An explosion rips a gaping hole in the cabin of a brand-new DC-10. Investigators discover a serious problem with the design of the plane’s rear cargo door, and recommend ways to fix it. Two years later, another DC-10 takes off on a routine flight - only to suffer the exact same problem, with deadly consequences. The accident could have - and should have - been prevented. But why wasn’t it?

Mayday does such a good job with these documentary's I found myself almost wishing there were more plane crashes, lol. Nooooooo

Only a pilot would announce that "We have a SMALL problem". I would never, ever, take off my shoes. Pretty hard to evacuate a plane that may end up on fire, or, dangerous wreckage to walk thru.

I can't imagine my family member being blown out of a plane in the casket and landing on the ground like that. Thank G-d for having his hand on that plane.

A familiar episode for me. The outward opening cargo door latches give out with higher plane altitude that causes higher interior air pressure.

Douglass luck ran out eventually they struck out to many accidents, the company died. Bowing came in like the thing and asord Douglas assets the only thing that remained of there revivals essence.

Here’s the worst part Douglass knew about the design flaw several years before and Windsor which my and extreme miracle every one survived and the fatal second crash in Paris. Douglass stupid prideful and gred put out performing there two rivals and maximizing the best finical stability. Over passenger safety. Passengers safety comes first always when designed such a massive heavy piece of technology costs be damed. You got to hold these big airplane feet to the proverbial moving buzz saw not fire to get them to cave in : even then some times it may be still to late the wounds our to severe for the company to survive.

America isn’t the only country in the world. But it is the only country that doesn’t use the metric system. Pretty sure this show is British.
