Some (a little) expansion on themes in other AE vids - Failure parties encourage sharing of the failure mode which may reduce re-occurrence and reduces latency in failure reporting. Also make sure incentives are aligned to the search for failure modes, the LfE. Mitigations, Costs and methods are aligned to 'success' in discovering and then addressing previously unknown failure modes
