnosed with narrow ankle and had a laser surgery to open the channel otherwise there will be more chances to have glaucoma, high eye pressure . Later I was told it did not open so a doctor had to shoot the laser again to my eye. Felt so weird. I will pay for any contact lenses. Very helpful.

hey thank you for that! Eye floaters and dry eye are not associated with each other, however both conditions are more common with age. Also, with bad dry eye, vision may fluctuate and even become blurry, and for some people the floaters may appear easier when their vision is out of focus.

If you have bad floaters, consider a floater-only vitrectomy. It’s a straightforward 15 minute operation and you’ll never have a floater again.

Never heard of them. I think they would be good for me because I have myopia pretty bad [-6.5] and sometimes by the end of the day one of my contacts will move and not sit right on one of my eyes and cause blurry vision out of one eye. About 2 years ago, my prescription went from -6 to -6.5. I recently read that this just crosses over to the severe range. I am only 37, should I be worried about this? My eye doctor didn't say anything about this being severe.

Usually, we are more worried about younger kids and young adults with advancing prescriptions. In adulthood, there can be changes to the prescription but it usually doesn't relate to an enlarging eyeball or larger axial length. But having a higher prescription does increase the odds of some eye conditions, such as cataracts, glaucoma, and retinal detachments.

I have about 70 % meibomain gland loss and 33-34 nm lipid layer thickness. I am thinking of getting scleral lense but I am concerned that Lipid layer thickness may be too low for scleral lenses. Should I go for it ?

@@simonhurst5705 I also bought acculens scleral lenses with hydrapeg coating but can't wear it for more than 3-4 hours. But I think it is fitting issue because it feels very tight. Do you also experience tightness while wearing scleral lenses?

I have heard of a few new treatments in the pipeline, even an oral medication...but they will still have to go through a lot of testing to get to market.

Any idea why I get extremely dry eyes at home and my eye doctor recommended refresh drops about 6x a day, eyelid scrubs and warm mask 2x a day....but when I was on vacation and traveling for two months, I didn't have dry eyes at all -- no refresh drops needed! So weird.

I have several patients who report the same thing. A common thought is that humidity levels may be playing a role. Also maybe sleep - poor sleep has been found to dramatically affect your dry eye quality.

Just get a pair of swim goggles. Put some eye drops in, then don the goggles. Space inside the goggles rapidly reaches 100%, keeping the eye surface moist. Been doing this for years. Definitely helps.

I like extended wear contact lenses because I hate taking them out every night. The one thing I didn't learn from this video is whether or not you can sleep in them!

Thanks for pointing that out. No, scleral lenses are usually not recommended to sleep in, but there are unique cases where doctors have prescribed these lenses to be slept in - but it's rarely done. Have you considered LASIK or other forms of refractive surgery instead of contact lenses?
